Plate Versus Nail Fixation in Periprosthetic Distal Femur Fractures: An Updated Systematic Review and Meta-Analysis.

BACKGROUND: Periprosthetic distal femur fractures (PDFFs) are increasing in frequency with the rise in primary total knee arthroplasty in an aging global population. Optimal surgical fixation remains controversial, with both plate and nail constructs widely utilized. PURPOSES: We sought to compare clinical and radiographic outcomes between plate and intramedullary nail fixation for PDFFs. METHODS: A systematic review and meta-analysis were conducted according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines comparing plate versus nail constructs for PDFFs. We

Helical Blade Versus Lag Screw Fixation in Cephalomedullary Nailing of Unstable Trochanteric Fractures: A Narrative Review.

Trochanteric (intertrochanteric) hip fractures are among the most common fragility fractures in older adults. Cephalomedullary nailing (CMN) has become the standard treatment strategy for unstable fracture patterns (AO/OTA 31-A2/A3), largely replacing the sliding hip screw (dynamic hip screw, DHS) for this indication. Two proximal fixation designs are in widespread use within CMN systems: the traditional lag screw, which relies on thread engagement in cancellous bone, and the helical blade, which is inserted by impaction, compacting surrounding cancellous bone rather than removing it. This nar

Outcomes of AO/OTA 43C Pilon Fractures Treated With Open Reduction Internal Fixation or Tibiotalocalcaneal Nailing.

BACKGROUND: AO/OTA 43C pilon fractures are severe intra-articular distal tibia injuries associated with high complication rates. Although open reduction internal fixation (ORIF) remains the standard treatment, tibiotalocalcaneal (TTC) nailing has been used in select patients; this study compared characteristics and outcomes of patients treated with ORIF or TTC nailing. METHODS: A retrospective cohort study was performed of adult patients (≥18 years) with AO/OTA 43C pilon fractures and ≥90 days of follow-up treated operatively at a single level 1 trauma center between 2020 and 2025. Patients t

A Biomechanical Analysis of Intramedullary Threaded Nail Fixation and Dorsal Plate and Screw Fixation for Metacarpal Base Fractures.

PURPOSE: The biomechanics of surgically treated metacarpal base fractures are poorly described. We hypothesized that intramedullary threaded nail (IMTN) fixation and dorsal plate and screw (DPS) fixation would perform similarly in multiple fracture locations approaching the metacarpal base. METHODS: Sixty-four metacarpals from eight matched pair cadaveric hands were stripped. Matched pairs were randomly divided into four cohorts based on fracture distance from the metacarpal base. Transverse osteotomies were created at 50%, 35%, 25%, and 15% of the total metacarpal length, measured from the m

Efficacy of Adjuvant Interferon-β Versus Observation for Resected Nail Apparatus Melanoma: A Multicenter Retrospective Study.

Nail apparatus melanoma (NAM) is a biologically and clinically distinct subtype of melanoma; however, the efficacy of adjuvant interferon (IFN) therapy in this population remains unclear. We conducted a multicenter retrospective study to compare adjuvant IFN-β (adj-IFN) with observation (OBS) in patients with resected stage IIB, IIC, or III NAM. We analyzed 282 Japanese patients treated at 42 institutions between 2014 and 2024. Recurrence-free survival (RFS) and distant metastasis-free survival (DMFS) were co-primary outcomes, and overall survival (OS) was a secondary outcome. Survival outcome

Refractory nail lichen planus treated successfully with the Janus kinase 1 inhibitor abrocitinib: A case report.

Nail lichen planus is a chronic inflammatory condition that may be isolated to the nail, resulting in longitudinal nail ridging, trachyonychia, and splinter hemorrhages, and may be highly refractory to conventional treatment. The management of nail lichen planus remains challenging because of the lack of standardized treatment guidelines, and the efficacy of existing therapies, both topical and systemic, is often inconsistent or unsatisfactory. Emerging reports have suggested that abrocitinib, a selective Janus kinase 1 inhibitor, may be safe and effective in treating severe nail lichen planus

Late Complication From Dislodgement of the Expert Tibial Nailing System Internal Locking Sleeve: A Case Report.

CASE: We report on a patient who underwent intramedullary nailing with the Expert Tibial Nailing System. Unappreciated intraoperative dislodgement of the distal internal polymer at the end of the nail occurred. This resulted in entrapment into the original fracture site and contributed to late infection. Treatment consisted of aggressive local debridement, foreign body removal, intramedullary nail removal, reaming, and antibiotics. The patient had an excellent clinical result 2 years postinjury. CONCLUSION: The plastic polymer sleeve of a tibial nail can become dislodged and trapped inside th

Gait changes after intramedullary nailing versus total hip arthroplasty for hip fractures in older adults.

This study aimed to investigate potential differences in spatiotemporal gait parameters and clinical outcomes between older adults undergoing intramedullary nailing (IN) and those undergoing total hip arthroplasty (THA) for unilateral hip fractures. A secondary objective was to identify predictors of postoperative falls in older adults following surgical treatment for hip fractures. We conducted a prospective study involving 42 community-dwelling older adults, assessed 6 months post-surgery. Of these participants, 21 (14 females, 7 males; mean age 76.0 ± 8.6 years) underwent IN, while the rema

Treatment of Pathologic Proximal Femur Fractures Using the Improvised Megaprosthesis: Combination of the Hip Prosthesis and Intramedullary Nail.

INTRODUCTION: The proximal femur is the most common long bone affected by metastatic disease. Pathologic fractures in this area are frequent, secondary to weight-bearing and deforming forces. Long-stem endoprosthetic replacement is often used to replace and bypass segments affected by metastases. However, implant cost remains prohibitive for patients in low-resource settings. An improvised megaprosthesis using a hip implant combined with Kuntscher nail provides an economic option. MATERIAL AND METHODS: This is a case series of three patients diagnosed with pathologic fracture of the hip secon

Minimally invasive fixation of a peri-implant femoral neck fracture without nail removal: A case report.

We present the case of a 53-year-old female patient with a peri-implant transcervical femoral fracture associated with an intramedullary nail (IMN) implanted over 10 years ago following a low-energy fall from standing height. The preexisting implant constrained traditional surgical options due to the risks associated with nail removal, such as iatrogenic fractures and soft tissue damage. Closed reduction assisted by a traction table was performed, followed by internal fixation with cannulated screws in an inverted triangular configuration, preserving the preexisting implant and avoiding surgic

Open reduction and internal fixation compared to intramedullary nail fixation in distal fibula fractures: a meta-analysis.

The operative treatment of distal fibula fractures is debated. This study compared intramedullary nailing (IMN) versus open reduction and internal fixation (ORIF) in patients with distal fibula fractures. The outcomes of interest were patient-reported outcome measures (PROMs) and the rate of anatomic reduction, bony union, complication, and nerve injury. This study was conducted according to the 2020 PRISMA statement. In October 2024, the following databases were accessed: PubMed, Web of Science, Google Scholar, and Embase, with no time constraint. All the clinical studies comparing IMN versus
