Prospective randomized controlled trial: early weight bearing after conservative treatment of Weber B ankle fractures (pancake trial).

PURPOSE: Different studies have shown that weightbearing is safe in stable transsyndesmotic, isolated lateral simple ankle fractures. Despite this evidence, AO guidelines still recommend immobilization with above-the-knee cast for 4-6 weeks for these fractures. The objective of this study was to compare the outcomes of mobilization and weightbearing to those of immobilization and non-weightbearing in patients with stable transsyndesmotic, lateral isolated simple ankle fractures. METHODS: Fifty patients were randomly assigned to permissive weightbearing in a walking boot or non-weightbearing i

Incidence and Reimbursement Trends for Facial Fracture Repair in the Elderly: A Medicare Analysis.

OBJECTIVES: The aim was to describe incidence and reimbursement trends of surgical repair of facial fractures among the Medicare population. METHODS: The annual procedure data from the Centers for Medicare and Medicaid Service National Part B Data File from 2000 to 2019 were queried. RESULTS: The total number of surgically corrected facial fractures increased from 10,148 in 2000 to 19,631 in 2019 in a linear pattern (r = 0.924). Specifically, nasal bone/septum fracture repairs increased the most by 200.6% (n = 4682 to n = 14,075), whereas operations for TMJ dislocations, malar/zygoma fractur

Revision Procedure After Surgery for Atypical Hangman's Fracture Primarily Performed Only from the Posterior Approach - An Attempt to Maintain Head Rotation: Case Report.

INTRODUCTION: Hangman's fracture, also known as traumatic spondylolisthesis of the axis, is defined as a bilateral fracture of the C2 pars interarticularis. In 1965, Schneider used this term to describe a pattern of similarities seen in fractures associated with judicial hangings. However, this fracture pattern is only observed in approximately 10% of injuries associated with hangings. CASE REPORT: We present a case of an atypical hangman's fracture caused by a headlong dive into a swimming pool and striking the pool's bottom. The patient had undergone surgery at another centre, where posteri

Radiomorphometric analysis of isolated zygomatic arch fractures: A comparison of classifications and reduction outcomes.

PURPOSE: Although different proposals have been made to categorize isolated zygomatic arch fractures (ZAF), an investigation about fracture type and clinical outcome has not been published. In this study, we analyzed the geometric fracture morphology in isolated ZAF and provide a survey of reduction outcomes in accordance with 4 independent classifications. MATERIALS AND METHODS: Geometric measurements were performed in radiologic images of 94 patients with isolated ZAF, which were consecutively treated by closed reduction over transbuccal approach. Fracture types were allocated to the classi

Treatment of Acetabular Fractures in Adolescents.

Although the treatment of acetabular fractures in adults has evolved substantially, treatment of these injuries in adolescents remains primarily nonoperative. We performed a retrospective review to evaluate outcomes of treatment of adolescent acetabular fractures. We identified 38 adolescent acetabular fractures (patient ages, 11-18 years), all treated by an experienced trauma surgeon. Open reduction and internal fixation (ORIF) was performed in 37 cases, and 1 case was treated nonoperatively. Mean follow-up was 38.2 months. All fractures healed. Reduction was anatomical in 30 cases, imperfect

Delayed union in pediatric forearm fractures.

BACKGROUND: The aim of this study was to assess the predictive factors of delayed union in pediatric forearm fractures. Few previous reports have approached this complication, but contrary to these studies we excluded cases with known pathogenic factors and included in the sample cases treated with conservative methods. METHODS: This is a case-control study of all pediatric shaft forearm fractures presenting delayed union in a minimum follow-up period of 12 months, from 2003 through 2009, in a hospital covering a health area. Exclusion criteria were fracture dislocations, infection, suboptima…

The periauricular transparotid approach for open reduction and internal fixation of condylar fractures.

INTRODUCTION: Treatment of fractures of the mandibular condyle fractures varies among centres as there still is no general consensus. The aim of this paper was to determine the safety and efficiency of surgical treatment using a transparotid approach for direct plating. PATIENTS AND METHODS: A prospective clinical study was conducted on 34 patients with 36 fractures of the condyle. All 36 fractures were displaced, and 14 (39%) of them were fracture dislocations. The fractures were treated surgically with a transparotid facelift or retromandibular approach using miniplates and screws for fixat…

The Influence of Dorr Type and Femoral Fixation on Outcomes Following Total Hip Arthroplasty for Acute Femoral Neck Fractures: A Multicenter Study.

BACKGROUND: The American Academy of Orthopaedic Surgeons guidelines report moderate evidence for cementing femoral stems for hip fractures, mainly derived from hemiarthroplasty literature. This is the first large, nonregistry study examining the influence of femoral fixation, implant type, patient characteristics, and radiographic factors on outcomes after total hip arthroplasty (THA) for acute femoral neck fractures. METHODS: A multicenter retrospective study was performed of 709 THA cases (199 cemented, 510 cementless) for femoral neck fractures from 2006 to 2020 at three large academic ins

New aspects of the skin barrier organization.

The original Kocher approach was published several times in the 18921907 period. It extends in the interval between the extensor carpi ulnaris and the anconeus and consists in subperiostal release of the lateral collateral ligament (LCL), joint capsule and origin of extensors at the lateral epicondyle and their retraction anteriorly, and a similar release of the anconeus from the distal humerus and its reflection posteriorly. This provides an extensive approach to the elbow. Today this approach is described in the textbooks in various modifications that have little in common with the original

Does the Degree of Liner Constraint Increase Risk of Complications in Articulating Spacers in Two-stage Revision After THA?

BACKGROUND: Two-stage revisions for chronic periprosthetic joint infections (PJIs) often include antibiotic-loaded cement spacers to control for infection and preserve function. While studies have reported on complications (dislocations, readmissions, and reoperations) after static versus articulating spacer types, there is a paucity of evidence about whether the degree of spacer constraint in articulating spacers affects these complications. This study aims to address a key gap in understanding as to whether the level of spacer constraint affects complications in two-stage revision THA utiliz

Incidence and Characteristics of Midfoot Injuries.

BACKGROUND:: The epidemiology of midfoot injuries is poorly known. It has been estimated that the incidence of Lisfranc injuries (intra-articular injury in the tarsometatarsal joint) is 1/55 000 person-years and the incidence of Chopart injuries (intra-articular injury in the talonavicular and calcaneocuboidal joint) 4/100 000 person-years. The purpose of our study was to assess the computed tomography (CT) imaging-based incidence (per 100 000 person-years) and trauma mechanisms of midfoot injuries. METHODS:: All CT studies performed due to acute injury of the foot and ankle region between Ja

Folate deficiency and total shoulder arthroplasty outcomes.

INTRODUCTION: Folate deficiency is inversely correlated with inflammatory (markers/conditions) and severe osteoarthritis. Osteoarthritis can lead to increased difficulty in shoulder reconstruction and longer operative times in total shoulder arthroplasty (TSA). Our goal is to see if folate deficiency is correlated with complications after TSA. METHODS: A retrospective cohort analysis using the TriNetX database was used to identify individuals with low (≤2.5 ng/mL) and normal (≥2.6 ng/mL) folate levels before TSA. Outcomes were compared at 90 days, two years, and four years following TSA. RES

Strain-Controlled Low-Cycle Fatigue Behavior and Microstructure Evolution of the Hot-Work Die Steel at 700 °C.

This study investigates the low-cycle fatigue behavior and microstructural evolution of a novel 30Cr2Ni3MoWV hot-work die steel at 700 °C under different strain amplitudes. High-temperature tensile tests demonstrated a tensile strength of 460 MPa and an elongation of 32%, confirming the material retains good ductility. Fracture analysis revealed ductile failure, supported by a 95% reduction in area. Low-cycle fatigue tests indicated notable cyclic softening at high strain amplitudes, with fatigue life declining rapidly as strain amplitude rose from 0.2% to 0.6%. A stress-softening coefficient

Unusual form of the distal bone defect of ulna with neurofibromatosis type 1: A case report.

RATIONALE: Bone malformation occurs in 10% to 25% neurofibromatosis type 1 (NF-1) patients, and the manifestations are scoliosis, congenital arch and pseudo-joint formation, bone cyst, and pathologic fracture. However, a large segmental defect without obvious signs of bone destruction has rarely been reported. PATIENT CONCERNS: A 4.5-year-old male presented with a 4-year history of shortening of the right upper limb and radial head dislocation. The X-ray indicated a lack of the distal part of the right ulna and radial head dislocation. DIAGNOSIS: The X-ray showed obvious bone resorption at t
