Modified Marcy repair of large indirect inguinal hernia in infants and children.

Based on a review of operative notes of recurrent inguinal hernia cases from the authors' primary series, a surgical technique modified from the Marcy repair is described. With this technique, emphasis is placed on preservation of the intact internal spermatic fascia and reduction in the size of the internal inguinal ring. Through the inguinal approach, the sleeve-like extension of the internal spermatic fascia is incised longitudinally along the cord and up to the internal ring. The cord structures are dissected off the sac, and as much of the fascial tissue as possible is preserved intact. B

[Blood supply to the cervical spinal cord and possible therapeutic consequences in cervical spinal cord injuries].

The study was designed to demonstrate the blood supply to the cervical cord through radicular branches and was carried out in 23 human cadavers into which preparations were injected by the intra-articular route. One corrosion cast of the head and neck showed extravertebral anastomotic pathways. After removal of the vertebral bodies and discs from C1 to D3, the anterior spinal artery and radicular branches were identified and dissected back to their origins from the vertebral, deep or ascending cervical arteries. Twelve preparations had only one or two radicular branches reaching the cervical s

Pneumothorax, pneumomediastinum, pneumoperitoneum, and SCE after colonic ESD: a case report.

BACKGROUND: Endoscopic submucosal dissection (ESD) has become a widely accepted technique for en bloc resection of colorectal laterally spreading tumors (LSTs). Although the procedure is generally considered safe, complications including bleeding and perforation may occur. The simultaneous development of pneumothorax, pneumomediastinum, pneumoperitoneum, and extensive subcutaneous emphysema following colorectal ESD represents an exceptionally rare but potentially serious adverse event. CASE SUMMARY: A 56-year-old female patient underwent ESD for a granular-type LST measuring 8 mm × 20 mm loca

A rare, invasive, and challenging cancer in childhood: diffuse sclerosing variant of papillary thyroid carcinoma.

OBJECTIVES: Pediatric thyroid cancers are rare, with papillary thyroid carcinoma (PTC) being the most common type. The diffuse sclerosing variant (DSV) is a rarer subtype associated with frequent recurrence and extensive lymphatic involvement. Among 51 pediatric patients treated for thyroid carcinoma at our tertiary center over a 20-year period, three were diagnosed with DSV-PTC. We present this retrospective case series to highlight its clinical course and diagnostic challenges. CASE PRESENTATION: Three patients (aged 8-17) presented with neck swelling; one also exhibited high fever and neck

Laparoscopic treatment of Mirizzi syndrome with subtotal cholecystectomy and electrohydraulic lithotripsy: A case report.

Mirizzi syndrome is a rare chronic cholecystitis complication. However, the current consensus on managing this condition remains controversial, especially through laparoscopic surgery. This report describes the feasibility of treating type I Mirizzi syndrome with laparoscopic subtotal cholecystectomy and electrohydraulic lithotripsic gallstone removal. A 53-year-old woman presented with dark urine and right upper quadrant pain for 1 month. On examination, she was jaundiced. Blood tests showed highly elevated liver and biliary enzyme levels. Abdominal ultrasound showed a slightly dilated common

Coexistence of submandibular epithelioid angiosarcoma and papillary thyroid carcinoma: A case report.

INTRODUCTION: Reports on the coexistence of epithelioid angiosarcoma (EA) and papillary thyroid carcinoma (PTC) are rare. Over the past 50 years, only 2 cases of coexistence of EA and PTC have been reported in English literature. Therefore, we report a rare case of coexistence of EA and PTC treated with surgery and adjuvant radiation therapy. PATIENT CONCERNS: A 64-year-old man visited our hospital with a painless mass in the left submandibular gland, with poor mobility. DIAGNOSIS: Neck ultrasonography revealed nodules in the left submandibular gland and multiple cystic-solid mixed nodules i

Colossal Parotid Tumors: A Diagnostic and Surgical Challenge.

Tumors of the salivary glands constitute 3% of all head and neck tumors. The parotid gland (PG) is the most common site involved in 85% of cases. PG tumors' size varies from a few millimeters to several centimeters and is about 2-6 cm on average. However, because of insidious growth and asymptomatic nature, untreated tumors of the PG can attain large size. Pleomorphic adenoma (PA), as large as 33 cm in size or 26.5 kg in weight, has been reported in the literature. Similarly, untreated Warthin's tumor (WT) rarely becomes giant, size up to 20 cm is reported. Giant PG tumors are commonly symptom
