[Surgical site infection after total knee arthroplasty: a monocenter analysis of 923 first-intention implantations].

PURPOSE OF THE STUDY: We report the results of a retrospective analysis of 923 cases of first-intention total knee arthroplasties. The objective was to determine retrospectively the rate of surgical site infections, including all infections diagnosed during the first year, and to search for risk factors. We also wanted to present our surveillance system planned for a 10-year period. MATERIAL AND METHODS: From January 1994 to January 2004, first-intention total knee arthroplasty (TKA) was performed on 999 knees. HLS prostheses were implanted. At minimum 12 months, follow-up data was complete f

Spectrum of Ophthalmic Manifestations in Patients With Transfusion-Dependent Thalassemia.

INTRODUCTION: Thalassemia is a hereditary blood disorder characterized by impaired hemoglobin production, necessitating regular blood transfusions to manage anemia and associated complications. This condition also poses a significant risk for a range of ophthalmic manifestations due to factors such as iron overload from repeated transfusions, organ dysfunctions, and metabolic imbalances. This research aimed to evaluate the prevalence of ophthalmic anomalies in patients with transfusion-dependent thalassemia (TDT) and to ascertain their correlation with serum ferritin levels, hemoglobin concent

Educational Priorities in Oncologic Critical Care: A Modified Delphi Study.

BACKGROUND: Advances in cancer diagnosis and treatment have prolonged survival, expanded therapeutic options, and introduced novel treatment-related toxicities. Patients with cancer now account for more than 20% of ICU admissions, exposing intensivists to increasingly complex clinical scenarios. Despite this evolving landscape, oncologic critical care education within PCCM fellowship training remains unstructured, with no consensus on educational priorities or expectations for trainee practice. OBJECTIVES: To define consensus-based educational priorities in oncologic critical care for PCCM fe

Artificial intelligence in nutritional health: sustainable consumers' right to quality of life with special reference to youth.

BACKGROUND: Maintaining optimal youth nutritional health is an urgent socio-economic imperative that underpins long-term human productivity and rights-based development. However, modern youth cohorts face unique dietary threats caused by the widespread availability of ultra-processed foods, targeted digital marketing, and complex food labeling protocols. Although Artificial Intelligence (AI) presents innovative avenues for personalized dietary profiling, existing systems remain largely technocentric and detached from statutory frameworks or behavioral realities. OBJECTIVE: This study bridges

A Qualitative Investigation of Therapist and Diagnostician Perspectives on Visual Disorders in Autism Spectrum Disorder.

Visual behaviors such as reduced eye contact, impaired eye tracking, limited joint attention, delayed responses to visual stimuli, and prolonged visual exploration of objects are central to autism spectrum disorder (ASD) diagnosis and intervention. However, these behaviors may also result from unrecognized visual impairments, increasing the risk of diagnostic overshadowing and suboptimal intervention planning. This study explored how diagnosticians and therapists perceive and address visual impairments in children with ASD, and how clinical reasoning and systemic factors influence their integr

Discordance between clinician- and patient-reported outcomes during radiotherapy for brain metastases: the prospective NeuroPRO study.

BACKGROUND: Clinician-reported outcomes (CROs) remain a central component of side effect assessment in radiotherapy, while patient-reported outcomes (PROs) are increasingly being integrated into standard practice to better capture the subjective symptom burden experienced by patients. Agreement between CRO and PRO has not been prospectively studied in patients undergoing radiotherapy for brain metastases. METHODS: NeuroPRO was a prospective longitudinal cohort study enrolling patients receiving radiotherapy for brain metastases. CROs were assessed using predefined CTCAE (version 5.0) items, w

Eye and Systemic Disease Management Changes After Teleophthalmology Screening in Primary Care: Retrospective Cross-Sectional Pilot Study of 200 Consecutive Patients.

BACKGROUND: Undiagnosed ocular diseases and ocular complications from systemic diseases are common in primary care populations, and many can be detected through retinal imaging before symptoms develop. Asynchronous store-and-forward teleophthalmology offers a scalable way to integrate eye screening into primary care, yet its broader impact beyond diabetes and diabetic retinopathy detection remains underexplored. OBJECTIVE: This study evaluated the outcomes of asynchronous store-and-forward teleophthalmology screening in a primary care clinic, including detection and triage of ocular condition

Impact of functional re-education and environmental adaptation in cancer patients with respiratory pathology: Study protocol.

BACKGROUND: In recent years, cancer survival rates have increased exponentially. However, this rise in survival comes with a significant drawback. As the number of treatment lines has grown, so too have the side effects, which can severely impact patients' functionality and quality of life. One of the most concerning effects is dyspnoea, a serious health issue that imposes substantial limitations on individuals. While traditional clinical approaches, primarily focused on pharmacological interventions, are commonly employed to manage dyspnoea, we argue that these methods may not always provide

What is quality in long covid care? Lessons from a national quality improvement collaborative and multi-site ethnography.

BACKGROUND: Long covid (post covid-19 condition) is a complex condition with diverse manifestations, uncertain prognosis and wide variation in current approaches to management. There have been calls for formal quality standards to reduce a so-called "postcode lottery" of care. The original aim of this study-to examine the nature of quality in long covid care and reduce unwarranted variation in services-evolved to focus on examining the reasons why standardizing care was so challenging in this condition. METHODS: In 2021-2023, we ran a quality improvement collaborative across 10 UK sites. The

Developing a Life Story Intervention for Older Adults With Dementia or at Risk of Delirium Who Were Hospitalized: Multistage, Stakeholder-Engaged Co-Design Study.

BACKGROUND: Older adults with chronic or acute cognitive impairment, such as dementia or delirium, who are hospitalized face unique barriers to person-centered care and a higher risk for negative outcomes stemming from hospitalizations. There is a need for co-designed interventions adapted for these patients to the hospital setting to improve care and outcomes. Patient life storytelling interventions have demonstrated promise in enhancing person-centered care by improving patient-care team relationships and providing information to enable care tailored to individual needs and values. Perception of People Diagnosed With Fibromyalgia About Information and Communication Technologies for Chronic Pain Management: Cross-Sectional Survey Study.

BACKGROUND: Chronic pain is prevalent in our society, with conditions such as fibromyalgia being notably widespread. The gold standard for aiding individuals dealing with chronic pain involves interdisciplinary approaches rooted in a biopsychosocial perspective. Regrettably, access to such care is difficult for most of the people in need. Information and communication technologies (ICTs) have been used as a way of overcoming access barriers (among other advantages). OBJECTIVE: This study addressed the little explored area of how individuals with fibromyalgia use and perceive different types o
