The relationship of agraphia to the severity of dementia in Alzheimer's disease.

Impairment of writing ability was studied in 20 patients with mild to moderate dementia caused by early-onset Alzheimer's disease. A multico … lzheimer's disease. Further studies are needed to show the potential usefulness of such measures of agraphia in subtyping this disease.

Agraphia in Korean patients with early onset Alzheimer's disease.

BACKGROUND: Agraphia in Korean patients may be different from agraphia in other patients who use alphabetical writing systems due to the "visuoconstructional script" characteristics of the Korean writing system, Hangul. Patients with early onset Alzheimer's disease (EOAD) have a severe degree of hypometabolism in the parietal area, which is known to be involved in processing visuospatial function. Thus, we explored the diverse error patterns manifested in writing single syllables in Korean patients with EOAD. METHODS: A study sample of 35 patients with EOAD and 18 healthy controls (HC) perfor…

Pure alexia without agraphia: a classical cortical syndrome revisited.

A patient who developed pure alexia without agraphia following a stroke is described. An infarction of the left occipital pole was demonstrated by Magnetic Resonance Imaging (MRI). The literature on this rare syndrome is reviewed, and the localisation of damage in relation to the clinical findings discussed.

A case of pure agraphia due to left thalamic hemorrhage.

Pure agraphia is an isolated writing disturbance without intellectual impairment, aphasia, alexia, or apraxia. The authors report a case of pure agraphia following left thalamic hemorrhage. The patient showed impairment in the writing of both kanji and kana. The most frequent error in the writing of kanji was substitution (46%) followed by no reaction (23%) and partial omission or addition of characters (21%). The copying of kanji by hand was normal. Even when the patient was unable to write a kanji character, he understood the meaning and correctly recited the other readings. He complained th…

Hanja (Ideogram) alexia and agraphia in patients with semantic dementia.

Posterior fusiform gyrus (BA 37) is responsible for Hanja (ideogram) alexia in stroke patients. Patients with semantic dementia (SD) have lesions in the basal temporal area. The close proximity in these two lesions and the fact that reading ideograms requires holistic processing as is necessary in recognition of objects, suggests a possibility that ideogram alexia/agraphia may occur in patients with SD. We established and carried out Hanja and Hangul (phonogram) reading/writing tasks on six SD patients and nine Alzheimer's disease (AD) patients as control to see if these two patient groups sho…

Alexia with and without agraphia: an assessment of two classical syndromes.

BACKGROUND: Current cognitive models propose that multiple processes are involved in reading and writing. OBJECTIVE: Our goal was to use linguistic analyses to clarify the cognitive dysfunction behind two classic alexic syndromes. METHODS: We report four experiments on two patients, one with alexia without agraphia following occipitotemporal lesions, and one with alexia with agraphia from a left angular gyral lesion. RESULTS: The patient with occipital lesions had trouble discriminating real letters from foils and his reading varied with word-length but not with linguistic variables such as…

Residual reading capability in a patient with alexia without agraphia.

A case of pure alexia due to an ischemic lesion of the occipital temporal region is described. Written words could be matched but not read. Immediate memory span for graphemes was defective. The reading defect probably depends on the inability to modify the written word "globally"; the phonological process was intact, but the memory disturbance impeded reading. The dissociation is explained by the preservation of word forms, which are linked to the semantic stage. Non-written stimuli trigger a "meaning" which evokes the word form and so the written word is recognized even though it cannot be r

Molecular neuroimaging in primary progressive aphasia with predominant agraphia.

A 62-year-old male presented with progressive isolated writing and spelling difficulties. Neurological, neuropsychological, speech, and language evaluations identified only minimal additional abnormalities. The presenting characteristics did not meet criteria for any particular variant of primary progressive aphasia; his clinical presentation is best described as primary progressive aphasia, with a predominant, almost pure agraphia. Brain MRI showed asymmetric, bilateral parenchymal volume loss, with left hippocampal atrophy. Fluorodeoxyglucose-F18 positron emission tomography showed hypometab…

Posterior cerebral artery stroke presenting as alexia without agraphia.

Alexia without agraphia (pure alexia) was the first of the disconnection syndromes to be described by Dejerine who reported a patient of alexia without agraphia secondary to an embolic occipital lobe infarct. We herein report a 55-year-old man who presented with alexia without agraphia with magnetic resonance imaging suggestive of left posterior cerebral artery infarct involving left occipital lobe and splenium of corpus callosum. Alexia without agraphia is a relatively uncommon clinical condition, which should always be thought in a patient presenting with difficulty in reading with normal vi…

[Bulbar-onset amyotrophic lateral sclerosis (ALS) with isolated agraphia].

We reported the two cases with bulbar-onset ALS showing isolated agraphia without overt dementia and aphasia. Patient 1 was a 69-year-old man and patient 2 was an 81-year-old woman, and both were right-handed. Each patient developed dysarthria as an initial symptom followed by dysphagia, and neurological examinations showed atrophy and fasciculation of the tongue with upper and lower motor-neuron involvement of the extremities. These characteristic features with electromyographic evidence including widespread acute and chronic denervation led to a diagnosis of bulbar-onset ALS. Around 1 year a…

[Alexia without agraphia: an exemplary deficit, cherished by neuropsychologists].

Alexia without agraphia is the result of a left inferotemporal cortical lesion. This unilateral lesion provokes an almost irrecoverable word reading disability. This anatomical-clinical observation has identified as crucial an area on the left fusiform gyrus, which represents a sort of "letter cranial bump". This area, located near the left V4, has been labeled the visual word form area (VWFA). The VWFA ensures visual word processing whatever the visual field stimulated. In normal subjects, imaging methods show an activation of the VWFA during reading. In patients, the alexia that follows a VW…

[Kanji-predominant alexia with agraphia in opticospinal multiple sclerosis].

Alexia with agraphia is very rare symptom in multiple sclerosis. We present a patient of opticospinal multiple sclerosis with kanji-predominant alexia with agraphia. A 55-year-old, right-handed man was admitted to our hospital because of difficulty in reading and writing in August 2001. The patient had been diagnosed as having relapsing-remitting opticospinal multiple sclerosis eight years prior to admission. Language examination showed alexia with agraphia predominantly affecting kanji and also mild naming difficulties, but a good comprehension and a normal repetition. T2-weighted MRI demonst…

Selective alexia and agraphia sparing numbers-a case study.

We report a patient (MT) with a highly specific alexia affecting the identification of letters and words but not numbers. He shows a corresponding deficit in writing: his letter writing is impaired while number writing and written calculation is spared. He has no aphasia, no visuo-perceptual or -constructional difficulties, or other cognitive deficits. A similar pattern of performance has to our knowledge only been reported once before [Anderson, S. W., Damasio, A. R., & Damasio, H. (1990). Troubled letters but not numbers. Domain specific cognitive impairments following focal damage in fronta
