Bone marrow pathology of culture proven typhoid fever.

The authors analysed bone marrow findings of sixteen cases of culture proven typhoid fever to reveal the pathologic changes according to the disease stage. The most frequent finding was chronic granulomatous inflammation (eight cases). Infection (bacteria) associated hemophagocytic syndrome (four cases), reactive marrow (two cases), and non specific findings (two cases) were also encountered. Granulocytic hyperplasia with hemophagocytosis appeared at the early stage and was followed by infection (bacteria) associated hemophagocytosis and granuloma in proliferative stage. In lysis (late) stage,…

Typhoid fever associated with adult respiratory distress syndrome.

Adult respiratory distress syndrome (ARDS) is rarely reported in association with typhoid fever despite the fact that sepsis is a common manifestation and endotoxemia has been described in this multisystem condition. We describe the course of a patient with ARDS and typhoid fever. With conventional treatment of the lung injury and with specific antimicrobial therapy, the patient survived. Recent consensus definitions of sepsis and ARDS simplified reporting of this case. Clinicians caring for individuals from or recent visitors to developing countries should be aware of the association of typho…

Unusual aspects of acute Q fever-associated hepatitis.

We describe a 37-year-old farmer with a 3-week history of fevers and hepatitis, in whom Q fever was diagnosed. The diagnosis was based on the findings of characteristic "ring" granulomas on a bone marrow biopsy specimen and confirmed by complement-fixation antibody tests to Coxiella burnetii. Unusual aspects of this case included (1) relatively low complement-fixation antibody titers, (2) prolonged prothrombin time, (3) false-positive results of a serologic test for the human immunodeficiency virus (HIV), and (4) ring granulomas that progressed to atypical granulomas in biopsy specimens.

Nutritional status of children with dengue hemorrhagic fever.

Nutritional status was assessed for 100 patients with serologically confirmed dengue hemorrhagic fever (DHF), 125 patients with other infectious diseases who were admitted to Children's Hospital (Bangkok), and 184 healthy children. The assessment was done with use of the following parameters: weight for age, height for age, and circumference of the middle of the left arm. There were 13 patients classified as undernourished by weight for age, nine of whom were determined to have first-degree malnutrition; six of these had grade II severity of DHF and three had grade III DHF severity. Four patie…

Uncommon presentation of typhoid fever: a case report.

A 24-year-old Chinese woman was admitted for cholestatic jaundice following a short history of fever associated with headache and diarrhoea. She had mild hepatomegaly. Initial laboratory investigations were non-contributory. A percutaneous liver biopsy revealed inflammatory changes more in keeping with a systemic infection than a primary hepatitic problem. Repeat blood and stool cultures finally grew salmonella typhi. Jaundice is a rare manifestation of typhoid fever; it occurs in less than one-third of patients with hepatomegaly. Such a presentation may mimic primary hepatic infections. Aware…

Complications of typhoid fever requiring laparotomy during childhood.

All the complications requiring laparotomy among 680 children who were diagnosed as having typhoid fever between 1981 and 1990, at Hacettepe University Children's Hospital, were retrospectively analyzed. Four patients developed intestinal perforation, two gastrointestinal bleeding, and two splenic abscess, with frequencies of 0.58%, 0.29%, and 0.29%, respectively. The overall frequency was 1.17%. While the risk was 1.58% among males, it was 0.42% among females with no statistically significant difference (P > .05). The risk was higher among patients above 6 years of age (P < .05). While perfor…

Unusual neurologic manifestations occurring during dengue fever infection.

This is a report on dengue fever in two young patients in France that were infected in New Caledonia and Thailand. Both presented with unusual neurologic manifestations. The first patient developed a focal subarachnoid hemorrhage that was associated with transient thrombocytopenia. No neurologic vascular malformation was detected; a mild dengue hemorrhagic fever after a previous dengue infection was suspected. The second patient showed peripheral facial palsy one week after apyrexia without any other etiology except the dengue infection. This case was probably a postinfectious syndrome associa…

[Simultaneous vaccination against hepatitis A and yellow fever].

Simultaneous vaccination against hepatitis A and yellow fever has been studied in our unit: Santé-Voyage, Bordeaux. This randomised study included 108 healthy subjects: 36 received hepatitis A vaccine (Havrix, SKF), 36 received yellow fever vaccine and 36 received both. For hepatitis A vaccine protocol comprised one injection at day zero, one at D15, and one booster dose at 6 months. Those who received yellow fever vaccine had it at D0. Simultaneous vaccination is well tolerated and immunogenicity is as good as it is for each vaccine separately. This is particularly interesting for tropical tr…

Acute Q fever: first case report in Taiwan.

A 70-year-old retired fisherman presented with fever, chills and headache for four days. The clinical presentation was mild, including a self-limiting fever, pneumonia and elevated liver enzyme levels. Acute Q fever was proved by a four-fold rise of phase II IgG antibodies. Interestingly, the man denied any animal contact or travelling over the past year. This case may suggest that Q fever should be added to the list of differential diagnoses for acute febrile disease in Taiwan.

Epidemic of louse-borne relapsing fever in Ethiopia.

During summer 1991 an outbreak of louse-borne relapsing fever occurred simultaneously in two transit camps established for prisoners of war being returned from Eritrea at the end of the Ethiopian civil war. Only antibiotic treatment was given at the Bahr Dar camp where the frequency of cases increased for 20 days. Vector control by delousing in addition to antibiotics was given at Mekele camp where the frequency of cases fell over a similar period. This difference was significant (p < 0.001). Furthermore, there was a significant correlation between the decreasing proportion of camp inhabitants…

Empirical antibiotic therapy for fever in neutropenic patients.

The necessity for administering empirical antibiotic therapy to febrile neutropenic patients has been well-established; however, no single regimen has been uniformly accepted. During the past decade, gram-positive organisms, often methicillin-resistant, have emerged as significant pathogens. The routine use of vancomycin may be appropriate at some institutions, but its widespread use may lead to resistance. The role of aminoglycosides as part of routine empirical regimens is controversial, and single extended-spectrum beta-lactam agents are often adequate therapy. Not all beta-lactam agents ar

Chloramphenicol clearance in typhoid fever: implications for therapy.

We prospectively studied the pharmacokinetics of intravenous Chloramphenicol succinate (CS) in children (age 6 months-14 years) with culture proven typhoid fever (n = 30) and non typhoidal illnesses (n = 10). CS was administered in three different dosage regimens (50, 75 and 100 mg/kg/d-q 6 hourly). Liver function tests were monitored. Plasma trough and peak chloramphenicol concentrations were measured by HPLC analysis after 42 hrs. The 50 mg/kg/day dosage schedule was terminated midway through the study, as blood levels were consistently low and two patients with typhoid relapsed, children wi…

Chloramphenicol and ciprofloxacin in enteric fever--cost-effectiveness.

Clinical decision analysis is a technique which is used to deal with uncertainities in clinical medicine. We used this technique to compare the cost-effectiveness of chloramphenicol and ciprofloxacin in the treatment of enteric fever. The study shows that the most economical alternative can be predicted if the cost of hospital stay and the sensitivity of the organism to chloramphenicol are known. Thus if the hospital costs are Rs 50/- per day, chloramphenicol will be the drug of choice if more than 52% of the organisms are sensitive to the drug. At lower levels of sensitivity to chloramphenico…
