Refining Indications for Orbital Floor Reconstruction in Zygomaticomaxillary Complex Fractures.

PURPOSE: Zygomaticomaxillary complex (ZMC) fractures are common, accounting for ∼40% of all facial fractures. Given the integral component the ZMC plays in the formation of the orbit, all fractures to the ZMC entail some degree of injury to the orbital floor itself. Standard treatment involves open reduction and internal fixation, but the necessity of orbital floor exploration in these cases remains debated. This study utilizes a statistical algorithm to identify clinical and radiographic characteristics that portend the need for this procedure. METHODS: A retrospective review of a prospectiv…

Magnetic Resonance Imaging of Paediatric Orbital Tumors With Pathological Correlation.

INTRODUCTION: Paediatric orbital tumours include a broad spectrum of benign and malignant lesions that range from developmental anomalies to primary and metastatic orbital malignancies. Sometimes, clinical signs and symptoms are not enough to differentiate between orbital lesions; hence, MRI plays a crucial role in cases where clinical and historical findings are inconclusive. AIMS AND OBJECTIVES: The aim of the study is to evaluate the different orbital tumours in paediatric patients by magnetic resonance imaging and their pathological correlation. MATERIALS AND METHODS: We conducted a pros…

Orbital Waldenström Macroglobulinemia With Angiopathic Paraprotein Deposition: A Case Report.

Ocular manifestations of Waldenström macroglobulinemia (WM) are most frequently related to hyperviscosity-related retinopathy but may also include conjunctival infiltration and vitritis. Orbital and ocular adnexal lymphomatous deposition, including within the orbit, is exceedingly rare. We report a unique case of a 66-year-old female with orbital infiltration from WM presenting with recurrent episodes of transient vision loss secondary to angiopathic deposition of eosinophilic hyaline paraprotein in the absence of hyperviscosity syndrome. Right orbital biopsy demonstrated adipose tissue with d…

Multiport combined endoscopic endonasal and transorbital approach to orbital schwannoma.

BACKGROUND: We present a case report describing the use of combined endoscopic endonasal and transorbital approach (EETOA) for intraorbital schwannoma that grew rapidly during pregnancy. CASE DESCRIPTION: A 27-year-old woman who presented with headache was incidentally diagnosed with a tumor mass 1 cm in diameter near the right superior orbital fissure. Treatment of the tumor was deferred to follow-up. One month later, the patient became pregnant, and in the last trimester, she developed right abducens palsy and mildly dilated pupil. Magnetic resonance imaging (MRI) showed that the tumor mass…

Guidelines for standard operation of imaging modalities in orbital diseases (2024).

Orbital disorders include conditions originating from the orbital bones, surrounding tissues, and post-orbital septum. They also include systemic ailments affecting the orbit. Different clinical symptoms make up the complex range of orbital disorders. Because these disorders mostly impact the orbital area instead of the intraocular compartment, there is little diagnostic usefulness for typical ophthalmic visual tests. As such, the vital instruments for diagnosing and evaluating orbital illnesses have become ophthalmic imaging modalities, including ocular ultrasonography (B-scan), computed tomo…

Fronto-orbital advancement: Comparison of syndromic and nonsyndromic craniosynostosis patients.

Craniosynostosis causes functional and aesthetic problems that require fronto-orbital advancement in patients to correct the cranial deformity and to prevent functional problems due to increased intracranial pressure (ICP). In this study, demographic information, operative details, preoperative clinical findings, and postoperative outcomes were reviewed for 106 craniosynostosis patients with at least 1 year of follow-up. Many factors such as functional losses due to increased ICP before surgery, resynostosis, fronto-orbital relapse, surgical complications and aesthetic results were compared in…

A pediatric case of significant oculocardiac reflex activation with orbital edema.

Oculocardiac reflex activation is a rare potential cause of severe hypotensive and/or bradycardic events that threaten hemodynamic stability. It is typically observed after facial trauma, but it can also result from edema secondary to orbital pseudotumor and myositis, sarcoidosis, IgG4 disease, and Graves' orbitopathy. A previously healthy 10-year-old girl with no history of trauma presented with a 2-week history of right eyelid swelling, "drooping," and photophobia, with significant bradycardia on lateral gaze. She did not tolerate initial eye examination because of worsening bradycardia, whi…

Computed Tomographic Features and Prevalence of Orbital Ligament Mineralization in Dogs.

Mineralization within the orbital ligament (OL) is occasionally observed on canine head computed tomography (CT) examinations, typically without associated clinical signs. This feature has been only briefly mentioned in the veterinary literature. The present retrospective descriptive study evaluated 402 dogs to determine the prevalence and CT characteristics of OL mineralization, including its location, morphology, margins, symmetry, size, and attenuation. Associations with signalment, medical history and concurrent mineralization were also assessed. Orbital ligament mineralization was identif…

Reactive Orbital Myositis as a First Manifestation of Renal Cell Carcinoma.

INTRODUCTION: Orbital myositis (OM) is an inflammatory condition of the extraocular muscles, often idiopathic but occasionally associated with systemic diseases or malignancies. Secondary OM due to metastatic disease is rare. We report a case of OM as the initial manifestation of metastatic renal cell carcinoma, emphasizing the need for comprehensive evaluation of atypical ocular presentations. CASE PRESENTATION: An 81-year-old male with a history of age-related macular degeneration presented with acute onset of pain and restricted movement in his left eye. Computed tomography imaging reveale…

Orbital apex syndrome in noninvasive fungal rhinosinusitis: a case report.

Noninvasive fungal rhinosinusitis is the most common type of fungal sinusitis, but it rarely leads to orbital apex syndrome. In this case study, we present a 44-year-old female who developed headache and sudden loss of vision in the left eye. Examination revealed ptosis, complete visual loss, and total ophthalmoplegia in the left eye. Imaging showed a fully opacified mass in the left orbital apex and spheno-ethmoidal recess, involving the adjacent ophthalmic artery, optic nerve, and extraocular muscles. The patient was diagnosed with orbital apex syndrome secondary to noninvasive fungal rhinos…

[Silent sinus syndrome, a rare outcome of an orbital floor fracture].

A 28-year-old man was referred to an oral and maxillofacial surgeon, complaining of fatigue, a tugging sensation in his right eye and difficulty focusing. Closer investigation revealed a progressive shifting of the position of the right eyeball as a result of silent sinus syndrome, as a delayed consequence of an orbital floor fracture. Although clinical symptoms usually occur soon after an orbital fracture, there are dormant sequelae that only develop later, such as the silent sinus syndrome. Displacement of the orbital floor can cause the infundibulum of the maxillary sinus to the nose to bec…
