The authors describe a new flap to enlarge or create a first web in patients with congenital deficiencies of the hand. This lozenge-shaped flap is harvested from the dorsoradial aspect of the index finger based on a narrow proximal skin bridge protecting its axial vascularization. A dorsal skin graft of the donor site is avoided by closure using a rhomboid flap. In the reported series of 35 cases bone reconstruction of large diaphyseal defects was performed in two stages. The first stage was the insertion into the defect of a cement spacer which was responsible for the formation of a pseudosynovial membrane. The second stage was the reconstruction of the defect by a huge fresh autologous cancellous bone graft. A series of eight axillary skin defect reconstructions in seven patients using the lateral thoracic island fasciocutaneous flap is presented. BACKGROUND: Increasing demand exists for cosmetic correction of soft tissue contour defects. Treatments include simple tissue augmentation techniques or more complex surgeries with consequent relevant recuperation time for the patient. The search for new simple techniques to correct scars and age-related wrinkles and folds is therefore one of the main goals of cosmetic dermatologic surgery.
OBJECTIVE: To improve the cosmetic outcome of patients suffering from soft tissue contour defects by the use of a novel surgical instrument and technique, subcutaneous dissection by wire scalpel.

We present a modification of the standard linear closure for small to medium-sized defects of the lower nose that avoids many of the limitations of flap or graft reconstruction. The surgical technique detailed yields predictable functional and cosmetic reconstruction with very few complications. The dermatologic surgeon should consider this option when reconstructing defects of the lower nose.

A new technique for web reconstruction in congenital syndactyly is described. Twenty-one web spaces in 12 patients were treated using this method. The dorsal metacarpal flap was used as an island V-Y advancement to cover the newly created web space, thus avoiding skin graft in this space. A follow-up of 6 months to 2 years showed neither recurrence of the deformity nor web creep of any degree. The operation is rapid, easy, and reproducible. It is suggested that this technique can be used in all types of syndactyly.

Twenty patients with brachial plexus injury have been treated with one fascicle nerve transfer to the bicipital branch of the musculocutaneous nerve for reconstruction of elbow flexion. The mean distance from the medial epicondyle to the origin of this branch is 17.9 centimeters. The correlation coefficient and test of statistic significance between distance and heights of patients is 0.76 and 5.07 respectively (t18, 0.995 = 2.88). There is no correlation between the arm span, weight, age and the distance.

A case of bilateral facial atrophy diagnosed as atrophic connective tissue panniculitis is presented. Reconstruction of both cheeks was performed with two staged latissimus dorsi muscle flaps. The initial good result on the right cheek deteriorated as the disease continued to progress after surgery. The good result on the left cheek, however, remained stable. Detailed clinical examinations, laboratory analysis, and deep biopsies from the affected areas are important for accurate diagnosis. Reconstructive procedures should be delayed while the disease is still active.
