Effect of aftercare regimen with extensor tendon repair: a systematic review of the literature.

Over the past several years, advances in the treatment of extensor tendon injuries have focused mainly on changes to postoperative protocols. Traditional static splinting has been found often to result in loss of flexion as well as extension lags at the metacarpophalangeal and interphalangeal joints. In addition, early motion, once thought to increase the risk of tendon rupture, has shown encouraging results. The purpose of this article was to conduct a systematic review of the literature to determine the optimal postoperative protocol following extensor tendon repair. A systematic review was

Effects of halfway house placement on retention of patients in substance abuse aftercare.

One hundred twenty-four male veterans were enrolled in the Birmingham VAMC Outpatient Substance Abuse Clinic (OSAC) aftercare program following inpatient treatment for alcohol, cocaine, or mixed alcohol-cocaine abuse/dependence. Forty-two of the patients were concurrently admitted to a nearby halfway house (HH) while the remaining 82 patients made community-based (CB) living arrangements. Chi-square analysis showed the two groups were matched, p > .05, in regard to age, race, marital status, presenting problem, and referral source. The CB group experienced significantly, p < .05, higher early …

Impact of a Pharmacist-Led Emergency Department Urinary Tract Infection Aftercare Program.

BACKGROUND: Current evidence demonstrates that a significant proportion of prescriptions for antibiotics that originate from the emergency department (ED) are inappropriate. Urinary tract infections (UTIs) are a frequent indication for prescribing an antibiotic in the ED. The Veterans Affairs Greater Los Angeles Healthcare System (VAGLAHS) piloted a pharmacistled ED aftercare program to promote appropriate antimicrobial management of outpatient UTIs. METHODS: A single center, retrospective chart review included veterans discharged with an oral antibiotic for UTI treatment from the VAGLAHS ED …

Community psychiatric nurse aftercare for alcoholics: a five-year follow-up study.

The aim of this study was to determine if community psychiatric nurse (CPN) aftercare for 1 year improved the 5-year outcome in patients following inpatient treatment for alcohol dependence. A 5-year follow-up study, observer blind, with non-random allocation of subjects to aftercare by CPN for 1 year or standard outpatient care, was used. Subjects had all received inpatient treatment for 6 weeks in a rural alcohol treatment unit. Subjects were traced and assessed in the community 5 years after the index admission. The participants consisted of 127 white male alcoholics. All were first admissi…

A qualitative study of experiencing cauda equina syndrome and its aftercare in the UK.

STUDY DESIGN: Qualitative, semi structured interviews. OBJECTIVES: Cauda Equina Syndrome (CES) is a neurological emergency that can cause permanent disability to the lower limbs, including pain, weakness, and bladder, bowel and sexual dysfunction. There is little evidence on the lived experience of patients with different severities of CES. This study sought to address this. SETTING: The interviews were conducted with persons who had experienced CES and been operated on for this condition in the UK. METHODS: A sampling frame was used on a pre-existing database to select a maximum variation

Integrated IT Platform for Coordination of Diagnosis, Treatment, and Aftercare of Prosthetic Joint Infections.

BACKGROUND/AIM: Prosthetic joint infections (PJI) are difficult to diagnose and treat. For a correct diagnosis, an array of information has to be processed and weighted. Successful treatment depends on the diagnosis, timing, and surgical strategy paired with treatment of the infectious agent. The complexity and interdisciplinarity needed cause difficulties concerning decision-making, the communication between disciplines, and the execution of a treatment strategy. The aim of this study was to develop a software platform to enhance the collection of information for the diagnosis of PJI, the int

[Compression glove has advantages in the functional aftercare of distal radius fractures].

INTRODUCTION: The beneficial effect of compression treatment for soft tissue swelling is undisputed. Yet it is only a subordinated option in trauma therapy and is used if at all preferably for the lower extremities. In a preliminary study we were able to show that fitted compression gloves had a positive effect. MATERIALS AND METHOD: In a prospective randomised study the effect of compression treatment on the rehabilitation period was examined. RESULTS: In this study including 57 patients it was shown that due to a quicker reduction of swelling, measured via the circumference and volume of t

Guideline-Based Cochlear Implant Care: A Model for Structuring Delegated Technical Aftercare.

Cochlear implant (CI) care in Germany is largely standardized through the national CI clinical practice guideline, the CI White Paper, and established certification procedures. Lifelong aftercare is essential for maintaining long-term treatment outcomes. In particular, technical aftercare may, in accordance with current guidelines, be delegated by a cochlear implant providing department (CIVE) to qualified external cooperation partners (QEK). To preserve the CIVE's overall responsibility, a clearly structured framework for collaboration is required. The aim of this manuscript is to present a p…

Communication barriers in aftercare: a qualitative study of allogeneic stem cell transplant patients.

BACKGROUND: Effective communication between patients and healthcare providers is a crucial component in aftercare after allogeneic hematopoietic stem cell transplantation (alloHSCT). Despite significant advances, unmet physical and psychological needs due to communication failures persist. OBJECTIVE: We performed a multicenter cross-sectional study assessing domains not communicated and underlying causes to identify and comprehend communication gaps in alloHSCT aftercare. STUDY DESIGN: Semi-structured in-depth interviews were conducted with patients after alloHSCT at the transplant centres R…

AI-assisted PEG aftercare education for older adults: clinician-informed chatbot design (PEGAssist).

PURPOSE: To evaluate whether a geriatric-focused, ChatGPT-based chatbot (PEGAssist) provides clinically adequate and comprehensible guidance for percutaneous endoscopic gastrostomy (PEG) aftercare in older adults. We examined whether its answers met expert expectations for depth/clinical usefulness, clarity/actionability, and scientific accuracy, with emphasis on complication recognition and triage. METHODS: A multidisciplinary panel (geriatrics, nursing, surgery) independently rated chatbot responses to a curated set of common PEG aftercare questions spanning education, routine care, trouble…

Your team is counting on you: Digital gamification and team motivation in alcohol aftercare.

BACKGROUND: Relapse after inpatient treatment for alcohol dependence is frequent, particularly within the first three months post-discharge. Cognitive Bias Modification (CBM), and specifically alcohol Approach Bias Modification (ABM), has demonstrated benefits in inpatient care, but its efficacy for aftercare remains uncertain. METHODS: A two-phase research program was conducted. The pilot study tested the feasibility of a desktop-based, home-delivered ABM. A total of 308 patients were randomized into two intervention groups, differing in reminder intensity, or a control group without any int…

[Aftercare for durability and profitability of single-unit and multi-unit fixed dental prostheses].

An important aim ofa treatment with single-unit and multi-unit fixed dental prostheses is a durable and profitable treatment outcome. That requires aftercare, too. First, the frequency of routine oral examinations should be assessed, using an individual risk profile. The objectives of the routine oral examinations are the prevention and, when necessary, the treatment of pathological conditions and complications. With regard to prevention, attention should be paid to information and instruction, oral biofilm and calculus, non-functional activities, hard tooth tissues, periodontal and peri-impla…

It takes a village: a multidisciplinary model for the acute illness aftercare of individuals experiencing homelessness.

Homeless individuals are often uninsured and are more likely than the housed to utilize acute health care services and experience longer hospitalizations. Currently in the United States, there are fragmented services available for the aftercare of these patients to ensure continuum of care, promote healing, and avoid re-entry into the acute care system. The Fourth Street Clinic Respite Program was created to address these issues. Patients are referred to the program from local hospitals and other service providers. Based on the acuity of illness and need for nursing care, patients are admitted…
