Acromioclavicular (AC) joint injury is a frequent diagnosis after an acute shoulder trauma - often found among athletes and people involved in contact sports. This injury occurs five times more frequently in men than in women, with the highest incidence in the 20- to 30-year-old age group. Patients usually complain of pain and tenderness over the shoulder, particularly over the AC joint.Depending on the degree of injury, the clavicle may become prominent on the injured site. The original classification was described by Rockwood and Green according to the injured ligament complex and degree and di

BACKGROUND: Acromioclavicular (AC) joint injuries, particularly sprains, are common in athletic populations and may result in significant time lost to injury. However, surprisingly, little is known of the epidemiology of this injury.
PURPOSE: To define the incidence of AC joint injuries and to determine the risk factors for injury.
STUDY DESIGN: Descriptive epidemiological study.
METHODS: A longitudinal cohort study was performed to determine the incidence and characteristics of AC joint injury at the United States Military Academy between 2005 and 2009. All suspected AC joint injuries were

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2006eng
A healthy 75-year-old man presented with a 9-month history of an enlarging painless shoulder mass. On examination, there was a 10 x 10 cm firm mass overlying the acromioclavicular (AC) joint. The active range of motion of the shoulder joint was 130 degrees of forward elevation, 90 degrees of abduction, 60 degrees of abduction and internal rotation to the level of the waist. There was weakness of the rotator cuff. Plain radiographs showed degenerative changes of the AC joint with superior migration of the humeral head. Magnetic resonance imaging showed a large cyst arising from a degenerative A

BACKGROUND: Standard open acromioclavicular (AC) stabilization is associated with increased postoperative complications including deltoid injury, infection, tunnel complications, loss of reduction, and wound/cosmetic concerns. Arthroscopy may offer superior visualization and advantages that limit these risks. The aim of this prospective non-randomized study is to evaluate advantages and long-term reliability of arthroscopic AC stabilization.
METHODS: Thirty-two patients with acute grade III, IV and V AC dislocations underwent arthroscopic AC reconstruction with long-term assessment by clinica

BACKGROUND: Injuries to the acromioclavicular (AC) joint are common and should be suspected in patients who have shoulder pain in the region of the acromion and clavicle. Injuries to the AC ligament can cause horizontal instability and are often neglected or underdiagnosed, which can lead to poor patient outcomes.
PURPOSE: To perform a systematic review of the literature on the diagnosis and treatment of horizontal instability of the AC joint.
STUDY DESIGN: Systematic review.
METHODS: The authors performed a systematic review using the PRISMA (Preferred Reporting Items for Systematic Review

BACKGROUND: We evaluated interobserver and intraobserver reliability of the classification and treatment of acromioclavicular (AC) joint dislocations and assessed the impact of adding 3-dimensional computed tomography (3D CT) on the reliability of classification and treatment choice.
METHODS: Ten surgeons independently reviewed plain radiographs and 3D CT in 28 cases with AC joint dislocation. Images from each case were randomly presented to the observers, with plain radiographs alone being presented first, followed by plain radiographs plus 3D CT 2 weeks later. Four weeks later, they repeate

The aim of this review is to address controversies in the management of dislocations of the acromioclavicular joint. Current evidence suggests that operative rather than non-operative treatment of Rockwood grade III dislocations results in better cosmetic and radiological results, similar functional outcomes and longer time off work. Early surgery results in better functional and radiological outcomes with a reduced risk of infection and loss of reduction compared with delayed surgery. Surgical options include acromioclavicular fixation, coracoclavicular fixation and coracoclavicular ligament …

Septic pyogenic arthritis of the acromioclavicular joint is a rare entity that occurs in immunosuppressed patients or those with discontinuity of defense barriers. There are only 15 cases described in the literature. The diagnosis is based on clinical features and the isolation of a microorganism in synovial fluid or blood cultures. The evidence of arthritis by imaging (MRI, ultrasound or scintigraphy) may be useful. Antibiotic treatment is the same as in septic arthritis in other locations. Staphylococcus aureus is the microorganism most frequently isolated. Our objective was to describe the …

Injuries to the acromioclavicular joint are among the most commonly occurring problems in the athletic patient population. However, these injuries are often confused with other problems associated with the shoulder complex. This confusion was noted by Hippocrates (460-377 BC), who realized that acromioclavicular dislocation often was misdiagnosed as a glenohumeral injury. Galen (129-199 AD) experienced an acromioclavicular dislocation and could not tolerate the tight bandaging recommended at the time and thus became one of the earliest noncompliant patients. The understanding of acromioclavicu…

A hybrid acromioclavicular joint fracture-dislocation in which an intra-articular fracture of the distal clavicle coexisted with a coracoclavicular ligament injury is described. The proposed mechanism of injury is a fall on the point of the shoulder that simultaneously drives the scapula both anteriorly and inferiorly. Forced anterior scapular protraction caused a displaced horizontal plane fracture of the inferior half of the distal clavicle, which was separated from the remainder of the clavicle. The inferiorly acromioclavicular ligament was still attached to this distal clavicle fragment. T…

Synovial chondromatosis involving the acromioclavicular joint is very rare. We report a case of synovial chondromatosis of the acromioclavicular joint associated with synovial cyst formation. Arthrography of the acromioclavicular joint revealed that the loose bodies lay within a synovial cyst which was in communication with the acromioclavicular joint. Synovectomy including removal of the synovial cyst and the loose bodies relieved symptoms completely. The patient was still pain- and recurrence-free at 4 years' follow-up.

Synovial chondromatosis involving the acromioclavicular joint occurred in a 20-year-old woman. Synovectomy including removal of loose bodies and resection of the distal end of the clavicle relieved symptoms. Synovial chondromatosis is a relatively rare disorder and seems not to have been reported in the acromioclavicular joint.

Tracking movement in the scapular plane and electromyograms from the upper trapezius, rhomboideus, middle deltoideus, and pectoralis major muscles were analyzed during the movement in ten subjects who had grade II dislocation of the acromioclavicular joint. The purpose of this study was to clarify whether or not the dislocation of the acromioclavicular joint affects the rhythmic movement of the shoulder joint, and to evaluate objectively fatigue on the shoulder girdle of the dislocated side. The dislocation of acromioclavicular joint affects the fast rhythmic movement of the shoulder joint fro…

BACKGROUND: Acromioclavicular joint (ACJ) space narrowing has been considered to be an important diagnostic image parameter of ACJ osteoarthritis (ACJO). However, the morphology of the ACJ space is irregular because of osteophyte formation, subchondral irregularity, capsular distention, sclerosis, and erosion. Therefore, we created the ACJ cross-sectional area (ACJCSA) as a new diagnostic image parameter to assess the irregular morphologic changes of the ACJ.
AIM: To hypothesize that the ACJCSA is a new diagnostic image parameter for ACJO.
METHODS: ACJ samples were obtained from 35 patients …

We present the case of a 32-year-old female who sustained a left acromioclavicular (AC) joint type V injury and brachial plexus injury. The patient's AC joint injury was identified 6 days after she was involved in a motorbike accident where she sustained multiple other injuries. She required operative fixation of the AC joint using a locking compression medial proximal tibial plate. At 3 months post operatively, the patient was found to have a subluxed left shoulder as a result of an axonal injury to the upper trunk of the brachial plexus. In addition, the tibial plate had cut out. The plate w

Acromioclavicular joint (ACJ) injuries represent a common injury to the shoulder girdle. In the management algorithm of acute ACJ injuries complete radiological evaluation represents the key to a successful therapy. According to the classification of Rockwood the presence of a horizontal component in addition to vertical instability has to be detected. Using axillary functional views or Alexander views dynamic horizontal ACJ instability can be diagnosed in a simple, efficient and cost-effective manner reducing the number of mis-/underdiagnosed ACJ injuries. MRI should not be the imaging modali…
