Cystic Fibrosis: Advancing Along the Continuum.

Cystic fibrosis (CF) is an autosomal recessive genetic disorder resulting from a mutation in the gene which encodes a cellular transmembrane protein channel known as the CF transmembrane conductance regulator. Located systemically on the surface of numerous cells, these altered channels yield multisystem dysfunction. Typical manifestations seen are chronic, progressive, obstructive lung disease, pancreatic insufficiency, CF-related diabetes mellitus, malabsorption and malnutrition, liver disease, and infertility.Once considered a pediatric disorder, through developments in innovative care and

Assistive technologies at home for people with a memory disorder.

The aim of this study was to assess in practice whether assistive technologies support and facilitate the work of a family caregiver or care staff, and whether these technologies support the independence of a person with a memory disorder. A comprehensive set of supportive devices and alarm systems were experimentally tested in the care of five test subjects in an assisted living facility by eight nurses, and in the care of four test subjects in a home environment by three family caregivers and one care team. Questionnaires, diaries and logged data were used to evaluate the benefits of the dev

Psychopathy: a confusing clinical construct.

Although psychopathy has traditionally been cited as a disorder of personality, confusion arises as the term is used interchangeably with the terms antisocial personality disorder and dissocial personality disorder, both of which are largely behaviorally based. This paper aims to provide an overview of the literature on the topic of psychopathy, which examines this conundrum. Included in the discussion are definitions of psychopathy, incidence, approaches to diagnosis, and the debates that surround causes, manifestations, and treatability. Experimental studies and theoretical papers have been

Falls associated with dementia: how can you tell?

In any practice setting, professional nurses must be skilled to assess and manage medical problems rapidly. Post-fall assessment among those with dementia is a daunting task. Emergent medical problems after a fall are difficult to detect among those with dementia, because impaired communication portrays a false reality that the older adult is uninjured. Furthermore, early detection of potential post-fall injuries may not occur within a health care system that relies on verbal communication and eyewitness accounts as the main source of fall occurrence information. Professional nurses must turn

Managing incontinence using technology, devices, and products: directions for research.

BACKGROUND: Millions of Americans with incontinence use some type of device or product to manage or collect urine or feces. However, research on their clinical uses, problems requiring nursing care, and patient satisfaction is lacking. OBJECTIVES: To review the various products and devices used for incontinence, identify directions for research and development on technology, and outline the ways nurses can influence and participate in those investigations. METHODS: Existing literature on incontinence technology, devices, and products was analyzed to generate a plan for future research. RESU

Central venous catheters: choosing the most appropriate access route.

Intravenous therapy and care is a complex and intricate area of practice, which is being subsumed into the core role of registered nurses. As more patients become recipients of a vascular access device (VAD), particularly those requiring intermediate to long-term central venous access, it is important to ensure that not only the device but also the most appropriate vein meets their clinical physiological and psychological needs. There is much research and literature on the insertion and care of central venous catheters (CVCs) as well as the detection and treatment of complications. However, ap

Barriers and facilitators to advance care planning implementation: a qualitative interprofessional study in the Spanish public health and social care system.

BACKGROUND: Advance Care Planning (ACP) is essential for improving the quality of end-of-life care. Despite having a well-established legal framework, the implementation of ACP in Spain is limited and largely bureaucratic. The existing literature has primarily focused on knowledge deficits among professionals, overlooking the emotional, cultural, and organisational dynamics shaping clinical practice. This study aimed to explore the experiences of healthcare and non-healthcare professionals regarding ACP implementation and to identify the multilevel factors that influence its integration into r

System of Psychological Support Based on Positive Suggestions to the Critically Ill Using ICU Doulas.

UNLABELLED: Surviving critical illness often creates a lasting psychological impact, including depression, anxiety, and post-traumatic stress. Memories of frightening and delusional experiences are the largest potentially modifiable risk factor, but currently, there is no proven intervention to improve these inciting factors. Psychological support based on positive suggestion is a psychotherapeutic approach that can be provided even to patients in altered cognitive states and is therefore a viable psychotherapy intervention throughout the ICU stay. Traditional ICU care team members have limite

The Effect of "Opt-Out" Regulation on Access to Surgical Care for Urgent Cases in the United States: Evidence from the National Inpatient Sample.

BACKGROUND: In 2001, the Center for Medicare and Medicaid Services issued a rule permitting states to "opt-out" of federal regulations requiring physician supervision of nurse anesthetists. We examined the extent to which this rule increased access to anesthesia care for urgent cases. METHODS: Using data from a national sample of inpatient discharges, we examined whether opt-out was associated with an increase in the percentage of patients receiving a therapeutic procedure among patients admitted for appendicitis, bowel obstruction, choledocholithiasis, or hip fracture. We chose these 4 diagn

Development of Home-Based Exercise Mobile Application for Patients with Peripheral Artery Disease.

This study aimed to develop a home-based exercise application to improve the exercising performance in patients with peripheral artery disease. This application is based on a theoretical framework and is paired with a wearable device, Fitbit Charge4. The application comprised five main categories with 30 screens: login, a record of exercise, goal achievement, information, and motivation. Using the application to nursing practice can manage and encourage patients to exercise without limitations of time and place.

Birth as Restorative.

A woman's past history of sexual trauma is inextricably woven into her pregnancy and birth experience. Two cases are presented by a practicing certified nurse midwife to demonstrate trauma-informed care in the childbearing setting. Providing trauma-informed care universally in the women's healthcare setting is imperative given not all women disclose their history. Empowering survivors may allow for a restorative birth, in which trust of one's own body can be regained.

Thyrotoxic Periodic Paralysis: A Puzzling Case of Chest Pain and Weakness.

Thyrotoxic periodic paralysis is an uncommon thyroid emergency that is associated with electrolyte disturbances and a progressive flaccid paralysis of lower and upper extremities. Although not typically diagnosed within the emergency department setting, advanced practice registered nurses may be key in identifying this unusual condition where rapid and appropriate treatment precipitated by hyperthyroidism, most commonly resulting from Graves' disease can mitigate adverse cardiac, renal, and neurologic sequelae.

Illustrations of Unexpected Infant Sleep Deaths.

Case illustrations from central Indiana provide the narrative for infant suffocations because of unsafe sleep environments. Accidental strangulation or suffocation in bed is caused by co-bedding, blankets and pillows in cribs, or wedging and entrapment. Knowledge of the evidence-based risks associated with case data may assist further in the prevention of unexpected infant sleep deaths and may better inform best practice for death scene investigation including forensic nurses.

Sexuality and multiple sclerosis.

The individual with multiple sclerosis (MS) experiences a severe alteration in lifestyle both physiologically and emotionally. Sexuality is an integral part of life. However, discussion of alterations in sexuality by health care workers has traditionally been viewed as taboo. Specifically, the sexual concerns in chronic debilitating disorders such as MS have been neglected. This paper will explore physical and psychological aspects of MS on a patient's sexuality. Implications for nursing practice will also be considered.

Postoperative functional rehabilitation for cervical spondylotic myelopathy: An integrative review guided by Orem's self-care deficit theory.

BACKGROUND: Cervical spondylotic myelopathy is the predominant cause of spinal cord dysfunction in adults, for which surgical decompression remains the mainstay of treatment. Surgical success alone does not guarantee functional restoration, and postoperative rehabilitation is a critical determinant of long-term outcome. A comprehensive, theory-grounded nursing framework to guide rehabilitation practice for this population has not been available. OBJECTIVES: To synthesise the current evidence on postoperative rehabilitation nursing for patients with cervical spondylotic myelopathy, to develop

How nursing students develop resilience through self-compassion and self-leadership: a mixed methods study.

BACKGROUND: Nursing students often experience high levels of stress due to various challenges during internships. To manage these challenges, students need to develop strategies, such as self-leadership and self-compassion. While the effectiveness of these factors has been explored, little research has investigated how students use them to remain resilient in adverse circumstances. OBJECTIVES: This exploratory mixed-methods study combining qualitative and quantitative research aimed to examine how nursing students apply self-compassion and self-leadership to foster resilience. Qualitative dat

Nurse-Led Interventions to Mitigate High-Alert Medication Errors in Critical Care: A Systematic Review of Effectiveness and Implementation Strategies.

BACKGROUND: Intensive care units (ICUs) are high-risk environments where nurses administer the majority of high-alert medications, making them the final checkpoint in the medication-use process. However, evidence on nurse-led interventions specifically targeting high-alert medication errors has not been systematically synthesized across all ICU populations. AIM: This systematic review evaluated the effectiveness of nurse-led interventions in reducing high-alert medication errors in neonatal, paediatric and adult intensive care units and examined implementation strategies that enhance patient
