We described a 15-year-old boy who underwent the catheter ablation for the nodoventricular (NV) tachycardia that had difficulty in differentiation from atrioventricular nodal reentrant tachycardia with upper common pathway. The modification of the fast pathway revealed an anterograde conduction of the NV fiber. We successfully performed the catheter ablation targeting for the right ventricular insertion site of the NV fiber.

We present a clinical case of a rare form of the WPW phenomenon due to anterograde conduction over fasciculoventricular accessory pathway in 20-year-old competitive athlete. The patient had no history of palpitations or syncope. ECG revealed shortening of PQ interval (112 ms) and wide QRS complex due to conduction via accessory pathway. To address the question of participation in competitive activity and the need for ablation the patient underwent endocardial electrophysiological study in the course of which we verified conduction via fasciculoventricular accessory pathway. The result of the s

BACKGROUND: Narrow QRS tachycardia or premature beat with bystander atrial activation or ventricular-atrial dissociation is an unusual arrhythmia that can present diagnostic and therapeutic challenges. When ventricular-atrial conduction is robust, these arrhythmias can be difficult to distinguish from atrioventricular nodal reentry.
OBJECTIVE: The purpose of this study was to describe the electrophysiology characteristics, diagnostic maneuvers, and treatment of these types of arrhythmias in a series of 7 cases.
METHODS: Standard multipolar recording and pacing electrodes were used during ele

Most of the tachycardia arising in the atrioventricular (AV) junction are reentrant in nature. The two most common variety are AV nodal reentry and AV reentry utilizing an accessory pathway of the Kent bundle type. Typically these tachycardias have narrow QRS complex and are regular but an associated right or left bundle branch block could result in a wide QRS complex. Other mechanisms for wide QRS in AV junctional tachycardia include: (a) antidromic reentry; (b) preexcited tachycardia using two accessory pathways; (c) AV nodal reentry with incidental accessory pathway participation; and (d) a

OBJECTIVES: This study set out to examine outcomes from pediatric supraventricular tachycardia ablations over a 20-year period. This study sought to examine success rates and repeat ablations over time and to evaluate whether modalities such as 3-dimensional (3D) mapping, contact force, and cryotherapy have improved outcomes.
BACKGROUND: Ablation of supraventricular tachycardia in pediatric patients is commonly performed in most congenital heart centers with excellent long-term results.
METHODS: Data were retrieved from the NICOR (National Institute of Clinical Outcomes Research) database in

A 50-year-old male patient with a history of severe valvular regurgitation underwent mitral and aortic valve replacement surgery 3 months ago. Preoperative 12-lead electrocardiogram presented atrial flutter (AFL) and atrial fibrillation. AFL complicated with ventricular pre-excitation was observed on current admission. The potential mechanisms underlying these changes were considered multifaceted, and valve replacement procedure may be a rare incentive factor.

A 44-year-old woman with a past history of surgical ablation of an accessory pathway presented to our facility with episodes of palpitations and dizziness. An ECG showed sinus rhythm with a prolonged PR interval and a preexcited QRS complex. An EP study revealed AV conduction through a postero-septal accessory pathway and a prolonged PR interval due to severe intra-atrial conduction delay.

Orthodromic atrioventricular reentrant tachycardia (AVRT) is the second-most-common form of supraventricular tachycardia (SVT) and is inducible in approximately 55% of individuals with Wolff Parkinson White (WPW) syndrome. Antidromic AVRT, where the accessory atrioventricular connection is used as the antegrade limb and the atrioventricular node serves as the retrograde limb of the circuit, has been clinically documented in less than 5% of patients with WPW syndrome and may be induced in less than 10% of these WPW cases in the electrophysiology laboratory. Left lateral pathways are considered

Atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular reentrant tachycardia in patients with accessory pathways (AP) are common supraventricular tachycardias. High long-term efficacy of about 97% (AVNRT) and 92% (AP) has been observed in children and adults. The risk of occurring atrioventricular block is low (0.4-0.8% during AVNRT, 0.1-0.2% for AP). Catheter ablation shows a lower efficacy of 87-93% and elevated atrioventricular block risk up to 10% in patient groups with complex congenital heart disease. Nonsynchronized ventricular activation during preexcitation or perma

Three cases are described in which accelerated atrioventricular conduction occurred during an acute myocardial infarction. The first patient, an 82-year-old woman, developed a WPW syndrome suggesting posterior right ventricular preexcitation, a pattern which persisted for four months until her death. An accessory bundle was found on autopsy. Fibrotic changes, associated with acute lesions (hemorrhage, polymorphonuclear infiltrates) were present in the atrioventricular node and His-Purkinje system. Two men, of 47 and 74 years, developed a short PR interval associated with supraventricular tachy

The authors report the case of a 56 year old man with paroxysmal reciprocating tachycardia. The participation of a right lateral Kent bundle, latent in sinus rhythm and with retrograde atrioventricular conduction during tachycardia was proved by : 1) the slowing of the tachycardia rhythm and lengthening of the ventriculo-atrial conduction time by 50 ms during right bundle branch block ; 2) atrial mapping during tachycardia showing right lateral atrial pre excitation ; 3) the spontaneous termination of some attacks after a blocked Hisian depolarisation. Analysis of the mechanisms of spontaneous

A case of Wolff-Parkinson-White syndrome type A with coexisting ipsilateral bundle branch block is presented. The diagnosis was suspected because of subtle electrocardiographic changes and was confirmed at electrophysiological study. Necropsy showed a vascular anomaly of the coronary sinus that contained bundles of myocardial muscle which crossed the atrioventricular ring at a site that was consistent with the predicted accessory pathway.

We present the ECG with broad QRS complex tachycardia and left bundle branch block (LBBB) morphology. There are negative P waves in inferior leads (II, III, aVF) and positive in V1 with the RP interval of 124-164 ms. The slow-slow atrioventricular nodal reentrant tachycardia (AVNRT) conducted with LBBB aberrancy was diagnosed. The differential diagnosis of slow-slow AVNRT from atrioventricular reentrant tachycardia using concealed posteroseptal accessory pathway is discussed.

This report describes the case of a 19-year-old patient with Ebstein's anomaly, who had an unusual combination of a right atriofascicular (Mahaim) and two ipsilateral right atrioventricular (Kent) accessory pathways participating in three types of antidromic and orthodromic reciprocating tachycardias in the absence of retrograde conduction over the bundle of His-atrioventricular node axis. All three pathways were ablated in a single session using temperature-guided radiofrequency current.

Sixty nine cases of paroxysmal supraventricular tachycardia (PSVT) were submitted to electrophysiological studies. The prevalent data in different types of PSVT are as follows: 49% are atrioventricular reentry tachycardia (AV-RT), 14.5% atrioventricular nodal reentry tachycardia (AVN-RT), 28% AV + AVN-RT, and 8.5% of other types. Altogether 77% in this group have atrioventricular accessory bypass, in which about half of them are concealed ones. Electrocardiographic characteristics during tachycardia give hints to diagnosis: (1) AV-RT has the fastest heart rate, with 62% greater than 200 beats/

This paper reports the authors' preliminary experience in catheter ablation by radiofrequency current of 46 accessory atrioventricular pathways in 45 symptomatic patients with supraventricular tachyarrhythmias resistant to medical therapy. With an average follow-up period of 5.5 +/- 4 months the global success rate was 80%, ranging from 96% in left lateral to 33% in antero septal accessory pathways. It increased from 65% in the first 20 patients to 88% in the last 25 patients. The average duration of radioscopy was significantly shorter in left lateral pathways than in other localisations (p <

BACKGROUND: The differential diagnosis between orthodromic atrioventricular reentry tachycardia (AVRT) and atypical AV nodal reentrant tachycardia (aAVNRT) is sometimes challenging. We hypothesize that aAVNRTs have more variability in the retrograde conduction time at tachycardia onset than AVRTs.
METHODS: We aimed to assess the variability in retrograde conduction time at tachycardia onset in AVRT and aAVNRT and to propose a new diagnostic tool to differentiate these two arrhythmia mechanisms. We measured the VA interval of the first beats after tachycardia induction until it stabilized. The

BACKGROUND: Catheter ablation of accessory pathways (APs) in patients with Ebstein anomaly (EA) has a higher recurrence rate than in subjects with normal hearts. Anatomic features could account for suboptimal ablation outcomes.
OBJECTIVE: The purpose of this study was to examine the right atrioventricular (AV) groove in autopsy hearts with EA, correlate with clinical data, and identify features relevant for catheter ablation.
METHODS: Thirty-three specimens with EA from our Cardiac Registry were examined. The right AV groove was inspected for gross anatomic features. Limited microscopy was p
