Pediatric Osteochondritis Dissecans of the Talus.

OCDs of the ankle are the third most common lesion in the body and a physician should include this in the differential while evaluating the pediatric population with the history of injury and/or pain in the ankle. Pain, stiffness, and swelling are the most common presenting complaints about the pediatric and adolescent patients with an OCD. Conservative treatment options should be attempted before attempting surgical intervention. Multiple surgical procedures exist to manage the lesion based on location, size, and other factors.

Interstitial cystitis: diagnosis and management.

Interstitial cystitis/painful bladder syndrome is a chronic condition that causes debilitating bladder pain which can be associated with urgency, frequency and nocturia. Its cause is not clear and it is still a disease diagnosed by exclusion. Oral or intravesical therapies are the main stay of treatment whilst surgical procedures are reserved for refractory cases. This condition usually warrants a multidisciplinary approach for optimum outcome. This article gives an overview of the changes in definition, aetiopathogenesis and available treatments.

[Neurophysiological and clinical aspect of patients with carpal tunnel syndrome].

In the group of patients with carpal tunnel syndrome (CTS), besides paresthesias in their fingers, we can see paresthesias in forearm; pain in elbow, sholder and neck, which is a reason for complicated diagnosis of it. This kind of complexnost of differential diagnosis pain in hand, is a reason for complet and strict diagnostic procedure. There are 30 patients with clinical and neurophysological findings for CTS in this paper who made provocative tests. Analising their results we made diagnosis of KTS.

Scaphoid malunion. The significance of rotational malalignment.

Three patients with painful malunions of the scaphoid and significant loss of active wrist extension were treated with an opening wedge multiplanar osteotomy that corrected flexion, ulnar deviation, and pronatory rotational malalignment of the distal fragment. After a minimum follow-up of 4 years, all three patients were satisfied with the procedure and were pain-free. The preoperative range of wrist motion had improved, and they had returned to their preoperative occupations.

Balloon compression of the Gasserian ganglion for the treatment of trigeminal neuralgia.

The authors describe the results of the treatment of trigeminal neuralgia by balloon microcompression of the Gasserian ganglion. One hundred and eighty-seven procedures were performed in 158 patients. There were no deaths or complications of clinical importance and pain was relieved in all patients. It was concluded that the treatment of trigeminal neuralgia by the microcompression method is safe, simple, cheap, and is especially recommended in cases of pain affecting the first division of the trigeminal nerve and in clinically ill or non-cooperative patients.

Abdominal pain resulting from cerebrospinal fluid pseudocyst and cholelithiasis.

Patients treated with ventriculoperitoneal (VP) shunt procedures for hydrocephalus may have abdominal pain related to complications stemming from the shunt tubing itself or to other intra-abdominal disease. In patients with VP shunts, usually children, lack of absorption of cerebrospinal fluid (CSF) may produce a painful abdominal CSF pseudocyst. We have reported the rare occurrence of a CSF pseudocyst in an adult. The patient was also found to have cholelithiasis.

Chronic traumatic trigeminal neuralgia.

Atypical facial pain or neuralgia and lower-half headache are confusing terms and should be discarded. Recurrent unilateral, throbbing, frontal headaches should be referred to as facial migraine. Patients whose trigeminal branches have been subjected to repeated surgical procedures and who have relentless unilateral face-jaw pain should be classified as having chronic traumatic trigeminal neuralgia. Effective treatment is available provided surgical manipulations cease.

Effect of preincisional ilioinguinal and iliohypogastric nerve block on postoperative analgesic requirement in day-surgery patients undergoing herniorrhaphy under spinal anaesthesia.

BACKGROUND: By choosing spinal anaesthesia instead of general anaesthesia, and by infiltrating the wound area with local anaesthetic the need for postoperative analgesics may be reduced. An ilioinguinal and iliohypogastric nerve block (IINB) in inguinal herniorrhaphy was, therefore, studied in a day surgery setting in combination with a spinal block. METHODS: One hundred ASA I-II adult patients scheduled for inguinal herniorrhaphy were given spinal anaesthesia with hyperbaric 0.5% bupivacaine. In a randomized and blinded fashion half of them received an IINB 5 min before the surgical incision

Pain Management During Intrauterine Device Insertion in Nulliparous Women: A Scoping Review.

This study investigates the various methods of pain management during the insertion of intrauterine devices (IUDs) in nulliparous women. Currently, the only recommended method of pain management is 800 mg of ibuprofen taken one hour before insertion of the IUD. However, women continue to experience pain during the procedure. A scoping review was conducted using CINAHL, Medline, Web of Science, and Embase with inclusion criteria being English peer-reviewed articles from the last 10 years, involving nulliparous women of at least 18 years of age. The research reveals that management to minimize p

T regulatory cells and TGF-β1: Predictors of the host response in mesh complications.

Polypropylene mesh is frequently used in urogynecology procedures; however, pain and mesh exposure into the vagina occur in ~10% of cases. Mesh-induced pain, which occurs with or without exposure, persists after removal in 50% of cases. Chronic pain history predicts poor response to mesh removal but only a fraction have this diagnosis. We hypothesize that mesh induced pain is correlated with fibrosis and failure to improve with a heightened inflammatory and fibrotic host response. Women undergoing mesh removal were offered participation in a mesh biorepository. Standardized questionnaires incl

A real-world evidence of a consecutive treatment of 42 spine-related pain using dorsal root ganglion-pulsed radiofrequency (DRG-PRF).

OBJECTIVES: Spine-related pain is oftentimes not managed satisfactorily by analgesic medications and physiotherapy. Pulsed radiofrequency targeting on dorsal root ganglion (DRG-PRF) is able to precisely relieve pain without permanent damage to nervous tissue. In this article, we provide a short-term result of DRG-PRF for spine-related pain. PATIENTS AND METHODS: A retrospective chart review of a consecutive of 42 patients who underwent PRF between 2015-2016 was conducted. All patients had received pharmaceutical treatment or physiotherapy before PRF. The diagnoses included cervicogenic headac

Endoscopist as a key determinant of patient pain: a sigmoidoscopy screening model for unbiased assessment.

BACKGROUND: Despite increasing standardization of training and growing attention to quality in lower gastrointestinal endoscopy, patient-reported pain during sigmoidoscopy varies considerably. This study explored whether this variability arises from differences in endoscopist performance or patient characteristics. METHODS: This cross-sectional study included unsedated sigmoidoscopies performed between 2014 and 2019 within a population-based screening trial. Neither patients nor endoscopists selected each other. Pain was self-reported on a Likert scale: no pain, mild, moderate, severe. Patien

Postoperative outcomes of combined interpectoral-pectoserratus versus combined deep-superficial serratus anterior plane blocks in minimally invasive coronary artery bypass surgery: A prospective randomized study.

Minimally invasive coronary artery bypass grafting (MICS-CABG) and minimally invasive direct coronary artery bypass (MIDCAB) reduce surgical trauma compared with sternotomy but remain associated with significant postoperative pain that may delay recovery and contribute to complications. Although regional analgesic techniques are increasingly used to reduce opioid requirements, comparative data on different block strategies are limited. This study evaluated the effects of combined interpectoral-pectoserratus and combined deep-superficial serratus anterior plane blocks on postoperative pain, ana

[Assessment of pain level and quality of life in children after otosurgical interventions].

UNLABELLED: A personalized approach is particularly important in modern pediatric practice, as postoperative pain and surgery-related negative emotions can have a significant impact on a child's physical, emotional and social functioning. Otosurgical interventions are typically accompanied by pain, limitations in daily activities, and temporary hearing loss. Due to this, quality of life assessment is an important component of postoperative monitoring. OBJECTIVE: To carry out a comparative assessment and analysis of pain and quality of life in children after middle ear surgery performed using
