Robotic repair of atrial septal defect: Pre-groove vertical right atriotomy approach.

OBJECTIVE: Minimally invasive repair using robotic systems has been accepted as an attractive alternative in the surgical repair of atrial septal defects (ASDs). Clear exposure of the entire rim of the ASD is often hindered during a robotic approach. Here, we describe an approach using vertical right atriotomy along the interatrial groove that may enhance the exposure of the ASD. METHODS: Patients who underwent ASD closure using the da Vinci Si surgical ystem (Intuitive Surgical) from 2007 to 2021 were included. During ASD closure procedures, right atriotomy was performed either conventionall

Single-Port Robotic-Assisted Excision of the Urachal Remnant in an Adult Female: A Case Report.

Urachal anomalies and their associated disease processes are quite rare in pediatric populations and even rarer in adults. Although often asymptomatic, patients with symptoms can be treated with a combination of surveillance, antibiotics, and sometimes surgical resection. In this case, we describe our experience using the single-port robotic approach for the excision of a symptomatic urachal remnant. The patient presented with a chief complaint of urinary frequency, dysuria, intermittent hematuria, and right flank pain. A CT scan of the abdomen and pelvis revealed a bladder wall thickening at

Total Venous Control and Vein-to-the-Right Superior Mesenteric Artery Approach in Robotic Pancreatoduodenectomy.

BACKGROUND: Robotic pancreatoduodenectomy is an increasingly accepted alternative for the treatment of pancreatic ductal adenocarcinoma (PDAC).1 However, the ability to perform a meticulous robotic-assisted superior mesenteric artery (SMA) dissection to obtain a margin-negative resection remains unknown.2 PDAC within the head of the pancreas (HOP) that involves the superior mesenteric vein (SMV) and portal vein (PV) requires total venous control (TVC) and a 'vein-to-the-right' (or anterior artery-first) approach to SMA dissection to minimize venous congestion and operative blood loss.3-5 Here,

Urinary Leakage after Robot-Assisted Radical Prostatectomy: Is Always Predictive of Functional Results?

INTRODUCTION: The aim of the study was to evaluate if and when the presence of radiological urinary leakages of vesico-urethral anastomosis, after robotic radical prostatectomy, could provoke urethral strictures or affect continence recovery. METHODS: We enrolled 216 patients, undergoing robot-assisted radical prostatectomy between January 2020 and December 2022 in three high-volume referenced centres for robotic surgery. Before removal of the bladder catheter, all patients underwent a cystourethrography in which the presence/absence of leakage was assessed at level of vesico-urethral anastom

Robotic-assisted surgical treatment of complex giant incisional hernias: A report of 3 cases.

Incisional hernia of abdominal wall is one of the most common complications following open surgery. Giant incisional hernia of abdominal wall is defined as having a maximum defect diameter >12 cm or a hernia sac volume-to-abdominal cavity volume ratio >20%. The Primary treatment for giant incisional hernia of abdominal wall is surgical repair; however, both open surgery and conventional laparoscopic surgery are associated with significant technical difficulty, high surgical trauma, frequent postoperative complications, and suboptimal outcomes. From May 2021 to November 2023, 3 patients with gi

Robot-Assisted Radical Cystectomy for Invasive Bladder Cancer With Vesicovaginal Fistula: A Case Report.

An 89-year-old woman was diagnosed with vesicovaginal fistula (VVF) during transurethral surgery for repeated bladder cancer recurrences. She was referred to our hospital for the treatment of VVF and pT1 invasive bladder cancer. Typical radical cystectomy procedures incising the vaginal wall are not suitable for her because of the high risk of disseminating tumors. Robot-assisted radical cystectomy with en bloc resection of the bladder, uterus, ovaries, and vagina was successfully performed without urine extravasation by dissecting the rectovaginal space to the pelvic floor, referencing the ro

Robotic transperitoneal versus retroperitoneal approach for anterior renal mass nephron-sparing surgery.

Robotic nephron-sparing surgery is traditionally performed via a transperitoneal (TP) approach. However, the retroperitoneal (RP) approach has gained popularity, particularly for posterolateral renal masses. The RP approach is associated with shorter operative time, less blood loss, and shorter length of stay, while preserving oncologic outcomes in selected masses. Here, we aim to assess the feasibility of the RP approach in excising anterior renal masses. Patients ≥ 18 years of age who underwent robotic nephron-sparing surgery for anterior renal masses were retrospectively identified (2008-20

Design and Reality-Based Modeling Optimization of a Flexible Passive Joint Paddle for Swimming Robots.

Rowing motion with paired propellers is an essential actuation mechanism for swimming robots. Previous work in this field has typically employed flexible propellers to generate a net thrust or torque by using changes in the compliance values of flexible structures under the influence of a fluid. The low stiffness values of the flexible structures restrict the upper limit of the oscillation frequency and amplitude, resulting in slow swimming speeds. Furthermore, complex coupling between the fluid and the propeller reduce the accuracy of flexible propeller simulations. A design of a flexible pas

A Delayed Presentation of Superior Mesenteric Artery Syndrome Treated With Robotic-Assisted Strong Procedure.

Superior mesenteric artery (SMA) syndrome, also known as Wilkie's syndrome, is a rare condition resulting from compression of the third portion of the duodenum between the aorta and the superior mesenteric artery. When symptomatic, this compression may result in nausea, vomiting, epigastric discomfort, and weight loss, requiring clinical attention and imaging to make the diagnosis. Typically, SMA syndrome presents in young females and is associated with an underlying condition such as anorexia nervosa, cachexia, postoperative development after scoliosis surgery, etc. In this report, we present
