VRAM flap for an above knee amputation stump.

Although routinely utilized in reconstruction of groin, perineal and thigh defects, the pedicled vertical rectus abdominis myocutaneous (VRAM) flap has only once been previously reported for coverage of above knee amputation (AKA) stumps. A 36 year old man sustained a traumatic above knee amputation after stepping on an improvised explosive device (IED). Following several sessions of debridement, an ipsilateral pedicled VRAM flap was utilized to provide padded soft tissue coverage and maintain bone length (6 cm below the greater trochanter), avoiding both a debilitating hip disarticulation and

[Stump problems after a lower limb amputation].

Problems in the amputation stump occur frequently in lower limb amputees. These problems can range from skin defects to serious vascular insufficiency and have a major impact on the amputee's quality of life. We illustrate this with 2 patients aged 20 and 65. The first patient developed a Candida skin infection of the stump skin as a result of silicone liner use. She was successfully treated with miconazole/hydrocortisone and hygiene measures. The second patient developed progressive lower limb ischemia that resulted in a stump wound and claudication. He was treated with vascular surgery. Pati

Salvage of foot amputation stumps of Chopart level by free medial plantar flap.

Blast energy-induced traumas usually result in some type of amputations of lower extremities. It is very hard to determine the amputation level of the feet of these cases at first, and secondary amputation stump revisions by bone shortening are often necessary. Among partial foot amputation levels, Chopart level is the most critical. Four male patients (20 to 24 years old) with modified Chopart amputation due to mine explosion injury have had skin-grafted amputation stumps where troublesome, recurrent unstable wounds had developed. These amputation stumps were electively reconstructed with neu…

Amputation stump salvage using a free forearm flap from the amputated part.

Upper extremity amputations are not as common as lower extremity amputations, but they present unique challenges to the surgeon, prosthetist and amputee. The tissues of amputated or non-salvageable limbs may be used to reconstruct complex defects resulting from tumour removal or trauma, i.e. the 'spare parts' concept. The use of a free fillet flap to treat massive trauma of the upper extremity is reported here for two cases. These patients were treated with a pedicled fillet of whole forearm flaps to achieve primary stump closure and to preserve upper arm length.

Salvage of major amputation stumps of the lower extremity with latissimus dorsi free flaps.

In some severe lower limb injuries, the level of bone trauma enables preservation of the knee joint or adequate length of the femoral stump only if the soft tissues can be reconstructed over the exposed bone. The options for soft-tissue reconstruction of an amputation stump are to use a flap from the amputated distal part, a local flap possibly after tissue expansion or a free flap. To preserve an adequate length of stump we reconstructed 10 stumps with latissimus dorsi free flaps: above the knee in one patient and below the knee in nine. The reconstructions were done during an acute post-trau

Primary closure of below-knee amputation stumps: a prospective study of sixty-two cases.

Between January 1, 1985, and December 31, 1988, we prospectively studied the outcome of 62 consecutive below-knee amputations with primary closure in 56 patients. There were 35 men and 21 women; mean age was 70 years. Above-knee amputation was performed for occlusion of the profunda femoris artery, acute thrombosis of a popliteal aneurysm with inadequate sural artery vascularity, intractable knee flexion contracture, suspended ischemia, and occasionally, when ischemia was found intraoperatively to extend proximally during below-knee amputation. Bedridden patients deemed unfit for prosthetic de

Early postoperative detection of tissue necrosis in amputation stumps with indocyanine green fluorescence angiography.

OBJECTIVE: Amputations of the lower extremity due to irreversible ischemic tissue loss are performed as distally as possible. Therefore, oft … blood clot. CONCLUSION: Indocyanine green fluorescence angiography allows a perfusion analysis of amputation stumps and therefore a prediction of the expected tissue necrosis. This tool may allow reliable prediction of amputation level.

Botulinum toxin and hyperhidrosis of the amputation stump in war amputees.

INTRODUCTION: Stump hyperhidrosis is a common condition after lower limb amputation. It affects the prosthesis use, and the quality of life of patients. Several case reports tried to prove benefit of using Botulinum toxin in its treatment. AIM: This study was to conduct a larger workforce clinical trial and to demonstrate benefits of botulinum toxin injection in the treatment of stump hyperhidrosis. METHODS: A prospective study was conducted. War amputees who complained of annoying excessive sweating of the stump were included. They received intradermal injection of botulinum toxin A in the

Treatment of Lymphorrhea Associated with an Amputation Stump with Lymphaticovenular Anastomosis.

Lower limb lymphorrhea is a condition with a considerable impact on the quality of life. It is usually associated with inguinal lymph node dissection and vascular procedures with femoral exposure. In this case report, we describe a patient who underwent a below-knee amputation and two years later developed lymphorrhea from the stump, preventing adaptation to the prosthesis. Lymphoscintigraphy showed a delayed lymphatic progression. After failure of conservative treatment, she underwent lymphaticovenular anastomosis with a successful outcome. Drainage cessation suggests that lymphaticovenular a

Multimodality imaging review of the post-amputation stump pain.

Limb amputation is one of the oldest known surgical procedures performed for a variety of indications. Little surgical technical improvements have been made since the first procedure, but perioperative and post-operative refinements have occurred over time. Post-amputation pain (PAP) of the stump is a common complication but is an extremely challenging condition to treat. Imaging allows early diagnosis of the underlying cause so that timely intervention is possible to minimize physical disability with its possible psychological and socioeconomic implications. A multidisciplinary approach shoul

Prevalence of stump overgrowth in pediatric burn patient amputations.

BACKGROUND: Pediatric patients sometimes require amputation as part of their life-saving burn care. Bone overgrowth is known to occur in these amputation patients at rates of 4% to 43% depending on the study. METHODS: We conducted a retrospective study of all patients who underwent amputation at a children's burn center for a 45-year span. There were 18,880 burn patients during this time frame and 149 pediatric patients with amputation who survived long-term were studied. Burn types included flame, electrical, scald, thermal, and combination types in order of prevalence. We studied bone overg

Lengthening of an amputation stump by the Ilizarov technique. A case report.

A woman, 29 years of age, sustained a below knee amputation following injury and was left with a stump 5.5 cm long. Good soft tissue cover was obtained with a myocutaneous flap using the gastrocnemius muscle and heel pad flap from the injured leg. The stump was then lengthened by 7 cm, to 12.5 cm, using the Ilizarov distraction technique. The patient was able to bear weight on the end of the fixator during the 8 months it was in position. The gap filled with bone and there were no complications. She recovered good function.

Amputation stump lengthening with the Ilizarov technique. A case report.

It is often difficult to create a functional stump from a traumatic amputation, especially in a child. A case is described of a traumatic, high, below-knee amputation in a five-year-old child. The resulting stump was too short to allow a conventional below-knee prosthesis, decreasing gait efficiency. The Ilizarov technique was used to increase tibial length. Bony lengthening was very successful, but several problems were encountered with the soft-tissue reconstruction. This technique of stump reconstruction holds promise only if the soft-tissue problems are anticipated. The suggestions for fut
