The successful management of postoperative pain in children requires a biopsychosocial approach. Situational anxiety and anxiety disorders among caregivers influence a child's perioperative experience. Tendon pain after repair of an acute Achilles tendon rupture can result from suture granuloma formation, modification of the threshold of the pain receptors inside the tendon by scar tissue, expansion of the paratenon by tendon enlargement with secondary stimulation of mechanoreceptors, or underlying tendon degeneration. In the present technique report, an endoscopic technique of Achilles tenolysis for denervation and debulking is described that might be applicable in cases in which conservative treatment fails to alleviate the pain.

AIM: To study the clinical effects of a disposable circumcision device in treatment of male patients of different ages with either phimosis or excess foreskin.
METHODS: One thousand two hundred patients between the age of 5 and 95 years underwent circumcision using this procedure in the 2-year period between October 2005 and September 2007. Of these cases, 904 had excess foreskin and 296 were cases of phimosis.

PURPOSE: We present our initial experience with laparoscopic renal denervation and nephropexy for ADPKD related pain in the adolescent population.
MATERIALS AND METHODS: Four patients 15 to 19 years old previously diagnosed with ADPKD presented with chronic pain refractory to narcotic analgesics. These 4 patients underwent laparoscopic renal denervation of 5 kidneys.
RESULTS: Mean operative time was 136 minutes and mean hospital stay was 2.75 days. All patients were pain-free at discharge home and remained pain-free at a mean followup of 11.5 months.

A case of secondary subarachnoidal dislocation of a lumbar epidural catheter is reported. A 76-year-old female underwent resection of the sigmoid colon. In order to provide postoperative analgesia, an epidural catheter was inserted between the 3rd and 4th lumbar interspaces prior to induction of anaesthesia. This study sought to discover whether day-surgery patients found it necessary to take analgesics to relieve the pain and discomfort experienced after discharge following gynaecological laparoscopic procedures. Pre-operative anxiety was also assessed as a possible contributory factor to post-operative nausea and headaches.
