Effectiveness, cost-utility, and benefits of a multicomponent therapy to improve the quality of life of patients with fibromyalgia in primary care: A mixed methods study protocol.

INTRODUCTION: Fibromyalgia (FM) is a chronic condition characterized by chronic pain, fatigue and loss of function which significantly impairs quality of life. Although treatment of FM remains disputed, some studies point at the efficacy of interdisciplinary therapy. This study aims to analyze the effectiveness, cost-utility and benefits of a multicomponent therapy on quality of life (main variable), functional impact, mood and pain in people suffering from FM that attend primary care centers (PCCs) of the Catalan Institute of Health (ICS). METHODS AND ANALYSIS: A 2-phase, mixed methods study

Efficacy of combined conservative therapies on clinical outcomes in patients with thumb base osteoarthritis: protocol for a randomised, controlled trial (COMBO).

INTRODUCTION: Management of thumb base osteoarthritis (OA) using a combination of therapies is common in clinical practice; however, evidence for the efficacy of this approach is lacking. The aim of this study is to determine the effect of a combination of conservative therapies for the treatment of thumb base OA compared with an education control group. METHODS AND ANALYSIS: This is a randomised, controlled, single-centre, two-arm superiority trial with 1:1 allocation ratio; with assessor and statistician blinded. Participants are blinded to the trial's hypothesis and to the interventions re

Developing an index for the orthodontic treatment need in paediatric patients with obstructive sleep apnoea: a protocol for a novel communication tool between physicians and orthodontists.

INTRODUCTION: Sleep disordered breathing in the paediatric population can manifest as an array of different systemic symptoms; among them is a distinct malocclusion and craniofacial phenotype. Emerging research suggests that the treatment of this malocclusion and/or craniofacial phenotype through orthodontic intervention may help with the symptoms of these patients. Selecting the patients who would benefit from orthodontic treatment can be a difficult task for the physician with minimal dental training. Therefore the aim of this study is to develop a simple index to be used by medical professi

Open versus arthroscopic decompression for subacromial impingement. A comprehensive review of the literature from the last 25 years.

The operative management (open versus arthroscopic) of subacromial impingement was investigated through a search of the English-language literature from 1970 to 1996. Thirty-four clinical studies comprising 1,935 patients met the following selection criteria: a study published in a peer reviewed journal, a valid materials and methods section (describing age, gender, number of subjects, follow-up period, treatment modality, and impingement stage), and acromioplasty without rotator cuff repair. Six hundred and ninety-eight patients had an open decompression (OD) and 1,237 had an arthroscopic sub

Impact of telemedicine on improving access to metabolic/bariatric surgery care in minority and other underserved patients with obesity.

BACKGROUND: Individuals who come from vulnerable groups such as those from racial or ethnic minorities and/or those from low socioeconomic groups have a high prevalence of obesity but are less likely to receive metabolic/bariatric surgery (MBS). This is believed to be related to inequities in access to MBS due to travel (those living in rural communities), financial burden, and/or lack of education. The objective of this study is to understand the impact of telemedicine interventions on improving access to MBS in such groups. SETTING: Academic Health Center. METHODS: A search was conducted u

Preschoolers' prosocial behavior in groups-Testing effects of dominance, popularity, and friendship.

Prosocial, other-benefitting behavior is a fundamental aspect of human social behavior. In the microsystem of their natural social groups, preschool children have to decide not only whether, but also whom to share with and help on a daily basis. In a study with 108 preschool children from five different childcare facilities in Vienna, we examined how individual measures of dominance and popularity of both the actor and the recipient, as well as their friendship influenced their prosocial behavior in a group setting. We assessed popularity and friendship with age-appropriate sociometric intervi

When the Past Is Backward and the Future Is Forward: An Embodied Cognition Intervention in Preschoolers with Developmental Language Disorder.

This quasi-experimental study investigates the impact of an embodied intervention on the semantics of transitive verbs in children with Developmental Language Disorder (DLD), grounded in the "TIME IS SPACE" conceptual metaphor-where the future is mapped as forward and the past as backward. The intervention involved a pretest and a posttest design, using the induced plasticity technique to saturate motor areas through repetitive arm movements (either forward or backward). Then, we determined the influence of this saturation on the auditory comprehension of past- and future-tense sentences. Fift

Follow-up of expressive language and general development at 12, 18 and 36 months for children with no canonical babbling at 10 months.

Little is known about the developmental trajectories in children who do not use canonical babbling (CB) at 10 months. The aim was to examine how speech, language, cognitive and motor abilities developed in children without CB. For 15 children identified as not having CB, consonant production, expressive vocabulary and general development were assessed at 12, 18 and 36 months from audio-video recordings. Twelve (79%) children still lacked CB at 12 months. At 18 months, six (40%) had parent-reported expressive vocabulary results below the 10th percentile, and two (14%) did not use dental/alveola

Effect of chrono-moxibustion and its influence on circadian rhythm for rheumatoid arthritis: A study protocol for a randomized controlled trial.

BACKGROUND: Rheumatoid arthritis (RA) is a chronic disease which is characterized by a circadian variation of key clinical symptoms and findings, with prominent joint swelling, stiffness and pain occurring in the early morning and light clinical symptoms during the day. Chrono-moxibustion is carried out at different time, which could result in dissimilar therapeutic effects. However, its efficacy has seldom been systematically demonstrated and few studies have reported that Chrono-moxibustion may regulate the circadian rhythm of RA. We therefore designed a randomized trial to explore the effec

Speech discrimination in infants at family risk of dyslexia: Group and individual-based analyses.

Deficiencies in discriminating and identifying speech sounds have been widely attested in individuals with dyslexia as well as in young children at family risk (FR) of dyslexia. A speech perception deficit has been hypothesized to be causally related to reading and spelling difficulties. So far, however, early speech perception of FR infants has not been assessed at different ages within a single experimental design. Furthermore, a combination of group- and individual-based analyses has not been made. In this cross-sectional study, vowel discrimination of 6-, 8-, and 10-month-old Dutch FR infa

Quantification of apparent axon density and orientation dispersion in the white matter of youth born with congenital heart disease.

BACKGROUND: White matter alterations have previously been demonstrated in adolescents born with congenital heart disease (CHD) using diffusion tensor imaging (DTI). However, due to the non-specific nature of DTI metrics, it is difficult to interpret these findings in terms of their microstructural implications. This study investigated the use of neurite orientation dispersion and density imaging (NODDI), which involves the acquisition of advanced multiple b-value data over two shells and provides proxy measures of apparent axon density and orientation dispersion within white matter, as a compl

Apheresis Medicine as a Multidisciplinary Subspecialty: International Models for Training, Quality, and Professional Societies.

Apheresis medicine comprises a group of extracorporeal therapies essential for the management of hematologic, renal, neurologic, immunologic, transplant-related, oncologic, and critical care conditions. Advances in technology, expansion of evidence-based indications, and integration into multidisciplinary care pathways have transformed apheresis from a technical service into a clinically complex subspecialty. We conducted a narrative review of published literature and international society documentation describing models for apheresis training, professional societies, quality assurance, and re

Aging in Place or Institutional Care? Exploring Daily Life Experiences and Attitudes Toward Nursing Home: A Comparative Qualitative Study on Older Adults.

BackgroundOlder adults may spend their later years living either in their own homes or in institutional care settings such as nursing homes. However, uncertainties persist regarding how different living arrangements are experienced and perceived by older adults. Addressing these uncertainties and ensuring that older adults are provided with environments that meet their needs requires careful consideration of their subjective perceptions and lived experiences within their current living contexts.ObjectiveThis study aims to explore and compare the daily life experiences and attitudes toward nurs

Motivations for testing and linkage to HIV services following HIV self-testing among fishermen in Kenya: a qualitative analysis.

HIV self-testing (HIVST) is a promising strategy to expand testing access for hard-to-reach populations such as Kenyan fishermen. We examined motivations and barriers to HIVST use and linkage to HIV prevention and treatment services (including PrEP or ART), within a social network-based intervention (NCT04772469). We conducted interviews (N = 64) and focus group discussions (N = 4) with fishermen in Siaya County, including 30 "promoters" who distributed HIVST kits and transportation vouchers within their social networks. Transcribed interviews were coded using framework analysis with a mixed i
