The test-retest reliability and limits of agreement of the balance evaluation systems test (BESTest) in young people with intellectual disability.

Clinical tests for the assessment of postural balance in people with intellectual disability have been the most commonly used single or multi-item tests, but some tests have been developed, such as the BESTest. The purpose of the study was to evaluate the test-retest reliability and limits of agreement of the Balance Evaluation Systems Test (BESTest) in young people with intellectual disabilities. A descriptive cross-sectional study was conducted with 65 young people (ages 16-25 years) with intellectual disability. The participants completed the BESTest (27 items) twice. Intraclass correlation

Posturographic destabilization in eating disorders in female patients exposed to body image related phobic stimuli.

Human postural control is dependent on the central integration of vestibular, visual and proprioceptive inputs. Psychological states can affect balance control: anxiety, in particular, has been shown to influence balance mediated by visual stimuli. We hypothesized that patients with eating disorders would show postural destabilization when exposed to their image in a mirror and to the image of a fashion model representing their body ideal in comparison to body neutral stimuli. Seventeen females patients attending a day centre for the treatment of eating disorders were administered psychometric

Susceptibility to motion sickness is not increased following spinal cord injury.

BACKGROUND: There are two leading theories on the origin of motion sickness. One, the sensory conflict theory, states that sensory information provided by one sensory channel does not match the expected input from another channel; commonly, these two inputs originate in the vestibular system and the eyes. The second theory - the postural instability theory - states that motion sickness comes about not through sensory conflict, but through an inability to control one's posture. OBJECTIVE: Given that people with a motor-complete spinal cord injury cannot control their muscles below the level of…

The geometric curvature of the spine of runners during maximal incremental effort test.

This study sought to analyse the behaviour of the average spinal posture using a novel investigative procedure in a maximal incremental effort test performed on a treadmill. Spine motion was collected via stereo-photogrammetric analysis in thirteen amateur athletes. At each time percentage of the gait cycle, the reconstructed spine points were projected onto the sagittal and frontal planes of the trunk. On each plane, a polynomial was fitted to the data, and the two-dimensional geometric curvature along the longitudinal axis of the trunk was calculated to quantify the geometric shape of the sp…

Balance control improves following replacement of paroxetine with venlafaxine and levodopa in a case of microvascular dementia.

BACKGROUND: Postural instability is a concern in several neurologic conditions and also among the elderly. Dysfunction in serotonergic, noradrenergic, and dopaminergic pathways may be involved in the etiology of postural imbalance. OBJECTIVE: The objective of this case report was to quantify, using computerized posturography, substitution with venlafaxine, and later levodopa, in a suspected case of postural instability with paroxetine. CASE SUMMARY: Presented is an 86-year-old woman with frequent falls and a Parkinson-like syndrome of the lower limbs secondary to microvascular dementia. Paro

A practical guide to the differential diagnosis of tremor.

Tremor is, by definition, a rhythmic oscillation of a body part. It is the most prevalent movement disorder in clinical medicine, so doctors working in many specialities and in general practice can expect to encounter it. Most tremors can be classified on the basis of four observable clinical characteristics: anatomical pattern; the relative prominence of the tremor at rest, on maintaining a posture, and with action; tremor frequency; and tremor amplitude. A resting tremor suggests Parkinson's disease, and the diagnosis then depends on a judgement about whether the patient has other signs of p…

Familial orthostatic tachycardia.

PURPOSE OF REVIEW: Postural tachycardia syndrome is an autonomic disorder primarily of younger women. The patient population is heterogeneous, making diagnosis and treatment a challenge. A mutation in the norepinephrine (noradrenaline) transporter gene prompted further genetic analysis. RECENT FINDINGS: Eleven new mutations were found in the human norepinephrine transporter gene, although none were directly associated with postural tachycardia syndrome. The 5'-flanking -1012C --> T variant of the dopamine beta-hydroxylase gene was slightly increased and protection was associated with a reduce

"Bicycle test" of van Gelderen in diagnosis of intermittent cauda equina compression syndrome. Case report.

The authors describe a simple clinical adjunct to the routine neurological examination of patients with intermittent cauda equina compression syndrome. The "bicycle test" helps exclude intermittent claudication due to vascular insufficiency and frequently confirms the relationship of posture to radicular pain.

Human body-sway steady-state responses to small amplitude tilts and translations of the support surface - Effects of superposition of the two stimuli.

BACKGROUND: For humans, control of upright standing posture is a prerequisite for many physical activities. Experimentally, this control is often challenged by the motion of the support surface presented as tilt or translation, or some combination thereof. In particular, we have investigated subjects balancing in situations where tilt and translation stimuli were presented in isolation and compared it to a situation where such stimuli occurred simultaneously. RESEARCH QUESTIONS: Is the human posture control system in the case of two or more superimposed external disturbances responding to the…

A dystonia-like movement disorder with brain and spinal neuronal defects is caused by mutation of the mouse laminin β1 subunit, Lamb1.

A new mutant mouse (lamb1t) exhibits intermittent dystonic hindlimb movements and postures when awake, and hyperextension when asleep. Experiments showed co-contraction of opposing muscle groups, and indicated that symptoms depended on the interaction of brain and spinal cord. SNP mapping and exome sequencing identified the dominant causative mutation in the Lamb1 gene. Laminins are extracellular matrix proteins, widely expressed but also known to be important in synapse structure and plasticity. In accordance, awake recording in the cerebellum detected abnormal output from a circuit of two La…

[Tremor. Differential diagnosis and treatment].

Tremor is one of the most frequent neurological signs; the diagnosis is mainly clinical. The most frequent tremor is essential tremor, which manifests itself as a postural and kinetic tremor. Tremor may occur not only in the hands, but also in the head and voice. Parkinsonian tremor is a tremor at rest; the legs and face are frequently involved. Orthostatic tremor mainly manifests itself in the legs and gives rise to postural instability. Dystonic tremor is an action tremor of the affected region of the body. Drug therapy mostly depends on the clinical manifestation. Postural and action tremor

The influence of air friction in speed skating.

With the use of a wind tunnel the air friction force Fw on six speed skaters of different body builds was measured. The dependence of the drag coefficient CD on air velocity v and the influence of different skating postures on drag were investigated. At an air velocity of v = 12 m/sec, an angle between upper and lower leg of 110 degrees and a horizontal trunk position, the measured air friction constant kn(=Fw/V2) of all subjects was calculated from their height l and weight m according to the formula 0.0205 l3 square root m (standard error 2%). CD and as a consequence k appeared to be strongl…
