Recently, the number of case reports detailing cancer recurrence in the pancreatic remnants, following surgical resection of intraductal papillary-mucinous carcinoma (IPMC) of the pancreas has increased. Solitary extramedullary plasmacytoma (EMP) is a rare plasma cell disorder mostly involving the upper airway; however, retroperitoneal infiltration is very rare. Kidney injury associated with EMP is exceptionally rare with only anecdotal reports. Herein we report a case of retroperitoneal EMP causing renal failure by the way of direct renal parenchymal infiltration. Renal parenchymal invasion should be considered in aggressive and refractory plasma cell dyscrasias with unexplained renal failure.

BACKGROUND: Invasive micropapillary carcinoma (IMPC) is defined as having characteristic pathologic features of small papillary cell clusters surrounded by lacunar spaces and is known as an aggressive variant in advanced stages due to the high incidence of lymph node metastasis. IMPC has been well described in other organs, including the breast, urinary bladder and lung but has been rarely described in the large intestine. To our knowledge, this is the first report of the cytology of colorectal lesion of IMPC in the English-language literature.
