Size and composition changes in diaphragmatic fibers in rats exposed to chronic hypercapnia.

OBJECTIVE: To test the hypothesis that chronic hypercapnia changes the composition of the respiratory muscle by continuous augmentation of ventilation. MATERIALS AND METHODS: Eighteen male Wistar rats were housed in 10% CO(2) in air for 19 weeks, and their minute ventilation V(E) was measured every 6 weeks. The diaphragm, excited at 19 weeks of exposure, was classified as fiber type I, IIa, or IIb. Cross-sectional areas of individual fibers were measured. Fibers with a target-like appearance on reduced nicotinamide adenine dinucleotide-tetrazolium reductase (NADH-TR) stain also were counted.

Exacerbations of COPD: predictive factors, treatment and outcome.

Chronic obstructive pulmonary disease (COPD) is a leading cause of death, and constitutes a major medical and an increasing economic problem for acute and long term care. A low level of irreversible airway obstruction when in stable condition, hypercapnia, hypoxia, the presence of comorbid heart disease, right ventricular failure, and low serum albumin are the main factors related to risk of exacerbations. Bronchial infections, bronchospasm, left ventricular failure, pneumonia, pneumothorax and thromboembolism are described as the most frequent relapsing causes of COPD. During exacerbation, th

An aneurysm involving the axillary artery and its branch vessels in a major league baseball pitcher. A case report and review of the literature.

Baseball pitchers appear to be prone to aneurysms of the axillary artery and its branches. The cause is probably related to repetitive compression of or tension on the vessels at the level of the pectoralis minor muscle and the humeral head, which is exacerbated by the pitching motion. The incidence of aneurysms of the axillary artery and its branches among pitchers and other athletes is not known, nor is it clear whether pitchers who are at high risk of vascular injury can be identified before irreversible damage to the vessels has occurred. Perhaps patients who have documented compression or

Sleep apnea in patients with hemispheric stroke.

Sleep pattern and breathing in humans are altered following cerebrovascular accidents involving the brainstem. Sleep apnea is a well-established complication of stroke involving the brainstem. On the other hand, the effect of cerebral stroke on sleep and breathing has not been well defined. The diffuse cerebral symptoms such as cognitive deficits, depression or fatigue, after hemispheric stroke mimic those present in patients with sleep apnea. To define the breathing pattern in patients with stroke involving cerebral hemispheres without brainstem lesion and without the prior history of sleep-d

Human circulatory and thermoregulatory adaptations with heat acclimation and exercise in a hot, dry environment.

1. Heat acclimation was induced in eight subjects by asking them to exercise until exhaustion at 60% of maximum oxygen consumption rate (VO2) for 9-12 consecutive days at an ambient temperature of 40 degrees C, with 10% relative humidity (RH). Five control subjects exercised similarly in a cool environment, 20 degrees C, for 90 min for 9-12 days; of these, three were exposed to exercise at 40 degrees C on the first and last day. 2. Acclimation had occurred as seen by the increased average endurance from 48 min to 80 min, the lower rate of rise in the heart rate (HR) and core temperature and th

[Therapeutic effect of paromomycin sulfate in the treatment of Diphyllobothrium latum infections and an observation on the worm tissues affected by the drug].

Paromomycin sulfate (aminosidine) at a single dose of 32 to 53 mg/kg was orally given to 24 cases with proven diphyllobothriasis. Evaluation of efficacy of the drug was based on stool examination for the eggs after 3--4 weeks of treatment. The cure rate was obtained as 96% (23/24), and 30 worms were expelled from 24 patients. Only 1 unsuccessfully treated case of 34-year-old man was retreated at the same dose of the drug 3 weeks later to obtain the cure. Thirty worms were composed of a single worm each from 21 patients, 2 worms from a patient, 3 from 1, and 4 from 1. Scolices of 7 (23.3%) out

Theophylline: short term treatment.

In this review article on short term-effects of theophylline, results of studies with single administration of theophylline are compiled, supplemented by several studies with a therapeutic administration up to 4 Days. Data at hand show limited to moderate bronchodilatator effect of theophylline, which is comparable to beta 2-adrenergic drugs only in some studies using high dose regimen with theophylline levels of about 20 mg/l, i.e. with a considerable risk of unwanted side effects. In normal subjects no bronchodilatator effect is present at all, thus differing from beta 2-adrenergic drugs. In

A machine learning pipeline identifies non-motor symptoms as key contributors to sarcopenia in Parkinson's disease.

OBJECTIVES: Sarcopenia, characterized by the loss of skeletal muscle mass and physical function, is more prevalent among patients with Parkinson's disease (PD) and is linked to poor clinical outcomes. This study aimed to investigate clinical features associated with sarcopenia in PD, which was further validated using a two-stage modeling pipeline. METHODS: Clinical and demographic data were collected and compared between the sarcopenia and non-sarcopenia groups. The dataset was split into a training and a hold-out test set (70:30). Following an initial variable screening in the training set b

Frailty during polymyalgia rheumatica, giant cell arteritis and other inflammatory rheumatic diseases.

Recent data suggest a pathophysiological role of aging and immunosenescence during polymyalgia rheumatica (PMR) and giant cell arteritis (GCA), by definition rheumatic disease of the elderly. During aging, there is a decline in major physiological functions (immune system, muscle, cognitive, endocrine, cardiovascular, respiratory and renal functions), which combined with multimorbidity, environmental factors and polypharmacy can lead to frailty. Frailty is a clinical syndrome and dynamic concept including a pre-frailty stage; it reflects a reduction in physiological reserve capacities which al
