Failure of humeral shaft fixation: construct characteristics.

PURPOSE: Fixation failure following open reduction and internal fixation (ORIF) of humeral shaft fractures can be a challenging complication. We aimed to identify the modes of failure and characteristics of failed fixation constructs. METHODS: We queried our institutional database for patients > 18 years old with fixation failure after ORIF with single plate and screw constructs of humeral shaft fractures from 2006 to 2017. Demographics, fracture characteristics, fixation construct design and mode of failure were recorded. RESULTS: Twenty-three failures were identified. Mean age was 55.9 yea

Clinical efficiency and safety of Hsp90 inhibitor Novobiocin in avian tibial dyschondroplasia.

Tibial dyschondroplasia (TD) is a bone defect of broilers and other poultry birds that disturbs growth plate and it causes lameness. Previously we evaluated differential expression of multiple genes involved in growth plate angiogenesis and reported the safety and efficacious of medicinal plant root extracted for controlling TD. In this study, clinical and protective effect of an antibiotic Novobiocin (Hsp90 inhibitor) and expression of Hsp90 and proteoglycan aggrecan was examined. The chicks were divided into three groups; Control, thiram-induced TD, and Novobiocin injected TD. After the indu

Internal fixation of dorsally comminuted fractures of the distal part of the radius: a biomechanical analysis of volar plate and intramedullary nail fracture stability.

INTRODUCTION: The purpose of the present study was to carry out biomechanical testing of "new generation" volar plates and an intramedullary nail. METHODS: Four volar locking plates (Column Plate, VariAx distal radius, 2.4 mm-LCP and 3.5 mm-LCP) and the intramedullary nail, Targon-DR, were implanted in biomechanically validated artificial bones after simulation of a wedge osteotomy with total transection of the volar cortex to mimic a type 23 A3-fracture according to the AO-classification. Axial load (250 Newton [N]) and volar and dorsal bending loads (both 50 N) were applied. Axial load was

Not all that lyses is malignancy: Multifocal hemophilic pseudotumor in a 13-year-old with hemophilia A.

Hemophilic pseudotumors (HPTs) are rare but serious complications of severe hemophilia, characterized by encapsulated hemorrhagic masses that can mimic neoplastic lesions. Although incidence has declined with modern prophylaxis, HPTs still occur, especially in patients with inadequate bleeding control. We present the case of a 13-year-old male with hemophilia A and multifocal pseudotumor formation in the distal femur and tibia. The diagnosis was supported by multimodal imaging, which revealed multiloculated, hemorrhagic lesions with cortical erosion. The patient underwent surgical resection wi

[Metal glenoid augmentation in revision arthroplasty].

Glenoid implantation in revision arthroplasty of the shoulder remains a technical challenge in the case of severe bone loss. Metal glenoid augmentation and patient-specific glenoid implants represent new treatment options with great potential. Virtual planning techniques allow a differentiated evaluation of the extent of the bone loss and the planning of different augmentation options. Although not clearly defined so far, a patient-specific implant can be indicated if adequate fixation of the base plate in the native bone appears questionable. The planning is carried out with the support of th

Paradigm Shift in Materials for Skull Reconstruction Facilitated by Science and Technological Integration.

The surgical repair of a bone deficiency in the skull caused by a prior procedure or accident is known as cranioplasty. There are various types of cranioplasties, but the majority entail raising the scalp and reshaping the skull using either the original piece of bone from the skull or a specially molded graft created from Titanium (plate or mesh), artificial bone in place of, a stable biomaterial (prefabricated customized implant to match the exact contour and shape of the skull). Cranioplasty, one of the oldest surgical treatments for cranial abnormalities, has undergone several changes thro

Pubertal abnormalities in adolescents with chronic disease.

Pubertal disorders in the context of chronic disease especially in those with chronic inflammatory disorders or those requiring prolonged periods of treatment with glucocorticoid are common reasons for referral to the paediatric endocrine clinic. Disorders of puberty are also common in adolescents with disability requiring management by paediatric endocrinologists. In these adolescents, impaired skeletal development is also observed and this can be associated with fragility fractures. Chronic inflammation, glucocorticoid and sub-optimal nutrition all impact on the hypothalamic-pituitary gonada

Low-energy atypical femoral shaft and ipsilateral neck fracture: a rare association.

This reports a case of a low-energy ipsilateral femoral shaft and neck fracture in a 69-year-old woman with vitamin D deficiency, who was taking long-term steroids and bisphosphonates. This is a fracture more commonly associated with a high-energy trauma. However, with an ageing global population and an increasing prevalence of bone insufficiency, we predict the incidence of this presentation to increase. Long-term bisphosphonate therapy has been associated with bone insufficiency and an increased rate of delayed union, adding to the complexity of management in these patients. There is current

Cranioplasty for repair of a large bone defect in a growing skull fracture in children.

INTRODUCTION: In infants, calvarial defects are generally repaired with autologous grafts. However, with large defects, these techniques can be associated with complications such as bone graft resorption, loss of blood, or local infection. Alternative materials are available for cranioplasty including metals or acrylic. CASE REPORT: We report the case of a 3.2kg boy who had a traumatic vaginal delivery and developed a growing skull fracture resulting in a large cranial defect (50cm(2)). We describe a specific technique of cranioplasty by interposing a titanium plate between the duroplasty and

Periosteal osteoblastoma: report of a case with a rare histopathologic presentation and review of the literature.

Osteoblastoma is an uncommon benign bone tumor most commonly located in the vertebral column or metaphysis of a long bone. Periosteal location is rare. We report a periosteal-based osteoblastoma, arising from the proximal tibia, in a 20-year-old woman who presented with knee swelling and pain of 2-year duration. Imaging studies showed a metaphyseal surface-based lesion with patchy radiodensities. The cortico-medullary junction was intact. The lesion was totally excised. Histopathologic evaluation disclosed immature bone and osteoid deposition in a vascularized stroma, associated with numerous

Radical reduction operation in the treatment of congenital dislocation of the hip. An analysis of 2,789 cases.

During the period 1960 to 1983, 2,789 operations were performed in patients with congenital dislocation of the hip. Of these, 784 patients were between 1 1/2 and 4 years of age, 1,646 patients were between 4 and 8 years of age, and 359 patients were between 8 and 14 years, with an average age of 10 years. The procedure is performed in a single operation by sectioning all contracted muscles, opening and debriding the joint, and reducing the femoral head into the acetabulum after removal of 2 cm to 3 cm bone segment from the subtrochanteric region of the femur. Fixation of the femoral shaft in a

Invariance of the relative positions of structures attached to long bones during growth: cross-sectional and longitudinal studies.

This article reports the results of cross-sectional and longitudinal studies on the positions of muscles relative to bones. The cross-sectional study reports on the positions of 20 muscles attached to long bones in the chicken from prehatching ages to maturity. The results show that these muscles maintain constant positional relationships during growth as was reported in an earlier study by the authors using rabbits. The longitudinal study reports on the migration of the semitendinosus muscle in the rabbit during growth. The results show that this muscle migrates the distance predicted by the

[The use of cartilage and tissue adhesives for the treatment of surgical-stomatological diseases in old age].

The authors used lyophilized cartilage in plate, bar, chip or powder form in elderly patients. The transplant was also utilized in managing defects resulting from the removal of jaw cysts, bone tumours, etc. Furthermore, the authors treated bone defects of the anterior wall of the maxillary sinus and of the hard palate as well as oro-antral perforations, infected wounds of the soft tissues and inflammatory bone processes. The patients in question had been treated in the surgical service during the years 1959--1975. The authors observed in elderly patients pathological alterations of the capill

Establishment of a novel experimental animal model for the treatment of tibial segmental bone defects in juvenile sheep.

Segmental long bone defects present a significant clinical challenge as critical-size defects cannot heal spontaneously. Most studies focus on adult bone defects, with limited research on pediatric cases. To enhance the study of bone defects in children, we established a juvenile sheep bone defect model. Juvenile small-tailed Han sheep were used to create a 2 cm tibial bone defect, stabilized with a plate and screws. Tissue-engineered bone scaffolds were implanted at the defect site, and the limb was immobilized with a plaster cast for 3 months. Sheep were euthanized at 3 and 6 months post-sur

Novel Subperiosteal Device Geometry and Investigation of Efficacy on Surrounding Bone Formation and Bone-Bonding Strength.

To develop a safer bone-bonding device that promotes early osseointegration with cortical bone perforation, novel subperiosteal device geometries were proposed and evaluated for their ability to facilitate surrounding bone formation and enhance bone-bonding strength. This study used animal experiments and mechanical testing to assess the performance of these designs. The experimental device consisted of two main components: a rounded rectangular plate and a centrally positioned cylinder. To promote the recruitment of bone-marrow-derived factors, slits were incorporated into the cylinder, and a

A New Microarchitecture-Based Parameter to Predict the Micromechanical Properties of Bone Allografts.

Scaffolds are an essential component of bone tissue engineering. They provide support and create a physiological environment for cells to proliferate and differentiate. Bone allografts extracted from human donors are promising scaffolds due to their mechanical and structural characteristics. Bone microarchitecture is well known to be an important determinant of macroscopic mechanical properties, but its role at the microscopic, i.e., the trabeculae level is still poorly understood. The present study investigated linear correlations between microarchitectural parameters obtained from X-ray comp
