The Muller maneuver (MM) collapses the upper airway and mimics apneic events during sleep. This study aimed to assess the effect of MM on the upper airway (UA) and surrounding structures of patients with OSA using cone-beam computed tomography (CBCT). This prospective study of 18 moderate-to-severe OSA patients included two CBCT scans, one during gentle breathing and another while performing MM, with standardized head and neck positioning. UA, soft tissue, and hyoid bone were analyzed using linear, area, and volumetric measurements with OnDemand 3D software version 10.0.1 (1008 measurements).

PURPOSE: While continuous positive airway pressure (CPAP) reduces cardiovascular risk in obstructive sleep apnea (OSA), its effects on renin–angiotensin–aldosterone system (RAAS) modulation remain controversial. This meta-analysis investigates CPAP's differential impacts on RAAS components and identifies responsive patient subgroups. METHODS: Sixteen studies were included in the systematic review and eight (231 patients) in the meta-analysis. Primary outcomes included plasma aldosterone concentration (PAC), plasma renin activity (PRA), and angiotensin II (AngII) changes. Subgroup analyses exam

PURPOSE: Obstructive sleep apnea (OSA) contributes to non-alcoholic fatty liver disease (NAFLD) via pathways involving insulin resistance (IR). The triglyceride-glucose (TyG) index, a widely used marker of IR, is associated with both OSA and NAFLD. However, the role of the TyG index in linking OSA to NAFLD remains underexplored.
PATIENTS AND METHODS: This study analyzed data from two cross-sectional studies: 920 participants from the Fifth Affiliated Hospital of Sun Yat-sen University and 1,603 participants from NHANES 2005-2008. Mediation analysis assessed the TyG index's role in the OSA-NAF

Obstructive sleep apnea (OSA) in children is linked to cognitive impairments, potentially due to blood-brain barrier (BBB) dysfunction. Exosomes, small vesicles released by most cells, reflect cellular changes. This study examined the effects of exosomes from children with OSA, with or without cognitive deficits, on neurovascular unit (NVU) models. Twenty-six children were categorized into three groups: healthy controls (Cont, n = 6), OSA without cognitive deficits (OSA-NG, n = 10), and OSA with neurocognitive deficits (OSA-POS, n = 10). Plasma exosomes were characterized and applied to human

BACKGROUND: Positive airway pressure (PAP) therapy is the primary treatment for obstructive sleep apnea (OSA), but long-term adherence remains challenging. This study aimed to identify key predictors of patient satisfaction with PAP therapy by examining the interrelationships between disease knowledge, care provision, health beliefs, and patient-reported experiences.
METHODS: A cross-sectional study was conducted at a tertiary sleep center in Germany. Adult patients with confirmed OSA diagnosis and experience with PAP therapy completed a comprehensive questionnaire assessing four domains: hea

PURPOSE: Obstructive sleep apnea (OSA) is common in type 2 diabetes (T2D). This study examined the prevalence of undiagnosed moderate-to-severe OSA (apnea-hypopnea index ≥ 15) in T2D patients, and assessed the diagnostic performance of the Berlin Questionnaire (BerlinQ) and Epworth Sleepiness Scale (ESS) compared with cardio-respiratory monitoring (CRM). We hypothesized that BerlinQ and ESS would have sufficient sensitivity to identify patients with treatment-recommended OSA, thereby supporting their use as triage tools to select patients for CRM.
METHODS: This cross-sectional study recruited

BACKGROUND: Simple and affordable methods for evaluating Insulin Resistance (IR) have been suggested, such as the Triglyceride-Glucose (TyG) index and its variants, including the TyG-Body Mass Index (TyG-BMI), TyG-Waist Circumference (TyG-WC), and TyG-Waist-to-Height Ratio (TyG-WHtR). The aim of this study is to investigate the relationship between these TyG-related indices, which measure IR, and Obstructive Sleep Apnea (OSA).
METHODS: This study analyzed NHANES data from 2007-2008, 2015-2016, and 2017-2020. TyG and its derivatives were evaluated as continuous and categorical variables in rel

BACKGROUND/OBJECTIVES: Pediatric obstructive sleep apnea (OSA) is a prevalent medical condition, affecting 1-5% of non-syndromic children and 30-90% of children with Down syndrome. Given the severity of the condition and the associated health risks, early and effective treatment is crucial. However, current treatment modalities are often invasive or suffer from poor patient adherence. Additionally, adenotonsillectomy, the first-line treatment in pediatric OSA, seems not to be effective in every child, leaving children with residual OSA postoperatively. These challenges are particularly pronoun

INTRODUCTION: Obstructive Sleep Apnea (OSA) is a prevalent condition affecting millions worldwide and is associated with an increased risk of cardiovascular complications, including ST-Elevation Myocardial Infarction (STEMI). The relationship between OSA and STEMI is complex, with OSA potentially exacerbating the severity of coronary artery disease and influencing outcomes following acute coronary events.
METHODS: We retrospectively analyzed data from the National Inpatient Sample database from 2016 to 2021. Hospitalized patients aged 18 and older diagnosed with STEMI were included. Key outco

Background/Objectives: Hypoglossal nerve stimulation (HNS) emerged as an alternative treatment for patients with obstructive sleep apnea (OSA) a decade ago. Long-term clinical trials and real-world data show that HNS treatment provides significant and sustained improvements in both OSA disease control and quality-of-life measures over time. Given the nature of HNS treatment, with the requirement of using an implantable neurostimulation system, patient safety is a critical domain in the assessment of this technology. The objective of this review was to evaluate adverse events (AEs) and complica

Obesity is the primary risk factor for the development of obstructive sleep apnea, and physical inactivity plays an important role. However, most studies have either only evaluated physical activity subjectively or objectively in obstructive sleep apnea. The objectives of this study were: (i) to assess the relationship between obstructive sleep apnea severity (both apnea-hypopnea index and desaturation parameters) and both objectively and subjectively measured physical activity after adjustment for anthropometry and body composition parameters; and (ii) to assess the relationship between objec

In recent decades, it has become clear that the gut is more than just a digestive organ; it also functions as an immune organ with regulatory capabilities and acts as a "second brain" that influences brain function due to the presence and regulatory roles of the gut microbiota (GM). The GM is a crucial component of its host and significantly impacts human health. Dysbiosis, or microbial imbalance, has been closely linked to various diseases, including gastrointestinal, neurological, psychiatric, and metabolic disorders. The aim of this narrative review is to highlight the roles of the GM in ma

IMPORTANCE: It is unknown whether children with primary snoring and children with mild obstructive sleep apnea (OSA) represent populations with substantially different clinical characteristics. Nonetheless, an obstructive apnea-hypopnea index (AHI) of 1 or greater is often used to define OSA and plan for adenotonsillectomy (AT).
OBJECTIVE: To assess whether a combination of clinical characteristics differentiates children with primary snoring from children with mild OSA.
