Sleep-related painful erections associated with obstructive sleep apnea syndrome.

Sleep-related painful erection is a rare syndrome recognized by reports of painful nocturnal erection, an association between REM sleep and pain, and the absence of pain during wakeful sexual activity. Approximately 30 cases have been reported in the literature. We add two more cases, each of which seemed to be associated with severe sleep apnea. Treatment of the apnea with Continuous Positive Airway Pressure device lessened the symptom in both men. Implications of this association are discussed.

Day-night pattern of autonomic nervous system modulation in patients with heart failure with and without sleep apnea.

INTRODUCTION: Among patients with congestive heart failure (CHF) both obstructive and central sleep apnea (SA) are associated with increased sympathetic activity. However, the day-night pattern of cardiac autonomic nervous system modulation in CHF patients with and without sleep apnea is unknown. MATERIAL AND METHODS: Twenty-five CHF patients underwent polysomnography with simultaneous beat-to-beat blood pressure (Portapres), respiration and electrocardiogram monitoring. Patients were divided according to the presence (SA, n=17) and absence of SA (NoSA, n=8). Power spectral analyses of heart …

Body position and breathing abnormalities during sleep: a systematic study.

BACKGROUND: Until now, studies about body position and nocturnal breathing abnormalities have been restricted to comparing supine versus lateral positions. OBJECTIVES: In this retrospective study, we systematically evaluated the effect of body position on nocturnal breathing in 105 patients with a sleep apnea hypopnea syndrome (SAHS). METHODS: All the patients had an apnea hypopnea index > 10/h, as judged from polysomnography performed in the sleep laboratory. A thoracic sensor allowed to detect nine distinct body positions: supine (S), supine right (SR), right (R), prone right (PR), prone (…

[Obstructive sleep apneas in symptomatic obese children: polisomnographic confirmation and its association with disturbances in carbohydrate metabolism].

INTRODUCTION: It has been shown that obesity is a risk factor for Obstructive Sleep Apneas (OSA) and that it could be related to insulin resistance (IR). OBJECTIVE: To establish the frequency of OSA in obese children and adolescents with suggestive symptoms of sleep disordered breathing (SDB) by polisomnografic study (PSG) and to clinically characterize the groups with and without OSA, and their association with IR. PATIENTS, MATERIAL AND METHODS: Descriptive, retrospective, cross-sectional study in patients with obesity and symptoms of SDB examined in the Hospital Nacional de Pediatría "Pro

Clinical utility of the Chinese version of the Pediatric Daytime Sleepiness Scale in children with obstructive sleep apnea syndrome and narcolepsy.

AIM: The present study examined the psychometric properties of the Chinese version of the Pediatric Daytime Sleepiness Scale (PDSS) and the utility of the PDSS as a screening tool for pathological daytime sleepiness in teenagers with obstructive sleep apnea (OSA) and narcolepsy. METHODS: The PDSS was first administered to 238 middle and high school students to assess the reliability of the scale, and then administered to 28 teenagers with OSA, 31 teenagers with narcolepsy, and 34 normal controls to evaluate its clinical utility. RESULTS: Test-retest reliability and internal consistency were

Inspiratory muscle dysfunction in patients with severe obstructive sleep apnoea.

Repetitive inspiratory effort against an obstructed airway and intermittent hypoxia may be deleterious to the inspiratory muscles in patients with obstructive sleep apnoea (OSA). We investigated muscular dysfunction by comparing the strength, endurance and fatigability of inspiratory muscles and knee extensors in patients with newly diagnosed severe OSA compared with matched controls. The measurements included strength and endurance tests of both muscles, and a fatigue trial with simultaneous surface electromyography of the diaphragm and the vastus lateralis during voluntary contractions and i

Alterations in sleep EEG activity during the hypopnoea episodes.

The Obstructive Sleep Apnoea Hypopnoea Syndrome (OSAH) means 'cessation of breath' during the sleep hours and the sufferers often experience related changes in the electrical activity of the brain and heart. The aim of this paper is to investigate any possible changes in the human electroencephalographic (EEG) activity due to hypopnoea (mild case of cessation of breath) occurrences by applying the non-linear and linear time series methods. The results from this one-subject study indicated significant changes in the human EEG activity due to hypopnoea episodes by applying the non-linear, Lyapun

Reticulocytes in untreated obstructive sleep apnoea.

BACKGROUND AND AIM: The short, repetitive hypoxaemic episodes observed in obstructive sleep apnoea (OSA) may determine small augmentations in mature red blood cells. It is unknown whether they affect reticulocyte release. This study explored whether the number and degree of maturation of circulating reticulocytes may be altered in OSA, possibly through the effect of erythropoietin. METHODS: Fifty male adult patients with suspected OSA, normoxic during wakefulness, were studied. After nocturnal polysomnography, a blood sample was withdrawn for blood cells count, erythropoietin, iron and transf…

Dream content analysis in persons with an autism spectrum disorder.

Dream questionnaires were completed by 28 young adults with autism spectrum disorder (ASD) participants. Seventy-nine typically developed individual served as the control group. In a subset of 17 persons with ASD and 11 controls matched for verbal IQ, dream narratives were obtained following REM sleep awakenings in a sleep laboratory. Questionnaires revealed that participants with ASD, compared to controls, had fewer recollections of dreaming, fewer bad dreams and fewer emotions. In the sleep laboratory, dream content narratives following REM sleep awakenings were shorter in ASD participants t

Respiratory apraxia in amyotrophic lateral sclerosis.

Respiratory dysfunction is a critical problem in amyotrophic lateral sclerosis (ALS). We report a patient with ALS who had respiratory apraxia. A 74-year-old female presented with progressive dysarthria and dysphagia. Clinical signs and evidence of widespread denervation on electromyography (EMG) confirmed the diagnosis of ALS. She had no signs of dementia. Irregular volitional inspiratory movements on verbal command were noticed, in contrast with rhythmic automatic inspiration - respiratory apraxia. Limb and buco-facial movements showed no signs of apraxia. EMG of respiratory muscles was norm

Physiology of sleep disordered breathing.

Obstructive sleep apnea (OSA) results from complex interactions between anatomy and physiology. A structurally small and abnormally collapsible upper airway predisposes to disease, and interacts with normal and pathologic physiologic mechanisms to determine severity of disease. Understanding the pathophysiology provides insight into airway collapse, and may improve treatment and lead to potential new medical and surgical treatments for OSA.

Sleep apnoea inducing hypoxemia is associated with early signs of carotid atherosclerosis in males.

The intima-media thickness (IMT) of carotid arteries as a marker of preclinical atherosclerosis was measured by ultrasonography in 49 subjects to determine, how strongly the obstructive sleep apnoea (OSA) syndrome is associated with atherosclerosis. Maximal IMT was higher in patients with cardiovascular diseases and with or without risk factors of atherosclerosis, presenting also OSA (apnoea-hypopnoea index=26.1+/-15.6/h) compared to controls without OSA (0.91+/-0.21 mm versus 0.77+/-0.18 mm, p<0.05). The prevalence of IMT > or = 0.85 mm was also higher in patients with cardiovascular patholog

New approaches to the study of periodic leg movements during sleep in restless legs syndrome.

STUDY OBJECTIVES: To describe a new approach for the analysis of quantity, type, and periodicity of the leg motor activity during sleep in patients with restless legs syndrome (RLS) and periodic leg movements (PLM). METHODS: The following parameters were taken into account for LM: duration, amplitude, area under the curve, sleep stage, side, interval, and bilaterality. The analysis of inter-LM intervals was carried out by drawing their distribution graphs. A new index evaluated their periodicity and was validated by means of a Markovian analysis. The differences in inter-LM intervals, LM dura

[Incidence, pathogenesis and importance of nocturnal hypoxaemia in patients with chronic obstructive pulmonary disease].

Noctural hypoxaemia (NH) in patients with chronic obstructive pulmonary disease (COPD) was first described over 40 years ago, however, its importance is not fully established. NH is defined most frequently as a drop in oxygen saturation below 90% and it occurs in almost all COPD patients PaO2 < 55 mmHg and in 30-40% of those with PaO2 < 60 mmHg. The main reason for NH is alveolar hypoventilation which is most pronounced during ohase REM. It is known that even short term episodes of NH increase pulmonary artery pressure, yet, the impact of chronic, isolated NH on pulmonary hypertension developm

Insomnia and daytime napping in older adults.

Insomnia, daytime sleepiness, and napping are all highly prevalent among the elderly, reflecting changes in sleep architecture, sleep efficiency, sleep quality, and circadian sleep-wake cycles. Insomnia is sometimes associated with subjective daytime sleepiness, as well as other clinical and socioeconomic consequences. The daytime sleepiness will at times lead to napping. Although napping is viewed as a common age-related occurrence, little is known about its benefits or consequences. Factors reported to be contributors to daytime napping include sleep-maintenance difficulty and sleep fragment
