Review of the 94th Annual Meeting of the British Association of Dermatologists, Glasgow 2014.

This is a review of the 94th Annual Meeting of the British Association of Dermatologists, held in Glasgow from 1 to 3 July 2014. The conference covered some of the latest developments in the treatment of atopic dermatitis, psoriasis and cancer, a follow-up on the methylisothiazolinone contact allergy epidemic, advances in genetically inherited disorders and somatic mutations underlying birth marks. In addition, there was an international perspective on vitiligo, leprosy and HIV, and a session discussing the regulatory process behind pharmaceutical development.

The mast cell in health and disease.

The presence of mast cells, often in an activated state or increased numbers, has been noted in various cutaneous disorders. Recent studies suggest that mast cells are of primary importance in these conditions and their presence does not merely represent a secondary event. This review focuses on atopic dermatitis, contact hypersensitivity reactions, bullous pemphigoid, fibrosing conditions, neurofibromatosis, wound healing, polycythemia vera, and psoriasis, in which there is significant evidence to indicate a pathophysiologic role for mast cells.

An observational study on the effects of aviation turbine fuel and lubricants on the skin of Indian Air Force ground crew in flying stations.

BACKGROUND: Ground crew of the Air Force routinely handle aviation turbine fuel (ATF) and lubricants and a need was felt to study the adverse effects of these substances on their skin so that remedial measures could be taken to prevent these adverse effects. METHODS: A multi-centric, cross-sectional, observational study was performed at three Air Force flying stations. 109 ground crew members of the Indian Air Force (IAF) who were in direct contact with ATF and lubricants were screened for dermatological manifestations on exposed areas. History of atopy was elicited, systemic and dermatologic

Periorbital (eyelid) dermatides.

Physicians in various specialties-and dermatologists in particular-frequently encounter various forms of inflammation of the eyelids and of the anterior surface of the eye. Distinguishing the cause of itchy, painful, red, edematous eyelids is often difficult. Because the uppermost layer of the eyelids is part of the skin that wraps the entire body, almost every skin disease in the textbook can affect the periorbital area as well. In this contribution, we focused on the most common such disorders that require special consideration, as a result of their special appearance, their challenging diag

Are adverse skin reactions to cosmetics underestimated in the clinical assessment of contact dermatitis? A prospective study among 1075 patients attending Swedish patch test clinics.

It is known that cosmetics and skin care products can cause adverse skin reactions. However, the frequency of adverse reactions reported to the Medical Product Agency (MPA) in Sweden is low. The purpose of the present study was to evaluate the occurrence of adverse skin reactions to cosmetics among patients referred for standard patch testing owing to suspected contact dermatitis in general, most frequently hand eczema. Consecutive patients at four patch test clinics in Sweden were invited to participate; 1075 were included. Of these, 47.3% (54.2% women and 30.8% men) reported current or previ

Therapeutic effects of antibacterial treatment for intractable skin diseases in Helicobacter pylori-positive Japanese patients.

In order to understand the pathogenic relationship between Helicobacter pylori (H. pylori) and skin diseases, we examined the serum levels of IgG antibody against H. pylori and then performed gastroscopic examinations in Japanese patients with chronic skin diseases. These H. prylori-positive patients were treated with antibacterial eradication therapy, and therapeutic efficacy was evaluated. A total of 198 patients who were resistant to conventional therapies were randomly selected. They included 50 cases with chronic urticaria, 32 with pruritus cutaneous, 74 with atopic dermatitis, 15 with nu

The Association Between Oral Health and Skin Disease.

OBJECTIVE: Oral health and mucocutaneous inflammation might play an important role in the etiopathogenesis of many skin diseases, especially those that also involve the oral mucosa. This review examines the relationship between skin conditions and various oral health metrics to better understand how oral diseases, especially periodontitis, might influence the development or prognosis of several conditions, including aphthous stomatitis, atopic dermatitis, lichen planus, pemphigus, pemphigoid, and psoriasis. METHODS: Using the PubMed search engine between Summer 2017 and Summer 2018, searches w

Irritant contact dermatitis: beyond a diagnosis of exclusion.

Irritant contact dermatitis (ICD) is the most common form of contact dermatitis and a leading cause of occupational skin disease, but remains underrecognized relative to allergic contact dermatitis (ACD). Direct chemical or physical injury to the epidermal barrier causes ICD, which can develop in anyone when exposure intensity or duration exceeds the skin's repair capacity. ICD is increasingly understood to involve more than barrier disruption: it is an innate immune-mediated inflammatory disorder involving keratinocyte-derived cytokines, neutrophil recruitment, C-C motif chemokine ligand 2-C-

Epidemiology and Comorbidity of Vitiligo in Germany: A Claims Data Analysis.

Vitiligo is a chronic skin disease resulting in destruction of melanocytes and loss of pigmentation. Little is known about epidemiology and comorbidity in Germany. This retrospective cohort analysis used nationwide statutory health insurance data (2016 to 2020). Disease rates were calculated using varying case definitions. Comorbidity was assessed against 3 control groups: persons without vitiligo, with atopic dermatitis, and with psoriasis (1:3 propensity score matching). In 2020, 4,631 persons were diagnosed with vitiligo in DAK-Gesundheit (DAK-G, mean age 56.93 years, ~148,437) and 5,820 pe

Proanthocyanidins as Therapeutic Agents in Inflammation-Related Skin Disorders.

Skin diseases, affecting one-third of the population, are a growing global health problem. The complexity of skin architecture, along with diverse symptomatology and intricate pathogenesis of dermatological disorders, highlights the urgent need for novel therapeutic strategies. Effective treatment of impaired wound healing and chronic skin diseases, including atopic dermatitis and psoriasis, remains challenging. Phytoterapeutics are increasingly investigated for their dermatologic potential, with numerous natural products of established use. Proanthocyanidins (PACs), a subclass of polyphenolic

Factors Associated With Irritant Reactions on Epicutaneous Patch Testing.

BACKGROUND: Allergic and irritant reactions on patch testing can be difficult to differentiate clinically. We aim to identify factors associated with irritant reactions and correlate skin barrier function, measured in the form of transepidermal water loss (TEWL). METHODS: Consecutive patients undergoing patch testing at a tertiary hospital dermatology department were included. Primary demographic data were collected along with the results of patch testing and clinical confidence scores. TEWL was measured on the volar forearm using a Tewameter TM Hex skin probe at the Day 2 reading. RESULTS:

Skin neurogenic inflammation.

The epidermis closely interacts with nerve endings, and both epidermis and nerves produce substances for mutual sustenance. Neuropeptides, like substance P (SP) and calcitonin gene-related protein (CGRP), are produced by sensory nerves in the dermis; they induce mast cells to release vasoactive amines that facilitate infiltration of neutrophils and T cells. Some receptors are more important than others in the generation of itch. The Mas-related G protein-coupled receptors (Mrgpr) family as well as transient receptor potential ankyrin 1 (TRPA1) and protease activated receptor 2(Par2) have impor

Nerve growth factor partially recovers inflamed skin from stress-induced worsening in allergic inflammation.

Neuroimmune dysregulation characterizes atopic disease, but its nature and clinical impact remain ill-defined. Induced by stress, the neurotrophin nerve growth factor (NGF) may worsen cutaneous inflammation. We therefore studied the role of NGF in the cutaneous stress response in a mouse model for atopic dermatitis-like allergic dermatitis (AlD). Combining several methods, we found that stress increased cutaneous but not serum or hypothalamic NGF in telogen mice. Microarray analysis showed increased mRNAs of inflammatory and growth factors associated with NGF in the skin. In stress-worsened Al

Presence of circulating CCR10+ T cells and elevated serum CTACK/CCL27 in the early stage of mycosis fungoides.

PURPOSE: Mycosis fungoides (MF), a common type of cutaneous T cell lymphoma with an indolent clinical course, has the characteristic that malignant T cell clones are recruited into the skin from the early disease stages. The mechanisms of recruitment have been suggested from our knowledge of various chemokine-chemokine receptor interactions. Recently, CCR10 and CTACK/CCL27 were proposed to play a role in the recruitment of other types of cutaneous T cell lymphoma. We examined the expression of CCR10 in peripheral blood and serum CTACK/CCL27 levels in patients with MF. EXPERIMENTAL DESIGN: Eig

Impaired antibody responses in the hyperimmunoglobulin E syndrome.

Patients with the hyper-IgE (HIE) syndrome have recurrent bacterial infections with Staphylococcus aureus and other polysaccharide encapsulated organisms. To determine whether an impairment of the antibody response to polysaccharide antigens contributes to infections in this syndrome, we measured serum antibody to the teichoic acid of S. aureus and to the capsular polysaccharide of Haemophilus influenzae type b. Compared to control subjects who had no history of S. aureus infections (N = 14), sera from patients with HIE (N = 9) lacked the expected elevation of serum antibody to teichoic acid (
