[Computer tomography in the diagnosis of adrenal lesions in arterial hypertension].

The method is based on reconstructing the image of the transverse section of the body on a television screen by means of a computer, depending on the capacity of the tissue to absorb X-rays. A cross-section is constructed on the basis of a large number of projections, where each tissue has its own color in a black-and-white or colour scale. The section obtained is topographo-anatomical and allows the shape, dimension, structure and mutual position of the internal organs to be clearly determined. Twenty one patients suspected of having various lesions of the adrenals were examined. Computer tom

Hypotensive response to angiotensin II analogue and angiotensin I converting enzyme inhibitor in pseudo-Bartter's syndrome.

We studied the effect of angiotensin II analogue (AII-A) and angiotensin I converting enzyme inhibitor (SQ 14,225) on blood pressure and the renin-angiotensin-aldosterone system in a patient with pseudo-Bartter's syndrome, who was a 26-year-old unmarried Japanese woman taking furosemide surreptitiously. The intravenous infusion of AII-A decreased blood pressure from 85/35 to 68/28 mmHg. This decrease in blood pressure was associated with an increment of plasma renin activity (PRA) and a decrement of plasma aldosterone concentration (PAC). Similarly, SQ 14,225 given orally decreased blood press

[Bartter's syndrome: seven cases in siblings. Hypothesis of mild forms (author's transl)].

Three children with a Bartter's syndrome have been investigated: all of them had growth retardation, hypokalemia (less than 3 mmol/l), raised plasma renin activity and urinary prostaglandins (PGE2 and PGF2 alpha) and a decreased sensibility for angiotensin. In the siblings two children had also growth retardation with mild biological signs of Bartter's syndrome, and two children had normal growth slight hypokalemia raised RPA and urinary PH, and normal sensibility for angiotensin. These data suggest mild forms of this syndrome which could be the Bartter's syndrome diagnosed in adults after lax

Growth characteristics in patients with Bartter's syndrome.

Studies on the growth and development of patients with Bartter's syndrome indicate that severe growth retardation occurs during infancy and early childhood. Delayed adolescent growth spurt has occurred in all patients studied thus far who had manifested the syndrome during infancy. Normal stature is eventually attained. Mental development ranges from normal to brain damage and dysfunction; however, the majority of patients show some degree of mental retardation. The coexistence of Leigh's encephalopathy with Bartter's syndrome in one patient and the finding of severe motor and congnitive retar

[Interdisciplinary cooperation in the diagnosis and therapy of diseases of the adrenal glands and the parathyroid glands].

It is reported on the interdisciplinary cooperation between internists, radiologists and surgeons in the diagnostics and therapy of endocrinological diseases (suprarenal gland, parathyroid gland). On the basis of instances diagnostic step programmes are represented and first experiences of the cooperation are reported.

Radiologic approach to adrenal lesions.

A large number of radiologic studies are currently (or will soon be) available for the evaluation of adrenal lesions. In any given set of clinical circumstances only selected studies will be necessary, and it is important that full clinical and laboratory evaluation be performed before proceeding to radiology, so that only the needed studies are performed. This approach will provide the highest degree of accuracy with the least risk and cost for the patient.

Conn's syndrome. A follow-up of thirteen surgically treated cases.

Thirteen patients were followed for 4-46 months after removal of an aldosterone producing adenoma. Normotension was achieved in all cases but two in whom moderate diastolic hypertension was easily managed on diuretic therapy. All were cured of hypokalemia and symptoms related to low plasm potassium. Persistaent selective hypoaldosteronism was seen in one patient. A gratifying regression of symptoms and signs related to arterial hypertension was seen. Medical treatment with aldosterone antagonists may "cure" the patient to the same extent as surgery. The present results encourage the use of sur

Surgical decision-making and referral patterns for adrenal vein sampling in primary aldosteronism: a national survey of endocrine surgeons.

BACKGROUND: This study aimed to discover and explore factors associated with endocrine surgeons' (ES) referral patterns to Interventional Radiologists (IR) for adrenal vein sampling (AVS) for patients with primary aldosteronism (PA). METHODS: Following IRB approval, an electronic survey was distributed to members of the American Association of Endocrine Surgeons (AAES). This cross-sectional, descriptive survey was designed to generate hypotheses about AVS referral behavior rather than test prespecified causal relationships. Data collected included demographics, practice type, referral pattern

Optimal Cut-Off Values of the Aldosterone-Renin Ratio in Different Age Groups for Primary Aldosteronism Screening.

This study sought to establish age-specific optimal cut-off values for the aldosterone-to-renin ratio (ARR) in screening for primary aldosteronism (PA). The analysis included 281 patients with confirmed PA and 1242 with essential hypertension (EH), categorized into four age groups: ≤39, 40-49, 50-59, and ≥60 years. A positive correlation was observed between ARR and age in both patient groups. Receiver operating characteristic analysis determined the optimal ARR cut-offs to be 2.3, 2.8, 3.1, and 3.6 for the respective age groups, with all demonstrating high diagnostic accuracy. Further evaluat
