Left-sided approach for mitral valve replacement in a case of dextrocardia with situs solitus.

Mitral valve surgery in dextrocardia is technically challenging due to its anatomical malposition. Minor modifications are required in the surgical technique to counteract the problems during cannulation and exposure of the mitral valve. We report a case of a patient with dextrocardia, situs solitus, rheumatic heart disease, severe mitral regurgitation, moderate pulmonary artery hypertension, and severe left ventricular dysfunction who underwent mitral valve replacement using a two-stage right atrial cannulation with left-sided left atrial atriotomy, with the surgeon standing on the left side

Right- and left-ventricular strain evaluation in repaired pediatric Tetralogy of Fallot patients using magnetic resonance tagging.

Residual pulmonary insufficiency in post-repair Tetralogy of Fallot (rToF) patients often mediates biventricular dysfunction which is associated with long-term adverse clinical outcomes. The objective of this study was to demonstrate the presence of impaired left ventricle (LV) circumferential strain (CS) in pediatric rToF patients as compared to controls using cardiac magnetic resonance imaging (CMRI). Additionally, bivariate analysis between right ventricle (RV) and LV functional measures in rToF patients was performed to further characterize the interventricular interactions thought to medi

Influence of alteration in preload on the pattern of left ventricular diastolic filling as assessed by Doppler echocardiography in humans.

We examined the influence of alterations in preload on pulsed Doppler indexes of left ventricular diastolic function in 50 patients including 12 without cardiovascular disease, 29 with coronary artery disease, and nine with critical aortic stenosis. Micromanometer left ventricular pressure was recorded simultaneously with pulsed Doppler echocardiography of left ventricular inflow and M-mode echocardiography of left ventricular diameter. Chamber stiffness constants, kd and kv, were obtained from the diastolic pressure-diameter and pressure-volume relations, respectively. Relaxation was measured

Management of Post-Myocardial Infarction Right Ventricular Failure.

Right ventricular failure (RVF) due to an acute myocardial infarction (MI) has been associated with high morbidity and mortality. Initial treatment is guided by early recognition and prompt revascularization. Current management of post-MI RVF is built upon expert consensus and is also informed by RVF from other etiologies, including massive pulmonary embolism, left ventricular assist device-associated right ventricular dysfunction, postcardiotomy shock, etc.; this speaks to the limited data available on the specific management of RVF in acute MI. The goal of this review is to discuss the curre

Improvement in Clinical Outcomes With Biventricular Versus Right Ventricular Pacing: The BLOCK HF Study.

BACKGROUND: Sustained right ventricular (RV) apical pacing may lead to deterioration in ventricular function and an increased risk of heart failure, especially in patients with pre-existing systolic dysfunction. The BLOCK HF (Biventricular Versus Right Ventricular Pacing in Heart Failure Patients With Atrioventricular Block) trial demonstrated that biventricular-paced patients had a reduced incidence of a composite endpoint of death, heart failure-related urgent care, and adverse left ventricular remodeling. OBJECTIVES: In a pre-specified analysis, this study examined clinical outcomes, inclu

Orthostatic Hypotension in the Elderly: Contributions of Impaired LV Filling and Altered Sympathovagal Balance.

Orthostatic hypotension, which occurs in 5%-18% of the elderly, may contribute to age-related disability. While autonomic dysfunction and alterations of cardiac structure and function likely to impair postural maintenance of blood pressure are common in the elderly, these have not been jointly studied in large cohorts. The authors evaluated the association of orthostatic hypotension with echocardiographic measures of cardiac structure and function, and with autonomic function determined by analysis of heart rate variability, in a large population of community-dwelling elderly. A total of 5201

Percutaneous Left Ventricular Unloading During High-Risk Coronary Intervention: Rationale and Design of the CHIP-BCIS3 Randomized Controlled Trial.

INTRODUCTION: Percutaneous coronary intervention for complex coronary disease is associated with a high risk of cardiogenic shock. This can cause harm and limit the quality of revascularization achieved, especially when left ventricular function is impaired at the outset. Elective percutaneous left ventricular unloading is increasingly used to mitigate adverse events in patients undergoing high-risk percutaneous coronary intervention, but this strategy has fiscal and clinical costs and is not supported by robust evidence. METHODS: CHIP-BCIS3 (Controlled Trial of High-Risk Coronary Interventio

[Risk stratification in non ischemic cardiomyopathy: a case report--Case 5/2010].

HISTORY AND ADMISSION FINDINGS: A 65-year-old male patient with rapid increasing shortness of breath and newly diagnosed atrial fibrillation was admitted to our hospital. INVESTIGATIONS: The ECG revealed atrial fibrillation. Echocardiography showed severe decreased left ventricular function. The magnetic resonance imaging (MRI) scan confirmed the severe reduced left ventricular function with a two graded mitral regurgitation as well as a pronounced late enhancement in the posterobasal area of the interventricular septum. Cardiac catheterisation showed mild diffuse atherosclerosis of the coron

Subunit expression of the cardiac L-type calcium channel is differentially regulated in diastolic heart failure of the cardiac allograft.

BACKGROUND: Left ventricular diastolic dysfunction is a major cause of cardiac allograft failure. Multimeric L-type calcium channels (alpha1-, alpha2/delta-, and beta-subunits) are essential for excitation/contraction coupling in the heart. Their gene expression was studied in allografts that developed diastolic heart failure. METHODS AND RESULTS: mRNA levels of calcium channel subunits were measured by competitive reverse transcriptase-polymerase chain reaction in microbiopsy samples from the interventricular septum. Size and tissue variabilities between biopsy samples were assessed by deter

Heart Failure With Reduced Ejection Fraction: A Review.

IMPORTANCE: Worldwide, the burden of heart failure has increased to an estimated 23 million people, and approximately 50% of cases are HF with reduced ejection fraction (HFrEF). OBSERVATIONS: Heart failure is a clinical syndrome characterized by dyspnea or exertional limitation due to impairment of ventricular filling or ejection of blood or both. HFrEF occurs when the left ventricular ejection fraction (LVEF) is 40% or less and is accompanied by progressive left ventricular dilatation and adverse cardiac remodeling. Assessment for heart failure begins with obtaining a medical history and phy

[Implantation of a pacemaker and superior vena cava anomalies: value of peroperative angiocardiography].

Rare abnormalities of systemic venous return may be encountered during the implantation of endocardiac pacemakers such as persistent left superior vena cava. Our patient, a 66 year old female, had, primary cardiomyopathy with sinus node dysfunction, left bundle branch block and a cardiothoracic ratio of 0,70. After left subclavian venous puncture the catheter seemed to advance down the left border of the cardiac shadow and a false route was suggested? Angiocardiography was performed immediately and showed a double superior caval system and allowed identification of the right ventricular apex.

Clinical management of arrhythmogenic right ventricular cardiomyopathy: an update.

Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a genetic heart muscle disease characterized by the peculiar right ventricular (RV) involvement. Distinctive pathologic features are myocardial atrophy and fibro-fatty replacement of the RV free wall, and clinical presentation is usually related to ventricular tachycardias with a left bundle branch block pattern or ventricular fibrillation leading to cardiac arrest, mostly in young people and athletes. Later in the disease evolution, progression and extension of RV muscle disease and left ventricular involvement may result in right or b

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) in four calves.

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), or Bland-White-Garland syndrome, is a rare congenital malformation described in children and adults. In this condition, the left coronary artery, which normally originates from the left coronary sinus in the aorta, instead originates from the pulmonary trunk, which results in retrograde flow of blood away from the myocardium into the lower-pressure pulmonary artery. Myocardial hypoxic-ischemic injury results in cardiac dysfunction, failure, and eventually in patient death if not surgically repaired. This report de

Impact of Left and Right Bundle Branch Block on Left Ventricular Systolic Function and Myocardial Mechanics Assessed by Speckle-Tracking Echocardiography and Cardiac Magnetic Resonance Feature Tracking: A Systematic Review and Meta-Analysis.

BACKGROUND: Bundle branch conduction abnormalities are associated with ventricular dyssynchrony, impaired myocardial mechanics, and progressive systolic dysfunction. Advanced deformation imaging techniques, including speckle-tracking echocardiography (STE) and cardiac magnetic resonance feature tracking (CMR-FT), may identify subclinical ventricular dysfunction beyond conventional left ventricular ejection fraction (LVEF). We performed a systematic review and meta-analysis to evaluate conventional systolic function and myocardial deformation abnormalities associated with left bundle branch blo

[Left ventricular electrode placement in cardiac resynchronization therapy].

Cardiac resynchronization therapy (CRT) is an important component of the treatment of heart failure patients with reduced left ventricular (LV) ejection fraction and a widened ventricular complex due to bundle branch block. The positioning of the LV lead, which is placed in lateral branches of the coronary sinus (CS), is of particular importance, as optimal positioning is crucial for the success of therapy; poor or incorrect positioning can worsen the prognosis. CS cannulation and LV lead implantation is particularly challenging when learning the procedure, and overall, has a flatter learning

Prevalence and prognostic impact of abnormal left ventricular ejection fraction in Hemodialysis patients with end-stage renal disease.

BACKGROUND: Patients with end-stage renal disease (ESRD) face a significantly elevated risk of cardiovascular morbidity and mortality, with left ventricular (LV) systolic dysfunction and heart failure (HF) being major contributors. Reduced left ventricular ejection fraction (LVEF) defines LV systolic dysfunction and is closely linked to adverse outcomes. This study aimed to assess the prevalence of abnormal LVEF in ESRD patients receiving hemodialysis and to examine the prognostic significance of varying LVEF levels on mortality and cardiovascular outcomes. METHODS AND RESULTS: A retrospectiv

[Diagnosis and treatment of heart failure with reduced ejection fraction].

The diagnosis of heart failure with reduced ejection fraction (HFrEF) is based on clinical symptoms, such as peripheral edema or dyspnea and a left ventricular ejection fraction (LVEF) of 40% or less. Treatment is based particularly on the "fantastic four" of renin-angiotensin-aldosterone system (RAAS) inhibitors, preferential angiotensin receptor neprilysin inhibitor (ARNI), beta-blockers, mineralocorticoid receptor antagonists (MRA) and sodium-glucose-linked transporter 2 (SGLT2) inhibitors. These agents have a class I recommendation and show significant benefits in terms of mortality and ho

Parvovirus B19-associated myocarditis in children: A systematic review of clinical features, management and outcomes.

BACKGROUND: Parvovirus B19 (PVB19) has emerged as a relevant etiologic agent of paediatric myocarditis, particularly during recent epidemiological surges in Europe and the United States. Despite increasing recognition, current knowledge remains fragmented, and standardised diagnostic and therapeutic strategies are lacking. METHODS: We conducted a systematic review of the literature up to May 2025, including 40 studies encompassing 53 individual case reports, 107 patients from registry-based cohorts, and 4 tissue-based investigations. RESULTS: Clinical presentation was frequently fulminant, w
