Treatment of distal biceps tendon ruptures using a single-incision technique and a Bio-Tenodesis screw.

No consensus exists in the literature on the optimal operative treatment method for distal biceps tendon ruptures. It is our hypothesis that a single-incision technique with a poly-L-lactide Bio-Tenodesis screw (Arthrex, Inc, Naples, Florida) is a safe and effective method for operative management of distal biceps tendon ruptures, with success and complication rates comparable to previous reports in the literature. This article describes a prospective case series of 29 consecutive patients (30 operations) managed by the same surgeon over 34 months. Average follow-up was 19.6 months. Two patien

Olecranon Fractures in Pediatric Patients With Osteogenesis Imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI) is a hereditary disorder characterized by an abnormality of the quality or quantity of type I collagen, leading to bone fragility. Fractures in children with OI may result from minor trauma and have atypical patterns. Previous studies have found a strong relationship between olecranon fractures and OI in pediatric populations, but the characteristics of olecranon fractures within the OI patient population have not been fully described. METHODS: We reviewed the records of 358 children with a diagnosis of OI. Of those, 29 had at least 1 olecranon fractur

Variability in Baseball Throwing Metrics During a Structured Long-Toss Program: Does One Size Fit All or Should Programs Be Individualized?

BACKGROUND: The variability of throwing metrics, particularly elbow torque and ball velocity, during structured long-toss programs is unknown. HYPOTHESES: (1) Elbow torque and ball velocity would increase as throwers progressed through a structured long-toss program and (2) intrathrower reliability would be high while interthrower reliability would be variable. STUDY DESIGN: Descriptive laboratory study. LEVEL OF EVIDENCE: Level 3. METHODS: Sixty healthy high school and collegiate pitchers participated in a structured long-toss program while wearing a validated inertial measurement unit, w

Elbow arthroscopy in children and adolescents: analysis of outcome and complications.

BACKGROUND: Chondral or osteochondral lesions, post-traumatic contracture and loose bodies of the elbow are often associated with chronic pain, stiffness, repetitive swelling and joint blockages. Therefore, arthroscopy of the elbow is often used in the elderly for the treatment of osteochondral defects or arthrolysis. There are only a few reports and studies about arthroscopic therapy of the elbow in children and adolescents. This study assesses the clinical outcome of arthroscopic therapy in this age group. METHODS: In a retrospective study, children and adolescents who underwent an elbow ar

Botulinum toxin for the treatment of motor imbalance in obstetrical brachial plexus palsy.

BACKGROUND: Residual muscle imbalance is a common problem affecting obstetrical brachial plexus palsy patients. The goal of this study was to examine the efficacy of botulinum toxin type A (Botox) in improving this muscle imbalance. METHODS: The authors retrospectively reviewed obstetrical brachial plexus palsy patients treated with Botox for muscle imbalance as an isolated procedure. Outcomes were the change in Active Movement Scale scores from pre-Botox scores to scores at 1 month after Botox and 1 year after Botox. RESULTS: Twenty-seven patients were included, 19 treated for shoulder imba

Nerve transfer strategies for spinal cord injury.

BACKGROUND: Spinal cord injury (SCI) is a devastating condition, which beleaguers its victims with long-term health issues. Nerve transfer is a feasible option for restoration of critical limb function in patients with SCI that potentially improves independence and quality of life. METHODS: This article delineates the general principles of nerve transfer and its specific application pertinent to SCI. The available nerve transfer strategies are described based on the targeted limb function, mostly involving critical upper extremity function. The role of nerve transfer for paraplegia, diaphragm

The prevalence, rate of progression, and treatment of elbow flexion contracture in children with brachial plexus birth palsy.

BACKGROUND: Elbow flexion contracture is a well-known complication of brachial plexus birth palsy that adversely affects upper-extremity function. The prevalence, risk factors, and rate of progression of elbow flexion contracture associated with brachial plexus birth palsy have not been established, and the effectiveness of nonoperative treatment involving nighttime splinting or serial casting has not been well studied. METHODS: The medical records of 319 patients with brachial plexus birth palsy who had been seen at our institution between 1992 and 2009 were retrospectively reviewed to ident

Surgical repair of the distal biceps brachii tendon: a comparative study of three surgical fixation techniques.

PURPOSE: Several fixation techniques exist to repair the distal biceps brachii tendon. We aimed to evaluate the clinical and functional outcomes after distal biceps tendon repair using three different fixation techniques. METHODS: A total of 54 patients were included in the study and were available for follow-up (mean, 30.7 months; SD 18.6). Surgery was performed using a single-incision approach with Corkscrews or Mitek suture anchors or with a two-incision exposure using transosseous sutures. The patients were evaluated with range of motion (ROM) measurements for flection/extension and prona

Four extremity fractures in a patient with renal osteodystrophy.

Renal osteodystrophy (ROD) is the skeletal complication of chronic kidney disease. Secondary hyperparathyroidism and 125 dihydroxy vitamin D3 deficiency are the major causative factors in ROD. Musculoskeletal problems remain among the main limitations of the quality of life of renal failure patients. In this report, a 60-year-old male with four extremity fractures due to a minor trauma was presented. The patient had been receiving haemodialysis for seven years due to hypertensive nephropathy. Our case emphasises the importance of multispecialty approach to the investigation and treatment of pa

Reinsertion of the distal head of the biceps using mini-anchors and the anterior access: a retrospective study.

The authors describe their experience in the treatment of the lesions of the distal tendon of the biceps through the anatomic insertion with Mitek mini-anchors using Henry anterior single-incision access. From 1996 to 2007, 12 patients, all male, average age 51.2 years, were treated for a lesion of the distal tendon of the biceps. In all cases, post-operative pain according to the visual analogical scale, the articular motion of the elbow, the occurrence of neurological disorders and the formation of ectopic ossifications appreciated by radiographies were evaluated. Furthermore, the patient's

Use of suture anchors for coronoid fractures in the terrible triad of the elbow.

PURPOSE: To report outcomes in 6 patients with the terrible triad of the elbow treated with our modified protocol. METHODS: 6 men aged 26 to 54 years underwent surgery for the terrible triad of the elbow by a single surgeon. All the patients had a displaced comminuted fracture of the radius, posterior dislocation of the elbow, and Morrey type-I fracture of the coronoid. They all underwent replacement of the radial head and repair of the lateral collateral ligament to the isometric part of the lateral condyle using suture anchors. Five had an additional capsular fixation to the anterior corono

Super-thinned iliac flap for major defects on the elbow and wrist flexion creases.

Four free iliac flaps were used to treat or prevent flexion contracture at the elbow or wrist flexion crease. Flap size ranged from 13 x 6 cm to 18 x 8 cm. Two flaps were used for primary coverage, and the other 2 flaps were used to treat established flexion contractures. All flaps survived without vascular complications. Full range of motion was obtained at the elbow and 40 degrees of active extension was obtained at the wrist. The flap has a very thin dermis with minimal panniculus that can be thinned as required, making it ideal to cover flexion creases. Despite the fact that anatomic varia

Case report: the split flexor carpi ulnaris as a local muscle flap.

The flexor carpi ulnaris is a useful local muscle flap in the forearm and elbow. It is, however, an important palmar flexor and ulnar deviator of the wrist, and functional loss may arise from the use of this muscle in its entirety. The flexor carpi ulnaris is made up of two distinct neuromuscular compartments. This arrangement allows for splitting of the muscle and the potential use of the larger ulnar compartment as a local muscle flap while maintaining the humeral compartment as an ulnar deviator and palmar flexor of the wrist. We report two cases illustrating the clinical use of the split f

Botulinum toxin for treatment of cocontractions related to obstetrical brachial plexopathy.

Botulinum toxin type A was recently introduced for treatment of biceps-triceps muscle cocontraction, which compromises elbow function in children with obstetrical brachial plexopathy. This is our preliminary experience with this new approach. Eight children were treated with 2-3 U/kg of botulinum toxin injected in the triceps (4 patients) and biceps (4 patients) muscle, divided in 2 or 3 sites. All patients submitted to triceps injections showed a long-lasting improvement of active elbow flexion and none required new injections, after a follow-up of 3 to 18 months. Three of the patients submit

Management of Gartland's type III supracondylar fractures of the humerus in children: the role audit and practice guidelines.

In 1995, we audited the outcome following Gartland's type III supracondylar fractures of the humerus in children. Closed reduction under anaesthesia (MUA) and cast immobilisation was the treatment of choice, with the treatment changed whenever displacement occurred. The result of the audit showed that 86% of cases were treated with this method. The rate of re-operation was 45.6%. Cubitus varus deformity developed in 50% of cases. Guidelines were introduced in order to abandon the use of MUA and cast immobilisation for type III fractures. The guidelines recommended treating these fractures with
