Empagliflozin for the treatment of type 2 diabetes mellitus: An overview of safety and efficacy based on Phase 3 trials.

In the treatment of type 2 diabetes mellitus (T2DM), a relatively new class of oral agents inhibits sodium-glucose cotransporter 2 (SGLT2), reducing reabsorption of filtered glucose and increasing urinary glucose excretion. Numerous SGLT2 inhibitors have been approved for the treatment of T2DM in adults, most recently empagliflozin, which was approved in Europe and the US in 2014. The Phase 3 program has enrolled >14,000 patients and has assessed the efficacy and safety of empagliflozin as monotherapy and in combination. These studies have demonstrated improvements in glycemic control, and mod

[Changes of the fat/water ratio in the erector trunci muscle in patients with lumbar disc herniation: a comparative study with 1H-MRS].

PURPOSE: To evaluate whether metabolic changes of the erector trunci muscle in patients with lumbar disc herniation can be detected with proton magnetic resonance spectroscopy ( (1)H-MRS). MATERIALS AND METHODS: In 10 patients with lumbar disc herniation and 16 healthy volunteers, proton spectra were obtained from the erector trunci muscle and analyzed for fat-water ratio. The axial images were evaluated for the degree of atrophy of the erector trunci muscle. The muscular tissue was histopathologically examined in 2 patients. RESULTS: The examination was well tolerated by all patients and vo

Distraction manipulation reduction of an L5-S1 disk herniation.

OBJECTIVE: A computed tomography (CT)-confirmed L5-S1 disk protrusion is reported to be reduced following chiropractic adjustment, as seen on repeat CT scanning. Correlation of the CT reports with the patient's symptoms before and after manipulation is reported. CLINICAL FEATURES: A 38-yr-old female was treated for low back pain and right lower extremity first sacral dermatome sciatica. CT confirmed disk herniations at both the L4-L5 and L5-S1 levels were found. Motor weakness of the right gluteus maximus muscle was found and extremely tight hamstring muscles accompanying positive straight le

Improvement of Progressive Vanishing Lung Syndrome in COPD: A 7-Year Radiological Evolution Case Report and Literature Review.

INTRODUCTION: Vanishing lung syndrome (VLS) is a clinical condition that characterized by large bullae that compress the normal lung parenchyma. Longitudinal documentation of VLS disease progression and outcomes following endobronchial valve (EBV) treatment remains scarce, particularly for right middle lobe (RML) involvement. CASE PRESENTATION: We report a 7-year longterm follow-up case of a 73-year-old female with chronic obstructive pulmonary disease(COPD) who presented with progressive dyspnea and declining pulmonary function. Serial computed tomography scans over 7 years documented the gr

A protocol for the rehabilitation of patients undergoing mechanical ventilation from the ICU to the post-hospital phase: the MOVE-ICUtoHOME, a multicentre randomised controlled trial.

INTRODUCTION: Intensive care unit-acquired weakness (ICU-AW) in patients with acute respiratory failure requiring invasive mechanical ventilation (IMV) is associated with reduced functional exercise capacity and increased mortality. Early mobilisation in the ICU may improve functional outcomes and reduce length of stay; however, evidence remains limited, protocols are heterogeneous and post-discharge rehabilitation is frequently lacking. This study aims to evaluate the effects of a continuous care programme combining early intensive ICU mobilisation with post-hospitalisation rehabilitation on

Soluble guanylate cyclase stimulator vericiguat alleviates skeletal muscle injury and exercise capacity decline in ApoE-/- mice by inhibiting oxidative stress and repairing mitophagy.

Patients with atherosclerosis suffer from exercise capacity decline and skeletal muscle injury. Soluble guanylate cyclase stimulator vericiguat plays a protective role in the blood vessels and kidneys in addition to treating heart failure, but its effect on skeletal muscles remains unclear. This study aimed to investigated whether vericiguat can improve exercise capacity and mitigate skeletal muscle injury of atherosclerotic ApoE-/- mice. Vericiguat dose-dependently increased the forelimb grip strength, hanging impulse, and exhaustive running time, and reduced the levels of creatine kinase and

Ferric carboxymaltose and exercise capacity in heart failure with preserved ejection fraction and iron deficiency: the FAIR-HFpEF trial.

BACKGROUND AND AIMS: Evidence is lacking that correcting iron deficiency (ID) has clinically important benefits for patients with heart failure with preserved ejection fraction (HFpEF). METHODS: FAIR-HFpEF was a multicentre, randomized, double-blind trial designed to compare intravenous ferric carboxymaltose (FCM) with placebo (saline) in 200 patients with symptomatic HFpEF and ID (serum ferritin < 100 ng/mL or ferritin 100-299 ng/mL with transferrin saturation < 20%). The primary endpoint was change in 6-min walking test distance (6MWTD) from baseline to week 24. Secondary endpoints included

Relation of Antecedent Symptoms to the Likelihood of Detecting Subclinical Atrial Fibrillation With Inserted Cardiac Monitors.

Atrial fibrillation (AF) comes to attention clinically during an evaluation of symptoms, an evaluation of its adverse outcomes, or because of incidental detection during a routine examination or electrocardiogram. However, a notable number of additional individuals have AF that has not yet been clinically apparent or suspect-subclinical AF (SCAF). SCAF has been recognized during interrogation of pacemakers and defibrillators. More recently, SCAF has been demonstrated in prospective studies with long-term monitors-both external and implanted. The REVEAL AF trial enrolled a demographically "enri

Cadence Tracking and Disturbance Rejection in Functional Electrical Stimulation Cycling for Paraplegic Subjects: A Case Study.

Functional electrical stimulation cycling has been proposed as an assistive technology with numerous health and fitness benefits for people with spinal cord injury, such as improvement in cardiovascular function, increase in muscular mass, and reduction of bone mass loss. However, some limitations, for example, lack of optimal control strategies that would delay fatigue, may still prevent this technology from achieving its full potential. In this work, we performed experiments on a person with complete spinal cord injury using a stationary tadpole trike when both cadence tracking and disturban

Cardiomyopathy induced by pulmonary sequestration in a 50-year-old man.

A 50-year-old black man presented at the emergency department with midsternal, nonradiating chest pressure and chronic dyspnea on exertion. Four years before the current admission, he had been diagnosed with nonischemic cardiomyopathy at another facility. After our complete evaluation, we suspected that his symptoms arose from left-to-left shunting in association with pulmonary sequestration, a congenital malformation. Our preliminary diagnosis of secondary dilated cardiomyopathy was confirmed by normalization of the patient's ventricular size and function after lobectomy. To our knowledge, th
