Pembrolizumab-induced severe oral mucositis in a patient with squamous cell carcinoma of the lung: A case study.

BACKGROUND: Immune checkpoint inhibitors, such as pembrolizumab, a humanized monoclonal antibody against programmed death-1, elicit antitumor activity in various types of cancers, including lung cancer. However, pembrolizumab has been reported to cause diverse immune-related adverse events associated with T-cell activation. CASE PRESENTATION: We present the case of a 61-year-old man with advanced non-small cell lung cancer who was administered pembrolizumab as first-line treatment. After the first dose, radiotherapy was also administered because of rapid progression of dyspnea due to bronchia

Common benign and malignant oral mucosal disease.

BACKGROUND: Mucosal diseases of the oral cavity are relatively common, and patients often seek initial assessment from their general practitioner. OBJECTIVE: The aim of this article is to provide an overview of common oral mucosal diseases to help with formulating a differential diagnosis and stratifying the urgency of referral. DISCUSSION: Pathological mucosal conditions of the oral cavity and jaws commonly present as a mucosal ulcer or a white, red or pigmented lesion. In this review, the authors outline the most common conditions organised according to their clinical presentation and desc

Severe toxic effects of low-dose methotrexate treatment for placenta accreta in a patient with methylenetetrahydrofolate reductase mutations.

WHAT IS KNOWN AND OBJECTIVE: Low-dose methotrexate (LDMTX) has been widely used for many decades in clinical settings, with good safety profiles compared with those of high-dose methotrexate. LDMTX is also used as one of the off-label conservative therapies in treating placenta accreta (PA). Until now, only a few mild adverse drug reactions (ADRs) have been published after short-term use of LDMTX, and no severe cases have been reported. CASE SUMMARY: We present a case of a 30-year-old female who developed acute severe oral ulcerative mucositis with degree IV myelosuppression and degree III he

Brainstem astrocytoma as a neuro-Behçet's disease mimic.

A 58-year-old man with a history of recurrent aphthous ulcers since childhood was admitted to the hospital with acute neurological decline characterised by loss of motor dexterity, dysarthria, dysphagia and unsteady gait. MRI brain was significant for symmetrical hyperintense T2 fluid attenuated inversion recovery (FLAIR) in the corticospinal tracts, including parts of the pons and the mesodiencephalic junction. Though initial concern was for neuro-Behçet's disease, brain biopsy ultimately revealed a diagnosis of astrocytoma. This report demonstrates a mimic of neuro-Behçet's disease and the i

Same strategy for pitfalls of radiotherapy in different anatomical districts.

Oral mucositis (OM) and radiodermatitis (RD) are serious side effects of radiotherapy (RT), often leading to its suspension, need for analgesics, and enteral/parenteral nutrition. Laser therapy is becoming a recommended treatment option. This prospective study aimed at demonstrating the beneficial effect of class IV laser therapy (HPLT) on RT-induced OM, an off-label use of HPLT to treat RD in breast cancer patients. Fifty-three cancer patients, during/after RT and/or chemotherapy (CT), affected by OM, were treated with HPLT during four consecutive days (970 ± 15 nm, 5 W (50 %), 35-6000 Hz, 24

Histological changes of rat soft palate with exposure to experimental laryngopharyngeal reflux.

OBJECTIVE: The possible effects of laryngopharyngeal reflux (LPR) on laryngeal and otologic disorders have been studied in the literature. There have been no reports explaining the possible effects of LPR on the soft palate. Therefore, in this study, we investigated the histopathologic changes in the rat soft palate using an experimental model of reflux. SUBJECTS AND METHODS: Eighteen healthy 200-220-g 20-week-old Sprague-Dawley rats were used. The animals were divided into three groups according to exposure time (1, 4, and 12 week exposures), and four rats were examined as controls who had u

Oral fungal infections: an update for the general practitioner.

Oral candidosis is the most common fungal infection encountered in general dental practice. It manifests in a variety of clinical presentations which may mimic more sinister diseases, and can occasionally be refractory to treatment requiring the attention of an oral medicine specialist. Management of oral candidosis should always include a thorough investigation of underlying predisposing conditions, as the disease often presents when the patient is systemically compromised. This update highlights the pathogenesis, clinical presentation, and management strategies of oral Candidal lesions commo

Prevalence of Behcet's syndrome in patients with recurrent aphthous ulcerations in Brazil.

Behcet's syndrome (BS) is a clinical entity characterized by the appearance of recurrent oral and genital ulcers, in addition to a broad spectrum of clinical manifestations. The aim of the present study is to evaluate the prevalence of BS in patients with recurrent aphthous ulcerations (RAU) and to test the hypothesis that BS could be under diagnosed in Brazil. This is a cross-sectional investigation of the prevalence of BS based on the International Study Group for Behcet's Disease (ISGBD) criteria in patients with RAU attended in a stomatology service. Three hundred and six patients were att

Isolation and crystallization of a unique size category of recombinant Rabies virus Nucleoprotein-RNA rings.

In order to study the packaging of rabies virus RNA inside the viral nucleocapsid, rabies nucleoprotein was expressed in insect cells. In the cells, it binds to cellular RNA to form long, helical or short circular complexes, depending on the length of the bound RNA. The circular complexes contained from 9 up to 13 N-protomers per ring. Separation of the rings into defined size classes was impossible through regular column chromatographies or gradient centrifugation. The size classes could be separated by native polyacrylamide gel electrophoresis. A large-scale separation was achieved with a 4%
