Substantia nigra iron affects functional connectivity networks modifying working memory performance in younger adults.

Brain iron affects working memory (WM) but the impact of iron content in deep grey matter nuclei on WM networks is unknown. We aimed to test whether deep grey matter nuclei iron concentration can affect resting-state functional connectivity (rsFC) within brain networks modifying WM performance. An N-back WM paradigm was applied in a hundred healthy younger adults. The participants then underwent a resting-state functional magnetic resonance imaging (fMRI) for brain network analysis and quantitative susceptibility mapping (QSM) imaging for assessment of deep grey matter nuclei iron concentratio

Magnetic resonance non-echo planar diffusion-weighted imaging accuracy in detecting cholesteatoma in comparison with clinical and surgical findings.

OBJECTIVE: Magnetic resonance imaging utilising non-echo planar diffusion-weighted imaging is known to have high sensitivity and specificity in detecting cholesteatoma. METHOD: Data was retrospectively collected from patients who had undergone non-echo planar diffusion-weighted imaging from 1 January 2010 to 31 December 2016. The ratio of average pixel grey-scale values between the middle-ear lesion, pons and temporal lobe was used to quantify the degree of restricted diffusion and assess the statistical significance to detect cholesteatoma. RESULTS: A total of 107 patients had non-echo plan…

[An Autopsy Case of Meningoencephalitis and Cerebral Infarction that Developed with Ramsay Hunt Syndrome and Disseminated Herpes Zoster].

We report here the clinical presentation and subsequent autopsy of a 90-year-old man who developed small papules with pain and swelling in his right ear. On admission, he exhibited right facial nerve paralysis, neck stiffness and Kernig's sign. The cell count was elevated and the varicella-zoster virus-PCR was positive in the CSF. Brain magnetic resonance imaging showed hyperintense lesions in the left pons and left temporal lobe, in FLAIR images. We diagnosed the patient with Ramsay Hunt syndrome and meningoencephalitis due to varicella-zoster virus. Although the symptoms of meningitis improv…

Circulating miRNAs expression profiling in drug-resistant epilepsy: Up-regulation of miR-301a-3p in a case of sudden unexpected death.

Sudden and unexpected death in epilepsy (SUDEP) represents one of the most challenging fields for clinical, forensic and preventative pathology. Several authors have emphasized the search of innovative biomarkers related to drug-resistance for an appropriate risk stratification in these patients. However, no reliable biomarker has been implemented into clinical practice, so far. Herein, we present a case of SUDEP due to drug-resistant mesial temporal lobe epilepsy (MTLE) in which we performed miRNA expression profiling (miR-301a-3p, miR-194-5p, miR-30b-5p, mIR-342-5p, and miR-4446-3p) from bot…

Selective changes of resting-state brain oscillations in aMCI: an fMRI study using ALFF.

Mild cognitive impairment (MCI) refers to a transitional state between normal aging and dementia and is a syndrome with cognitive decline greater than expected for an individual's age and educational level. As a subtype of MCI, amnestic mild cognitive impairment (aMCI) most often leads to Alzheimer's disease. This study aims to elucidate the altered brain activation in patients with aMCI using resting-state functional magnetic resonance. We observed Frequency-dependent changes in the amplitude of low-frequency fluctuations in aMCI patients (n = 20), and normal subjects (n = 18). At the same ti

Syncope, seizure-induced bradycardia and asystole: two cases and review of clinical and pathophysiological features.

Episodes of transient loss of consciousness (TLOC) are commonly due to syncope or epileptic seizures. The distinction between both entities on clinical grounds and eyewitness accounts can be challenging and is often hampered by similar clinical features. We briefly summarize syncope-related symptoms and present the case of a female patient who suffered from TLOC episodes due to both reflex syncope and epileptic seizures. Seizure-induced syncope is a rare complication particularly of non-generalized temporal lobe seizures and may be suspected in people with epilepsy who report new semiological …

Subcortical cystic lesions within the anterior superior temporal gyrus: a newly recognized characteristic location for dilated perivascular spaces.

Cystic parenchymal lesions may pose an important diagnostic challenge, particularly when encountered in unexpected locations. Dilated perivascular spaces, which may mimic cystic neoplasms, are known to occur in the inferior basal ganglia and mesencephalothalamic regions; a focal preference within the subcortical white matter has not been reported. This series describes 15 cases of patients with cystic lesions within the subcortical white matter of the anterior superior temporal lobe, which followed a CSF signal; were located adjacent to a subarachnoid space; demonstrated variable surrounding s…

[Brain electrical source analysis of the response to visual target and distractor stimuli].

In this paper, time courses of discrete cortical current sources evoked by target and distractor stimulus are presented in a visual three-stimulus oddball paradigm. 64-channel electroencephalogram (EEG) signals were recorded in healthy subjects in the paradigm where the P3b component of the P300 is evoked in the detection of rare events (target and distractor) and the P3a component is mainly produced by distractor events. A regional source model with constraints of spatial coordinates from fMRI was then applied to event-related potentials (ERPs) data in the target and distractor conditions. Ac

Stimulus type affects Wada memory performance.

The effects of amytal injection side, seizure focus laterality, and stimulus type (real and line-drawn objects, printed words, and faces) on recognition memory were studied during the Wada procedure. To-be-remembered stimuli were presented during cerebral anesthesia to 35 patients with left temporal lobe epilepsy (LTLE) and 28 patients with right temporal lobe epilepsy (RTLE), all with left hemisphere language dominance. In both groups, recognition of real and line-drawn objects was best after anesthetization of the lesional hemisphere. Recognition of faces was poor after either injection in p…

Unexplained sudden amnesia.

BACKGROUND: In almost all cases of acute, sudden, persistent amnesia, the cause can be determined. Ischemic stroke, hypoglycemia, syncope, and seizure are the most likely causes. PATIENTS AND METHODS: In a clinical study, 2 elderly men are described in which sudden, permanent amnesia developed in the absence of a satisfactory explanation. In 1 case, a neuropathologic study disclosed Alzheimer disease; in the other, magnetic resonance imaging showed a temporal lobe abnormality bilaterally. RESULTS: A man aged 80 years suddenly lost his memory of the previous 60 years. Neuropathologic study 5 …

Auditory evoked potentials in epileptic patients.

Auditory brain stem response (ABR) and middle latency response (MLR) were recorded in 49 epileptic patients. Responses were evoked and recorded at 90 dB nHL down to threshold. A statistically significant number of epileptic patients showed elevated ABR (30.1%) and MLR (40.7%) thresholds, even though their pure-tone audiograms showed normal hearing sensitivity. Threshold elevation was more frequent in subjects with grand mal epilepsy compared to subjects with temporal lobe epilepsy reflecting poorer response in the former subgroup. Furthermore, chronicity of illness was significantly related to…

Identifying potential surgical candidates in patients with evidence of bitemporal epilepsy.

PURPOSE: To determine which patients with evidence of medically refractory bitemporal epilepsy are potentially good candidates for surgical therapy. METHODS: We reviewed 42 adults with intractable seizures who were found to have bitemporal ictal onsets, based on scalp video-EEG long-term monitoring (LTM). All underwent invasive LTM before surgery. Surgical outcomes were classified as seizure free, >75% reduction in seizures, or <75% reduction in seizures, >or=1 year after resection. We related the following factors to outcome: (a). >75% preponderance of interictal scalp EEG discharges to one

Different effects of anterior temporal lobectomy and selective amygdalohippocampectomy on verbal memory performance of patients with epilepsy.

The advantage of selective amygdalohippocampectomy (SAH) over anterior temporal lobectomy (ATL) for the treatment of temporal lobe epilepsy (TLE) remains controversial. Because ATL is more extensive and involves the lateral and medial parts of the temporal lobe, it may be predicted that its impact on memory is more important than SAH, which involves resection of medial temporal structures only. However, several studies do not support this assumption. Possible explanations include task-specific factors such as the extent of semantic and syntactic information to be memorized and failure to contr…

Sphenoid Wing Dysplasia in the Absence of Neurofibromatosis: Diagnosis and Management of a Novel Phenotype.

Congenital sphenoid wing dysplasia is one of the major diagnostic criteria for neurofibromatosis type 1, and is often considered pathognomonic for the disease. Between 5% and 12% of neurofibromatosis type 1 cases have evidence of sphenoid wing dysplasia. Sequelae of this deficiency include slow expansion of the middle temporal fossa and progressive herniation of the temporal lobe into the orbital cavity, resulting in pulsatile exophthalmos. Herein, we report a patient with greater sphenoid wing agenesis and middle temporal fossa enlargement requiring transcranial orbital reconstruction in the …

[Transtemporal supralabyrinthine approach].

Transtemporal supralabyrinthine approach is a modified middle cranial fossa approach. It offers all the advantages of a middle cranial fossa procedure and avoids its disadvantages, mainly the extensive temporal lobe retraction and frightening complications. The principle of the approach is to gain sufficient access toward the internal auditory canal by removing bone from the roof of the petrous pyramid rather than by elevating the middle fossa dura away from it. Fifteen patients underwent this approach for decompression of paralysed facial nerve resulted from temporal bone fracture, Bell's pal…
