Estimation of tibiofemoral static zero position during dynamic drop landing.

OBJECTIVE: The objective is to assess the in vivo knee secondary motions intrinsic to flexion in isolation from actual displacements during a landing activity. For this purpose a "static zero position", which denotes the normal tibiofemoral position to the static flexion angle, was introduced to describe the intrinsic secondary motion. METHODS: The three-dimensional motion data of the healthy knee were collected from 13 male and 13 female young adults by using an auto motion analysis system and point cluster technique. First, the relationship between flexion and secondary motion in the static

Dynamic motor compensations with permanent, focal loss of forelimb force after cervical spinal cord injury.

Incomplete cervical lesion is the most common type of human spinal cord injury (SCI) and causes permanent paresis of arm muscles, a phenomenon still incompletely understood in physiopathological and neuroanatomical terms. We performed spinal cord hemisection in adult rats at the caudal part of the segment C6, just rostral to the bulk of triceps brachii motoneurons, and analyzed the forces and kinematics of locomotion up to 4 months postlesion to determine the nature of motor function loss and recovery. A dramatic (50%), immediate and permanent loss of extensor force occurred in the forelimb bu

Bilateral oscillatory hip movements induce windup of multijoint lower extremity spastic reflexes in chronic spinal cord injury.

After spinal cord injury (SCI), alterations in intrinsic motoneuron properties have been shown to be partly responsible for spastic reflex behaviors in human SCI. In particular, a dysregulation of voltage-dependent depolarizing persistent inward currents (PICs) may permit sustained muscle contraction after the removal of a brief excitatory stimulus. Windup, in which the motor response increases with repeated activation, is an indicator of PICs. Although windup of homonymous stretch reflexes has been shown, multijoint muscle activity is often observed following imposed limb movements and may ex

Influence of spinal reflexes on the locomotor pattern after spinal cord injury.

In complete spinal cord injured (cSCI) subjects a shift from dominant early (60-120ms latency) to dominant late (120-450ms latency) spinal reflex (SR) components occurs over time after injury. This shift is assumed to reflect a spinal neuronal dysfunction below the level of a spinal lesion. The neuronal pathways of SR are suggested to be closely connected with spinal locomotor circuits. The aim of this study was to explore the influence of the two SR components on the electromyographic (EMG) pattern induced by assisted locomotion in cSCI subjects. Leg muscle EMG activity was analysed during as

The percutaneous surgical approach for repairing acute Achilles tendon rupture: a comprehensive outcome assessment.

BACKGROUND: Treatment modalities for acute Achilles tendon rupture can be divided into operative and nonoperative. The main concern with nonoperative treatment is the high incidence of repeated ruptures; operative treatment is associated with risk of infection, sural nerve injury, and wound-healing sequelae. We assessed our experience with a percutaneous operative approach for treating acute Achilles tendon rupture. METHODS: The outcomes of percutaneous surgery in 29 patients (25 men; age range, 24-58 years) who underwent percutaneous surgery for Achilles tendon rupture between 1997 and 2004

Postoperative and midterm outcomes of minimally invasive and endoscopic great saphenous vein harvesting for coronary artery bypass grafting - a prospective analysis.

BACKGROUND: We sought to determine whether the postoperative and midterm outcomes of minimally invasive and endoscopic great saphenous vein harvesting are comparable. METHODS: From February 2004 to September 2006, 120 patients underwent minimally invasive vein harvesting, and subsequently 150 patients had endoscopic vein harvesting for CABG. Patients were evaluated prospectively for wound-healing disturbances, residual leg oedema, pain intensity and saphenous neuropathy on the 7th postoperative day and after 3 months. RESULTS: Both harvesting techniques were associated with a low incidence o

Impact of sensorimotor training on the rate of force development and neural activation.

The ability to generate high muscular strength within short time periods is of functional importance not only as a basic quality in many sports disciplines but also for active stabilization of joints. The rate of force development (RFD) is generally used to describe this ability. The purpose of the present study was to examine the functional adaptations of a specific sensorimotor training on the explosive strength qualities of the leg extensor muscles during maximum isometric actions. Force and surface EMG were recorded in 17 subjects before and after a 4-week sensorimotor training period. Max

Short-contact topical tretinoin therapy to stimulate granulation tissue in chronic wounds.

BACKGROUND: The use of retinoids in wound healing is increasing. It has been shown that retinoic acid reverses the inhibitory effects of glucocorticoids on wound healing and accelerates the formation of healthy granulation tissue. Pretreatment with tretinoin before epidermal injury such as chemical peeling and dermabrasion has shown accelerated wound healing. Enhanced healing of full-thickness skin wounds has also been demonstrated in early wound healing studies. However, tretinoin therapy can be quite irritating. OBJECTIVE: Our purpose was to observe the clinical and histologic effects of to

Necrotizing fasciitis: case study of a nursing dilemma.

Necrotizing fasciitis usually manifests as a low grade cellulitis that quickly deteriorates to a limb and life threatening soft tissue infection. Immediate surgical debridement is essential, after which wound management becomes the nurse's primary concern. Case #1 reports on a 72 year old female who, upon presenting to the ER with a "sore bottom," subsequently had these diagnoses: (1) anal-rectal abscess, (2) Fournier's gangrene, (3) ulcerative enterocolitis, (4) chronic blood loss/anemia, and (5) protein caloric malnutrition. After debridement, her anal-rectal wound extended from labia to lef

The impact of synbiotic administration through in ovo technology on the microstructure of a broiler chicken small intestine tissue on the 1st and 42nd day of rearing.

BACKGROUND: According to research, fast skating on short distances increases functional asymmetry of leg muscles. As has been proven in many sporting disciplines, this asymmetry can increase the risk of injury. The aim of the study was to analyze the level of right and left myolectrical manifestations of fatigue asymmetry of gluteus maximus muscle in elite skaters on a short track and to compare this phenomenon to a control group. The muscles were chosen deliberately, due to their influence in maintaining the right position during training on ice. METHODS: The experiment compared a group of e

Effect of end-to-side repair of proximal nerve stumps of transected peripheral nerves on the development of neuroma (experimental study).

OBJECTIVE: Neuroma is a psychologically and physically disabling problematic condition without any current standard therapy. For that reason, we investigated whether end-to-side anastomosis of the proximal end of the transected nerve into the adjacent nerve will prevent the development of neuroma in different types of nerve injuries. STUDY DESIGN: In this study, hind legs of 18 Sprague-Dawley female rats were used. Six groups were formed. In group I, peroneal nerves were transected and its proximal end was attached end-to-side through the epineural window to the adjacent tibial nerve. In grou
