Double-blind placebo-controlled study of cisapride in patients with nonspecific esophageal motility disorder accompanied by delayed esophageal transit.

BACKGROUND: Nonspecific esophageal motility disorder (NEMD) represents a difficult therapeutic challenge because of the heterogeneous nature of the esophageal motor functions. We studied the effects of cisapride on the esophageal symptoms and esophageal motor function in a group of patients with NEMD showing delayed esophageal transit. METHODS: Seventy eligible patients were entered into a 4-week, double-blind randomized comparison of 10 mg of cisapride or placebo, four times daily. Symptom assessment, esophageal manometry after wet swallows, and esophageal scintigraphy after intake of a liqu…

Novel technique using tensor fascia lata graft to reconstruct a floor of mouth postablative defect from invasive ectopic papillary carcinoma of the thyroglossal duct tract.

We describe a rare case of ectopic papillary thyroid cancer in the thyroglossal duct tract invading the floor-of-mouth musculature. The postablative defect was reconstructed with a bone-anchored tensor fascia lata graft to resuspend the floor of mouth to the mandible as a neogeniohyoid sling, enabling maintenance of a functional tongue position for normal speech and swallowing. This reconstruction should be considered when suprahyoid musculature is resected without breaching the oral lining.

[Tonsillotomy].

In tonsillotomy, only the obstructive tonsillar tissue is removed without touching the surrounding tissue of the palatine tonsil. Tonsillotomies have been mainly performed for children due to tonsillar hypertrophy. Less postoperative pain is recorded after tonsillotomy compared with tonsillectomy. In addition, recovery is quicker and there are fewer complications. Tonsillotomy is an effective method in cases where the indication for surgery is tonsillar hypertrophy and associated symptoms such as snoring or difficulties in swallowing.

Special groups: head and neck cancer.

Head and neck cancer is a unique cause of dysphagia. Altered swallow function can be secondary to the mechanical effects of a tumor invading normal anatomy needed for deglutition, or as a direct sequela of cancer treatment (surgery and/or radiation ± chemotherapy). This article outlines the incidence of head and neck cancer, effects of anatomic changes associated with common surgical intervention, and the consequences of treatment-induced dysphagia. Assessment and rehabilitation techniques applicable for this population are discussed.

Airway obstruction from accidental ingestion of a live fish.

We present a case of a fisherman who swallowed a live fish, which resulted in severe upper airway obstruction. The firm attachments of the fish's external fins and scales to the airway limit the removal when impacted. We outline our emergency airway management strategies and focus our discussion on the technique used to remove the impacted fish from the upper airway, which was paramount for the successful outcome of this case.

Prosthetic rehabilitation of a preschooler with induced anodontia - A clinical report.

A case of prosthetic rehabilitation with complete dentures for a three year nine months old child is presented. Dental prosthesis are frequently used to avoid psychologic, speech, or swallowing problems in preschooler children. The case was followed up for a period of three years. These prosthesis were modified as the child grew and as the permanent teeth erupted into the oral cavity the dentures were trimmed from the areas of erupting teeth to facilitate their eruption.

Prolonged foreign body impaction in the oesophagus.

It is not unusual for a foreign body to be swallowed and become lodged in the oesophagus. It is, however, very unusual for such a foreign body to remain lodged for a period of six months. This particular case, a 37-year-old man, is interesting because of the length of time the foreign body, a denture, remained in the oesophagus without complications, its successful removal and the nature of the foreign body, which is prone to cause complications on prolonged stay or during removal.

The development of dysphagia and dysphonia due to anterior cervical osteophytes.

Cervical osteophytes may cause dysphagia by compressing the esophagus and may cause dysphonia by compressing the larynx and inferior laryngeal nerve. The occurrence of dysphagia and dysphonia due to cervical osteophytes has rarely been reported in literature. In this article, a case, in which the multiple cervical osteophytes were found to be the cause of dysphagia and dysphonia, was evaluated by imaging methods and electrophysiological evaluation of swallowing and the case was discussed in the light of relevant literatures.

[Intestinal disorders].

Bed ridden patients commonly suffer a series of intestinal disorders which include: loss of appetite, swallowing disorders, a tendency to undergo gastric tract reflux, slowness in intestinal movements, accumulation of gas or fecal impactation. These contribute to an increased risk of potentially dangerous complications such as intestinal obstruction which can lead to necrosis or a perforation of different parts of the digestive tract, duodenal ulcer, hemorrhoids, or anal fissures.

Proving possession of drugs in so-called body stuffers.

In 2001 a new policy for dealing with drug traffickers was introduced in Hamburg. Any person suspected of having swallowed drugs in order to hide the evidence is taken to the Institute of Legal Medicine where the drugs will be recovered by pharmacologically induced vomiting. This method has proved to be safe and successful as drugs were recovered in nearly two-thirds of the cases. The offenders were predominantly male Africans 16-25 years old.

[Ankyloglossia in an Anatolian Shepherd dog].

Ankyloglossia, commonknown as tongue-tie, is a rare congenital oral anomaly in dogs. A complete attachment of the lingual frenulum to the floor of the oral cavity leads to limited mobility of the tongue including problems during eating and swallowing. In humans ankyloglossia is a common anomaly in newborn infants. In our report a 5-month old Anatolian Shepherd dog was surgically treated and full function of the tongue could be achieved with a horizontal-to-vertical frenuloplasty.

Kyphosis, a rare cause of dysphagia.

Degenerative kyphosis is mostly asymptomatic. When symptomatic, pain and humping of the back are the commonest features. Dysphagia associated with kyphosis is a rare presentation, though it is otherwise common in older people due to various reasons. We report the case of an 81-year-old kyphotic woman who presented with dysphagia. On barium swallow examination, she was found to have an acute bend in the mid-oesophagus leading to narrowing, secondary to the kyphosis.

Pneumopericardium due to intrapericardial perforation of a gastric ulcer.

A 88-year-old male patient presented with fever, singultus and retrosternal pain. After 8 days of antibiotic therapy not resulting in clinical improvement, he suddenly developed a pneumopericardium. Contrast swallow and endoscopy showed intrapericardial perforation of a benign gastric ulcer. Excision of the ulcer and suturing of both the stomach and the diaphragm as well as lavage of the pericardium were done over a left thoracotomy. The patient recovered uneventfully.

Computerized tomography of the glottis after intracordal autologous fat injection.

According to the committee on speech, voice, and swallowing disorders of the American Academy of Otolaryngology-Head and Neck Surgery, various surgical methods such as laryngeal framework surgery, laryngeal re-innervation, and injection laryngoplasty might be used to palliate inferior laryngeal nerve paralysis. In the present case report we document the survival and exact location of the boluses of autologous fat in one patient in whom this material was used for injection laryngoplasty.

Adult hypertrophic pyloric stenosis: case report and review.

A case of adult hypertrophic pyloric stenosis presented in an older female with no apparent predisposing factors which might be likely to precipitate the condition. There was a history of chronic dyspepsia and occasional episodes of vomiting. No contributory medical history was identified. Barium swallow and follow through showed a markedly reduced emptying time of the stomach. A partial gastrectomy was eventually done and on histopathological examination a diagnosis of adult hypertrophic pyloric stenosis was made.

A thyroglossal duct node as presenting symptom of Hashimoto's thyroiditis: a report of two cases.

Two patients with thyroglossal duct nodes are described. Both patients were suffering from autoimmune thyroiditis. The tumours resembling thyroglossal duct cysts presented as palpable nodes in the midline of the neck and moved upwards on swallowing. Both patients had normal serum thyroxine levels and slightly elevated TSH. To avoid unnecessary surgery, a diagnostic work-up with echography and fine needle aspiration cytology of the tumour is recommended.

Epistaxis as a cause of hematemesis and melena.

Epistaxis was diagnosed in 10 patients with apparent upper gastrointestinal bleeding, comprising a 0.55% incidence of hematemesis and melena in the population studied. A sufficient amount of blood can be swallowed during epistaxis to cause hematemesis and melena. Recent facial trauma or epistaxis, absence of a history of chronic dyspepsia, and impairment of blood coagulation emerge as strong indicators of the diagnosis and should lead to a careful examination of the nose and nasopharynx for the source of bleeding.

Engineering visual realism in pureed foods for dysphagia care.

While texture modification ensures safe swallowing in dysphagia, homogeneous purees often compromise visual appeal. Treating shaping as the core design objective, this review evaluates established methods for engineering visual realism in pureed foods against available evidence, while explicitly identifying areas where evidence does not yet exist. Direct shaping methods are evaluated across IDDSI (International Dysphagia Diet Standardization Initiative) compliance, sensory acceptability, and practical implementation. Structural approaches that modify food structure rather than reshape purees a

Interfacial thickness and roughness of sesame oil bodies by pH regulation for 3D printed dysphagia foods: Texture, rheology, and oral tribology.

For individuals with dysphagia, oral processing and easy swallowing characteristics are crucial. This study aimed to explore the mechanism by which pH regulates the interface membrane thickness and roughness of sesame oil bodies (SOBs) and its effects on the physicochemical properties, rheological behavior, textural characteristics, 3D printability, and applicability in dysphagia-friendly foods. SOBs were treated with four pH levels (5.5, 6.0, 6.5, and 7.0), and systematic characterizations were conducted. Results showed that pH significantly modified the composition of interface proteins in S
