Herpes zoster (HZ) is the painful reactivation of latent varicella zoster virus infection. The incidence of HZ may be increased in some autoimmune diseases, including systemic lupus erythematosus (SLE). We examined the incidence and risk factors for HZ in a prospective cohort of patients with physician-diagnosed SLE compared to those diagnosed with non-inflammatory musculoskeletal conditions (MSK). After excluding participants with a history of prior HZ at enrollment, we followed 1485 SLE patients and 2775 MSK with semi-annual mailed questionnaires for incident HZ between 2001 and 2010. Age-ad

Joint arthropathy secondary to recurrent hemarthroses remains a debilitating complication of hemophilia despite the use of prophylactic factor concentrates. Increased vascularity and neoangiogenesis have been implicated in the progression of musculoskeletal disorders and tumor growth. We hypothesized that de novo blood vessel formation could play a major role in the pathogenesis of hemophilic joint disease (HJD). We observed a 4-fold elevation in proangiogenic factors (vascular endothelial growth factor-A [VEGF-A], stromal cell-derived factor-1, and matrix metalloprotease-9) and proangiogenic

BACKGROUND: Childhood routine immunization in Nigeria, like most developing nations, do not include vaccination against Haemophilus influenzae type b (Hib) infection. This is probably because infection with Hib is uncommon in children younger than two months due to passive acquisition of maternal antibodies which protects newborn till about four to six months of life.
OBJECTIVE: To illustrate a case of neonatal meningitis caused by Haemophilus influenzae and to highlight its other peculiarities.
METHODS: A 22-day old baby presented with excessive crying, refusal of feed, progressive abdomina

Fibromyalgia is a chronic functional illness that presents with widespread musculoskeletal pain as well as a constellation of symptoms including fatigue, cognitive dysfunction, sleep difficulties, stiffness, anxiety, and depressed mood. The diagnosis of fibromyalgia, similar to other functional disorders, requires that organic diseases are not causing the symptoms. Systemic and rheumatic diseases can be ruled out by a patient history, physical examination, and laboratory investigations. Because there are no specific laboratory tests for fibromyalgia, the 1990 American College of Rheumatology (

Between 1988 and 2006, 18 patients had a custom-made endoprosthetic replacement of the distal humerus for bone tumours at our institution. There were 11 primary malignant neoplasms, six secondary deposits, and one benign aggressive tumour. The mean follow-up was for 4.4 years (1 to 18.2). Complications occurred in nine patients and included aseptic loosening in three (16.6%), local recurrence in two (11%), infection in two (11%), neuropraxia of the radial nerve in one (5.5%) and a peri-prosthetic fracture in one (5.5%). Excision was inadequate in four patients (22%), all of which developed loc

Marfan syndrome is a multisystem connective tissue disorder usually associated with mutation in fibrillin, and occasionally with mutation in TGFBR1 or 2. The clinical diagnosis is made using the Ghent nosology, which will unequivocally diagnose or exclude Marfan syndrome in 86% of cases. Use of a care pathway can help implementation of the nosology in the clinic. The penetrance of some features is age dependent, so the nosology must be used with caution in children. Molecular testing may be helpful in this context. The nosology cannot be used in families with isolated aortic dissection, or wit

A novel pigmented dermatosis was observed in four unrelated boys, three of whom had insulin-dependent diabetes. Three patients were the offspring of consanguineous parents. All four boys had pigmented hypertrichotic patches or induration on the upper inner thighs, with variable involvement of the genitalia, trunk, and limbs. Two boys had episcleritis and orbital proptosis with similar facies and musculoskeletal abnormalities including clinodactyly, flat feet, and short stature. One child had paraaortic and inguinal lymphadenopathy and three patients had an enlarged liver and spleen. A large, s

PURPOSE OF REVIEW: This article reviews the systemic effects of chronic obstructive pulmonary disease in general and the musculoskeletal effects and cachexia in particular.
RECENT FINDINGS: Shift in muscle fiber type from I to II, reduction of capillary/mitochondria ratio, reduction of oxidative enzymes and oxidative capacity, alteration in amino acid profile, changes in respiratory muscle dynamics, activation of the ubiquitin-proteosome pathway with loss of contractile proteins are some of the factors found to cause muscle dysfunction in chronic obstructive pulmonary disease. Chemotaxis and

Osteopathic manipulative medicine (OMM) is known for its therapeutic potential on the musculoskeletal system, and its emerging role and potential benefits in oncology care are gaining attention. Patients with primary and metastatic bone cancer tend to experience pain, restricted movement, and lower quality of life due to the pathology and its treatment. This narrative review examines the mechanistic rationale and available clinical evidence supporting the use of OMM in the management of pain and functional impairment among patients with bone malignancies. A literature search was conducted usin…

Breast cancer treatments, including surgery and chemotherapy, may affect the musculoskeletal system even before the patient has received postoperative radiotherapy. This study aimed to investigate the viscoelastic properties of the pectoralis major (PM), upper trapezius (UT), and sternocleidomastoid (SCM) muscles in patients with breast cancer treated with neoadjuvant taxane-based chemotherapy and to examine their associations with surgery type, age, and body mass index (BMI). In this observational study, viscoelastic muscle properties of the PM, UT, and SCM muscles were assessed using the Myo…

Lower-limb impairment caused by neurological injury, musculoskeletal disorders, or reduced mobility often requires repetitive rehabilitation exercises, but continuous therapist-assisted therapy may be difficult to access in low-resource settings. In this study, we developed and preliminarily evaluated a low-cost, lightweight, portable hip-knee exoskeleton prototype capable of generating controlled hip and knee joint motion for rehabilitation-oriented range-of-motion training. The prototype was constructed mainly of 3D-printed polylactic acid parts and powered by a pair of high-torque servo mot

Knee osteoarthritis (KOA) is a prevalent musculoskeletal disorder characterized by pain, functional impairment, and structural degeneration. While quadriceps and hip abductor weakness are well-established in KOA, the role of hip extensor strength remains underexplored despite its biomechanical importance in lower limb alignment and joint loading. This cross-sectional study aimed to examine whether hip extensor strength differs between asymptomatic individuals and symptomatic KOA patients, and its relationship with pain, function, and radiographic severity. Adults aged 45-70 years ( N = 112; 56

Chronic low-intensity inflammation is emerging as a potential key mechanism in diseases that were previously considered degenerative, such as tendinopathies, osteoarthritis and intervertebral disc disease. This new approach recognises the role of structures such as the subchondral bone in knee osteoarthritis and the vertebral endplate in disc disease. In this sterile inflammatory context, plasma rich in growth factors (PRGF) a specific type of platelet-rich plasma (PRP) without leucocytes and activated with calcium chloride, has emerged as a promising therapy that exhibits reparative, anti-inf

BACKGROUND: The management of reproductive health in individuals with autoimmune rheumatic diseases (AIRDs) has evolved into a primary clinical priority. Wide variations in clinical practice and drug availability in the Asia-Pacific regions necessitate localized frameworks. These consensus statements, developed by the Asia Pacific League of Associations for Rheumatology (APLAR) Special Interest Group for Women's Health & Reproductive Issues in Rheumatic and Musculoskeletal Diseases, aim to bridge the gaps within the region, foster multidisciplinary collaboration, and address regional knowledge

BackgroundCentral sensitization (CS), characterized by amplified nociceptive processing within the central nervous system, is considered a key mechanism in chronic nonspecific low back pain (CNLBP). Although manual therapy (MT) is widely used in musculoskeletal rehabilitation, its effectiveness in patients with confirmed CS remains unclear. This study evaluated the effects of MT on CS, pain, disability, and quality of life in individuals with CNLBP.MethodsForty participants with physician-diagnosed CNLBP and CS (CSI ≥40) were randomly assigned to either an MT group (n = 20) or a conventional p

OBJECTIVE: Patients with knee osteoarthritis (KOA) exhibit proprioceptive deficits and changes in neuromuscular control, which may consequently lead to functional motor impairment. However, how altered proprioceptive feedback interacts with changed neuromuscular control after injury remains unclear. This study aimed to elucidate the spatiotemporal interaction patterns between lower-limb muscle activity and proprioceptive feedback in patients with knee osteoarthritis during walking, to reflect changes in their neuromuscular control mechanisms.
METHODS: Fifteen KOA patients and fifteen healthy
