The authors report a case of cerebellar hemangioma excision in a 13-year-old girl with unsuspected pheochromocytoma. This patient had an uneventful attempt at embolization of her tumor under general anesthesia but manifested hypertensive crisis during the intracranial surgery. The intra- and postoperative course of events and management are described. The possible causes leading to hypertension in an unsuspected pheochromocytoma are discussed. The anesthetic management of this patient for excision of pheochromocytoma is also presented.

In this paper, a case of closed medial subtalar dislocation and accompanying talar head fracture in a 22-year-old man which occurred while walking on a downhill road is reported. Closed reduction under general anesthesia was unsuccessful. The obstacle for closed reduction was determined at surgery for open reduction and internal fixation as buttonholing of the talar head through the extensor retinaculum. At the 26-month follow-up, he was pain-free in his daily activities.

Surgical procedures commonly associated with parenteral sedation or general anesthesia were completed in an office operating room with local anesthesia and oral medication. Diazepam and alprazolam were used in combination to produce a relaxed state, allowing local anesthesia to be instilled. Surgery was completed successfully in all patients, and no untoward effects were seen in the patients. Amnesia was associated with the drug combination, and a protective action of the long-lived drugs against lidocaine toxicity is theorized.

This case, which was presented as a log diary for the 1997 Diploma of Membership in General Dental Surgery (MGDS) examination, describes the examination, treatment planning, and subsequent treatment of a patient displaying mild hypodontia with retained deciduous teeth and lateral excursions guided by group function. It incorporates preventive treatment and management of early interproximal caries, prerestorative orthodontics, reorganisation to a canine-guided occlusion using conventional and adhesive cast restorations, and management of an occasional clenching habit.

Nd YAG Laser photocoagulation for the purpose of residual cavity treatment following echinococcectomy was introduced in 1996, and practically implemented in the Department of General and Operative Surgery. Eleven patients with uncomplicated echinococcus of the hepar, 4 with purulent echinococcosis and 4 with rupture of the biliary tree, aged 26 to 80 years, are subjected to operation. The advantages and possibilities of dispersed laser application in hydatid disease treatment are discussed. The short-term results are estimated as satisfactory.

Therapeutic laparoscopic techniques are well accepted in many areas of general surgery. The use of laparoscopy for the staging of malignancy, in a diagnostic role, is not used widely. Urologists use pelvic lymph node dissection (LPLND) to define those patients who are suitable for curative local therapy in carcinoma of the prostate. We present a case of a patient with metastatic malignant melanoma in whom LPLND was employed as a minimally invasive way of staging and treating her disease.

The authors describe a patient with stenosis of the renal artery undergoing bilateral kidney autotransplantation and reconstruction of the aorta with autologous saphena. This study also provides a general evaluation of renovascular hypertension incidence, etiopathogenesis, symptomatology, diagnostic criteria, therapy. The presence of a diastolic pressure > 110 mmHg, with normal creatinin levels and urinalysis results, is very significant and leads to invasive investigations. Site and size of the anatomic anomaly are evaluated by arteriography. Surgery is the treatment of choice.

We report the case of a 51-years-old female who was prophylactically treated with diphenylhydantoin after surgery of an intracranial aneurysm. Twenty-four days after beginning the treatment, a general syndrome appeared, in addition to a diffuse cutaneous exanthema. Four days later, a cholestatic syndrome, compatible with "toxic cholestatic acute hepatitis", was developed. The suppression of the drug, was followed by rapid clinical improvement and gradual normalization of transaminases values, although biochemical cholestasis persisted for months.

Securing the airway following surgery can be a problem in certain patients. Preparation, therefore, should include a plan for safe reintubation, if necessary. We report two patients with a difficult airway (recurrent thyroid carcinoma, inoperable carcinoma of the hypopharynx) who were at a high risk for intubation under general anaesthesia. In addition, one of the patients suffered from an acute abdomen and ileus. We used a guidewire placed into the trachea via a fiberscope to facilitate fiberoptic reintubation.

The authors anesthetized a 48-year-old woman with endometrial cancer and a large ovarian cyst. She developed cardiac failure initially followed by the sick sinus syndrome and A-V block from hypertrophic cardiomyopathy, prior to neuromuscular symptoms. Epidural anesthesia assisted by general anesthesia was carried out safely without intravenous administration of any muscle relaxants. From this experience, it is considered that epidural anesthesia assisted with some other proper methods is suitable for surgery of lower abdomen.

A size 3 laryngeal mask was used in 20 patients who underwent elective general surgery. It consists of a silicon tube attached to mask which must be inserted into the hypopharynx and then inflated. No curarisation nor laryngoscopy is needed. Operations were performed in assisted or spontaneous ventilation, with good airway patency and absence of coughing, secretions and sore throat. In 2 cases introduction of the mask failed. Technique, advantages and problems related to its use are described.

We report postoperative pain management of two adolescents after upper abdominal procedures, one with Hurler-Scheie syndrome and a second with Duchenne muscular dystrophy, and both had progressive spinal scoliosis with poor pulmonary function. A patient not known in advance to have the sleep apnoea syndrome (SAS) was administered a combined epidural-general anaesthetic for a proposed radical prostatectomy. After surgery which had to be discontinued due to extensive tumoural spread, morphine 5 mg was administered through the epidural catheter for analgesia. Severe respiratory depression occurred eight hours later and was successfully reversed by repeated injections of naloxone. The potential danger of epidural morphine administration to SAS patients is discussed.

Two cases of asymptomatic visceral larva migrans (VLM) involving the myocardium, discovered during open heart surgery, are reported. Only 7 cases of VLM myocarditis were found in recent English publications. All 7 cases involved children, with most of the patients acutely ill. Our 2 asymptomatic cases, coupled with the high rate of positive serologic tests in the general population, stress the widespread nature of Toxocara canis infection in human and the large number of subclinical, undetected cases.

Pseudocholinesterase activity was investigated in three groups of unselected patients. A slight rise of the enzyme was found during surgery under local anesthesia with lidocaine. In general anesthesia with thiopentone and halothane, the pseudocholinesterase activity diminished. The inhibition was more pronounced when pancuronium had been injected. From these data one may conclude that pancuronium must be carefully given in patients with low level of pseudocholinesterase when other drugs inhibiting the enzyme activity, like succinylcholine, procaine and propanidid, are used.

Athough in general, measurement of renal vein renin appears to give a good prediction as to the subsequent response to surgery, its main value lies in its ability to reflect changes in renal plasma flow; true changes in renin secretion rate being much more difficult to detect. Although it is a little early to say how much information can be derived from saralasin infusions, caution must be exercised in necessarily assuming that the test accurately reflects subsequent surgical response.
