Drug harm prevention needs among adolescents in Aotearoa New Zealand: findings from the Youth19 Survey.

BACKGROUND: Drug use (including tobacco, alcohol and illicit drugs) is a leading cause of premature death, health loss and health inequities in Aotearoa New Zealand. Effective prevention and early intervention have potential to reduce drug-related human suffering across the lifecourse, thus decreasing pressure on health and social services. METHODS: To inform policy and practice, we investigated drug harm indicators among secondary students in Aotearoa and identified sub-populations at greatest need. We used Youth19 survey data (N=7,721) to investigate five indicators related to e-cigarette,

Aging and street capital: Older men's status and strategies in street culture.

BACKGROUND: The growing population of older adults with long-term drug use has been largely overlooked in research. While most research on street culture have focused on younger men, the experiences of aging within these environments have received limited attention. This study seeks to address that gap. METHODS: We conducted 20 semi-structured interviews with men aged 55 or older with current and long-term drug use, in Malmö, Sweden. The data were analyzed using a thematic approach. RESULTS: Our analysis identified three overarching themes that illustrate the men's status and strategies with

Demographic Differences Among Adults and Emerging Adults With, Without, and in Recovery from Alcohol and Other Drug Use Disorders.

BACKGROUND: Nearly two thirds of adults who have experienced a substance use disorder (SUD) are in recovery. Differences in recovery from SUD remain understudied. We examined demographic characteristics related to SUD and recovery in a nationally representative sample. METHODS: Data were from the 2021 National Survey of Drug Use and Heath (unweighted N = 44,877). Binary logistic regressions compared demographic groups' odds of past-year SUD and lifetime recovery from SUD among adults and emerging adults (ages 18-25). RESULTS: Among all adults, 17.2% reported past-year SUD and 8.4% reported b

Performance of Xylazine Qualitative Immunoassays Designed for Drug Checking in Urine Samples.

OBJECTIVES: Xylazine is a public health threat for individuals who use drugs. Data informing xylazine concentrations are hampered by a lack of point-of-care urinalysis testing. This study evaluated the performance of qualitative immunoassay strips designed for drug checking in identifying xylazine in urine samples. METHODS: The ability of 2 noncleared xylazine qualitative immunoassay test strips (W.H.P.M. Inc, 500 ng/mL and Rapid Response [BTNX Inc., 1000 ng/mL]), developed for use in drug-checking paradigms, to detect xylazine in urine samples of persons presenting for opioid use disorder tr

Investigating Key Elements of Peer Support Programs Focused on Recovery and Reentry in Community-Based Organizations: A Qualitative Implementation Science Study.

BACKGROUND: Peer support programs are recognized as an evidence-based practice to effectively support individuals in recovery from substance use and reduce recidivism. Peer support is provided by trained individuals with lived experience who guide individuals navigating similar life challenges. Organizations design and structure peer support programs differently; some have developed unique approaches to meet the needs of the individuals they serve. This study provides guidance for implementing peer support programs within community based organizations with a focus on recovery or reentry after

CYP450-based reclassification of urinary metabolic ratio cutoffs for pain management and substance abuse treatment drugs.

Traditional 95 percent reference intervals (RIs) for urinary metabolic ratios (MRs) are useful for identifying unusually slow or fast metabolism, but these do not account for cytochrome P450 (CYP450)-mediated variability in drug metabolism. In this study, we re-evaluated MR cutoff limits for 18 metabolite-parent drug pairs using genotype-predicted phenotype frequencies reported by Zhu et al. These pairs include dextrorphan/dextromethorphan, oxymorphone/oxycodone, hydromorphone/hydrocodone, O-desmethyltramadol/tramadol, hydromorphone/-morphine, norbuprenorphine/buprenorphine, norfentanyl/-fenta

The Role of Lactate in Drug Addiction.

Drug addiction is defined as frequent exposure to and compulsive searching for psychotropic medications despite their negative effects, leading to relapse after a period of abstinence. The possible psychophysiological processes associated with drug addiction include significant changes in neurotransmission, presynaptic and postsynaptic plasticity, and the structure of the reward circuit and neurons in other corticolimbic regions. These neuroadaptation processes indicate significant modifications to the machinery involved in protein synthesis and neuronal energy balance. Lactate, which was prev

Recreational Drug-induced Cardiopulmonary Injury.

Recreational substance abuse and associated emergency department visits are rising in prevalence, with ever-increasing numbers of overdose-related fatalities. Recreational substances are primarily administered through intravenous and inhalational routes, which are favored owing to their quick and intense onset of action but are associated with higher risks of unintentional overdose and cardiopulmonary complications. Acute insults to the cardiopulmonary system range from infectious to inflammatory to thermal types, with long-term consequences that include emphysema, pulmonary fibrosis, pulmonar

GLP-1 agonists: a game changer in pain treatment and addiction.

Chronic pain imposes a significant healthcare burden, with treatments often limited by side effects, opioid dependency, and preventable surgeries. Emerging evidence suggests that glucagon-like peptide-1 receptor agonists (GLP-1RAs), developed for diabetes and obesity, may offer novel analgesia and treat drug-seeking behavior. This review examines the role of GLP-1RAs in pain management, focusing on inflammation, macrophage repolarization, oxidative stress, and dopaminergic pathways in substance use disorders. We conducted a literature search in PubMed and Embase (Ovid) from January 2000 to Feb

Does Aripiprazole Increase Compulsive Urges to Use Substances? Case Reports and Literature Review.

PURPOSE/BACKGROUND: Aripiprazole is commonly used to treat schizophrenia, bipolar disorder, and major depressive disorder and is preferred because of its relatively favorable side-effect profile. In 2016, the Food and Drug Administration released a warning regarding the risk of new impulse control problems with aripiprazole, including urges to gamble, binge eat, shop, and engage in sexual intercourse. These problems are rare but may cause significant harm if not recognized in time. METHODS: This report presents 2 clinical cases to hypothesize that aripiprazole may increase urges and compulsiv

BDNF gene polymorphisms and substance use disorders: a systematic review.

The development of substance use disorders (SUDs) is partly determined by genetic factors. Brain-derived neurotrophic factor (BDNF) underlies the neurobiological mechanisms of action of psychoactive substances (PASs) and development of SUDs, while genetic markers within the BDNF gene may be associated with a risk of SUDs and accompanied clinical manifestations. This is a systematic review of the relationships between single nucleotide polymorphisms (SNPs) within the BDNF gene locus and various aspects of SUDs. We searched, appraised, and summarized the research evidence of these associations f

Distal and proximal risk factors of problematic cannabis use associated with psychotic-like experiences.

BACKGROUND: Problematic cannabis use is associated with endorsement of psychotic-like experiences (PLEs) in non-clinical samples. However, little is known in regard to predictors of this relationship, which may be relevant to prevention and intervention. In the present research, we investigate impulsivity and cannabis use motives as potential distal and proximal risk factors for PLEs using conditional process analysis. METHODS: Using an online cross-sectional survey of N = 300 students, we assessed endorsement of PLEs using the Community Assessment of Psychic Experiences (CAPE), problematic c
