[Crisis management during regional anesthesia including peripheral nerve block, epidural anesthesia and spinal anesthesia].

Crisis management during regional anesthesia including peripheral nerve block, epidural anesthesia and spinal anesthesia was reviewed. Common crisis which is encountered during regional anesthesia includes toxic reaction to local anesthetic drugs, allergic reaction induced by local anesthetic drugs, reaction induced by epinephrine, nerve injury, hematoma etc. Concerning peripheral nerve block, crisis encountered during brachial plexus block, interscalene block and supraclavicular block used for surgical operation of upper extremity was discussed. On the other hands, there are various common cr

[Clinical presentation of a dorsal epidural arachnoid cyst after an epidural anesthesia].

BACKGROUND: Arachnoid cysts are dural diverticula with liquid content similar to cerebrospinal fluid, with 1% occurring in the spinal cord. They locate mainly in the dorsal region of the thoracic spine, and are unusual causes of spinal cord compression. CLINICAL CASE: The case is presented of a previously healthy 15-year-old boy, with a 20-month history of spastic paraparesis that started apparently after epidural block for ankle osteosynthesis. There was decreased sensitivity and strength of the pelvic limbs and gradually presented with anaesthesia from T12 to L4 dermatomes, L5 and S1 bilate

The reliability of epidural anesthesia for repeat ESWL: a study of changes in epidural compliance.

Changes in epidural compliance were investigated before and after extracorporeal shock wave lithotripsy (ESWL) in 11 outpatients. Epidural pressures were unchanged acutely by ESWL (p greater than 0.15). In contrast to previous reports of failures of epidural anesthesia in patients having repeat ESWL treatments, a review of our population of patients having repeat ESWL indicates that epidural anesthesia is reliable and not associated with any greater failure rate than for initial ESWL treatments. The difference between our results and previous reports of high failure rates is likely due to diff

Postpartum headaches after epidural or spinal anesthesia.

PURPOSE OF REVIEW: Postpartum headache is a common occurrence with various obstetric, neurologic, and anesthetic etiologies. Post dural puncture headache (PDPH) after neuraxial anesthesia is a culprit that may be less familiar to obstetricians. In this review, authors will discuss the differential diagnosis and management of postpartum headache, review PDPH, and explore evidence suggesting that PDPH may have greater implications than previously considered. RECENT FINDINGS: Emerging evidence suggests that PDPH, previously believed to be benign and self-limited, is associated with significant a

Bilateral Vocal Fold Paralysis After Epidural Anesthesia.

Cranial neuropathies are known potential complications of spinal anesthesia, with most reports describing upper cranial nerve involvement. Intrathecal hypotension resulting in traction injury of the cranial nerves is the likely mechanism of injury. Unilateral vagal neuropathy was first described recently. The patient discussed in this case presented with hoarseness and dysphagia after receiving epidural anesthesia for childbirth. Following videostroboscopy and laryngeal electromyogram, she was diagnosed with bilateral vocal fold paralysis. The patient was managed conservatively with expectant

Non-analgesic effects of thoracic epidural anesthesia.

Thoracic epidural anaesthesia is selected usually to provide adequate postoperative analgesia; however with administration of local anaesthetics to epidural space selective sympatolysis ensues. The effects of this transient sympathectomy on cardivascular, respiratory and other systems deserve certainly some interest as it may influence postoperative morbidity or mortality. Thoracic epidural anaesthesia has succesfully been used in cardiac, thoracic and major abdominal surgery. It provides dynamic analgesia, rapid mobilization, blunted stress response, early extubation with reduced pulmonary co

Epidural anesthesia in labor. Benefits versus risks.

Epidural anesthesia is used for relief of labor pain by 29% of women having hospital deliveries in the United States, a number that has doubled within the past 10 years. Although epidurals provide objective pain relief that is exponentially better than the other pain relief methods, there are many purported complications and side effects. This article reviews how epidurals work, summarizes the literature regarding complications, and presents some of the ethical dilemmas inherent in the use of this technology for labor.

[Neurological accidents after epidural anesthesia in obstetrics].

Several neurological complications have been described after epidural anaesthesia, including direct trauma to the spinal cord or nerve roots, epidural haematoma, meningitis, epidural abscess, spinal cord infarction. neurologic toxicity of injected agents. In obstetric practice, these complications are very uncommon. However, their real occurrence may be underrated, partly for medicolegal reasons. Different complication mechanisms are described; they should be kept in mind while evaluating post block neurological deficits so that prompt corrective measures can be taken whenever appropriate to p

[Horner syndrome following combined spinal-epidural anesthesia].

Horner syndrome is rarely observed in connection with epidural anesthesia. It is characterized by ptosis, enophthalmos, miosis, anisocoria, and conjunctival hyperemia in the affected eye, as well as anhydrosis and flushing on the affected side of the face. It is usually a complication spontaneously resolved without permanent neurological deficits. Intraoral anesthesia; stellate ganglion, cervical or brachial plexus blocks; thoracic, lumbar or caudal epidural anesthesia, and intrapleural analgesia are the main causes for Horner syndrome related to anesthesia. Among other causes of Horner syndro

[EnglischerTitel: Epidural anesthesia - complications and side effects].

Thoracic epidural anesthesia remains the gold standard of intra- and postoperative analgesia for a multitude of surgical procedures as a reliable and relatively safe procedure with a low rate of complications as long as contraindications are respected. However, if complications do occur they often result in catastrophic sequelae for the patient. Apart from the well-known side-effects and complications there are innumerous reports about procedural complications during placement and use of the catheter. Procedural complications often result in additionally required procedures and treatments, hen

[Spinal myoclonus possibly caused by epidural anesthesia].

We report a patient who developed a rare neurological complication of spinal myoclonus possibly caused by an epidural catheter. A 24-yr-old female received laparoscopy and intrauterine curettage under general combined with epidural anesthesia. Spinal myoclonus started about 4 hours after the last epidural drug injection and disappeared 2 hours following removal of the epidural catheter. The patient was discharged without any untoward neurological sequelae.

Transient Horner's syndrome after lumbar epidural anesthesia.

Horner's syndrome as a complication of lumbar epidural anesthesia is a relatively benign and transient condition that usually does not warrant further extensive investigation. Its occurrence is unpredictable, although more frequently associated with epidural anesthesia performed for obstetric conditions. It may indicate high sympathetic blockade, and those patients should be monitored closely for autonomic complications. We report two new cases of iatrogenic Horner's syndrome from lumbar epidural anesthesia.

Aortic stenosis, cesarean delivery, and epidural anesthesia.

A 23-year-old female was referred to the University of Arkansas for Medical Sciences at 32 weeks' gestation with a history of aortic stenosis following aortic valve replacement. Evaluation by echocardiography showed an approximately 90 mmHg transvalvular pressure gradient. Pregnancy progressed to 36 weeks' gestation without problem, at which time the patient underwent cesarean section with lumbar epidural anesthesia. Invasive hemodynamic monitors were used to assess cardiac performance and as a guide for anesthetic management. The impact of aortic stenosis on pregnancy is discussed, as are man
