Closed and penetrating abdominal injuries in Nigerian Igbos.

Fifty-two patients undergoing operations for abdominal injury were studied. Road traffic accidents were the commonest cause of injury. The spleen was the most commonly injured organ; injuries of several abdominal organs were rare. There were no deaths.

[Diagnosis and management of abdominal injuries (author's transl)].

Prognosis and mortality rate of abdominal injuries are substantially influenced by early diagnosis and thereby early definite operative treatment. Especially in cases of multiple traumata a subtle search for intraabdominal lesions in needed. 242 patients with abdominal traumata were analysed. 142 (= 58%) of them had contusions of the abdominal wall only, 83 (= 36%) had blunt and 17 (= 6%) had perforating abdominal injuries. In two-third of blunt traumata with organ lesions the operation was indicated only by clinical symptoms. The preoperative radiological examinations revealed organ lesions i…

[Sigmoid colon injuries caused by blunt abdominal trauma].

Circumferential seromuscular stripping of the sigmoid colon with denudation of the mucosal tube over a length of 4 to 10 cm has been observed in four patients after blunt abdominal trauma. Three of the patients had been driving a motor car, with two of them wearing safety belts. One patient was injured when he was caught between steel bars at work. In two cases the sigmoid lesion was associated with a tear in the mesoileum, leading to ischemic bowel necrosis in one. In one patient the lesion was found 5 days after the accident when he presented the symptoms of diffuse peritonitis caused by sec

[Management of Injuries to the Parenchymal Abdominal Organs].

The most common organs affected by abdominal trauma are the spleen and the liver, often in combination. Pancreatic injuries are rare. In the case of blunt abdominal trauma, which is much more common, a clinical and laboratory examination as well as sonography should be performed. In the initial assessment, the circulatory situation must be screened. If there is haemodynamic instability and presentation of free fluid, an emergency laparotomy is indicated. If the situation is stable or stabilised and a pathological sonography is present, it is essential to perform triphasic contrast enhanced com

[Luxatio cordis in blunt-trauma thoraco-abdominal injuries].

A 56-year old woman was admitted to the emergency ward after suffering a blunt thoraco-abdominal high-velocity trauma as she hit the wheel when her car drove into a tree. A laparotomy was performed because of haemodynamic instability and radiographic suspicion of a diaphragmatic rupture. Besides haemorrhage from liver and spleen injuries, an abdominal herniation of the heart through a ruptured pericardium and diaphragm was found. Haemostasis of liver, splenectomy and suturing of defects in pericardium and diaphragm resulted in a haemodynamically stable situation. A high index of suspicion of r

[Disorders of immunity in chest and abdominal injuries].

Under study were disturbances in the immune competent system in 245 patients with injuries of the chest and abdomen in dynamics of the early posttrauma period. Specific features of the disturbances were analyzed in the complicated and noncomplicated course of the trauma disease. The informative value and prognostic significance of certain indices are discussed as well as approaches to immune correction.

Predicting and preventing infection after abdominal vascular injuries.

Of 210 patients with major intra-abdominal vascular injuries, 111 (53%) survived more than 48 hours. Of these, 41 (37%) developed serious infections resulting in death or a hospital stay exceeding 14 days (mean, 55 +/- 49) in the 33 who survived. The most frequent serious infections were intraperitoneal and resulted in a 35% mortality rate (8/23). The 111 patients surviving 48+ hours were divided into two groups based on their initial E.D. BP and injuries. The "high-risk" patients (with no obtainable blood pressure on admission, five or more injuries or a colon injury with a systolic BP of 40

[Diagnostic peritoneal lavage in abdominal injuries. Our experience].

Personal experience is reported in the use of peritoneal lavage in closed abdominal trauma to provide rapid, definitive diagnosis of haemoperitoneum. The techniques to be employed in order to obtain accurate diagnosis without any of the complications reported in the literature are specified and the advantages of the technique in resolving diagnostic uncertainties are analysed.

Intra-abdominal injuries in the battered baby syndrome.

In the period 1961 to 1970 six battered children suffering from intraabdominal visceral injuries were treated in the children's hospitals of Manchester. In addition to the signs of visceral lesions all the children were found to have histories and physical signs clearly pointing to the diagnosis of the battered baby syndrome. Epigastric and small intestinal injuries predominated in these children, and in this respect the pattern of trauma differs from that seen in children injured in road traffic and other accidents. Delay in referral contributed strongly to the death of two of the battered ch

Don't Forget the Abdominal Wall: Imaging Spectrum of Abdominal Wall Injuries after Nonpenetrating Trauma.

Abdominal wall injuries occur in nearly one of 10 patients coming to the emergency department after nonpenetrating trauma. Injuries range from minor, such as abdominal wall contusion, to severe, such as abdominal wall rupture with evisceration of abdominal contents. Examples of specific injuries that can be detected at cross-sectional imaging include abdominal muscle strain, tear, or hematoma, including rectus sheath hematoma (RSH); traumatic abdominal wall hernia (TAWH); and Morel-Lavallée lesion (MLL) (closed degloving injury). These injuries are often overlooked clinically because of (a) a

Abdominal injuries without hemoperitoneum: a potential limitation of focused abdominal sonography for trauma (FAST).

BACKGROUND: Focused abdominal sonography for trauma (FAST) relies on hemoperitoneum to identify patients with injury. Blunt trauma victims ( … umothorax; hematuria; pelvic fracture; and thoracolumbar spine fracture. CONCLUSIONS: Up to 29% of abdominal injuries may be missed if BTVs are evaluated with admission FAST as the sole diagnostic tool. Consideration of examination findings and associated injuries should reduce the risk of missed abdominal injury in BTVs with negative FAST results.

Abdominal Injuries in the "Found Down": Is Imaging Indicated?

BACKGROUND: We sought to investigate the incidence of abdominal injuries in "found down" trauma patients to better understand the value of emergency department (ED) imaging. Found down patients are at high risk for injuries to the head or neck and low risk to the abdomen or pelvis, so imaging with CT of the abdomen/pelvis (AP) or Focused Assessment with Sonography for Trauma (FAST) is of questionable value. STUDY DESIGN: The trauma registry was queried over a 10-year period ending December 2013 for found down patients. Demographics, CT AP, FAST scans, and injuries were abstracted from the tra…

Intra-abdominal solid organ injuries: an enhanced management algorithm.

The organ injury scale grading system proposed by the American Association for the Surgery of Trauma provides guidelines for operative versus nonoperative management in solid organ injuries; however, major shortcomings of the American Association for the Surgery of Trauma injury scale may become apparent with low-grade injuries, in which conservative management may fail. Nonoperative management of common intra-abdominal solid organ injuries relies increasingly on computed tomographic findings and other clinical factors, including patient age, presence of concurrent injuries, and serial clinica
