[Characteristics of the fetal and infant respiratory system : What the pediatric anesthetist should know].

Respiratory complications are the most frequent incidents in pediatric anesthesia after cardiac events. The pediatric respiratory physiology and airway anatomy are responsible for the particular respiratory vulnerability in this stage of life. This article explains the aspects of pulmonary embryogenesis relevant for anesthesia and their impact on the respiration of preterm infants and neonates. The respiratory distress syndrome and bronchopulmonary dysplasia are highlighted as well as the predisposition to apnea of preterm infants and neonates. Due to the anatomical characteristics, the low si

Emerging mucormycosis posing threat to community: Anesthetist's perspective in a tertiary care center.

Mucormycosis has become an ever-growing threat to human health, particularly after the COVID-19 pandemic. As the number of cases of mucormycosis increased, it put a burden on anesthesiologists. Here we describe the etiopathogenesis, clinical presentation, and anesthesia management of patients suffering from mucormycosis.

[Forced opioid detoxification under general anesthesia--a new challenge for anesthetists and intensive care physicians].

Treatment of opioid addicts by means of competitive opioid receptor antagonists was developed at the University of Vienna in 1987 by Loimer and co-workers. They compared two withdrawal regimens: The short Opiate withdrawal using a staggered naloxone regimen and the rapid opiate detoxification during general anesthesia by means of high doses of naloxone. Based on the latter concept, various modifications have been developed world-wide using either naloxone or as an alternative, naltrexone, an antagonist available for oral administration only. However, there are considerable objections to opioid

Evaluation of the safety and efficacy of deep sedation for electrophysiology procedures administered in the absence of an anesthetist.

UNLABELLED: Several procedures performed in the electrophysiology laboratory (EP lab) require surgical manipulation and are lengthy. Patients undergoing such procedures usually receive general anesthesia or deep sedation administered by an anesthesiologist. In 536 consecutive procedures performed in the EP lab, we assessed the safety and efficacy of deep sedation administered under the direction of an electrophysiologist and in the absence of an anesthetist. Patients were monitored with pulse oximetry, noninvasive blood pressure recordings, and continuous ECGs. The level of consciousness and v

Nurse Anesthetists' and Operating Theater Nurses' Experiences with Inadvertent Hypothermia in Clinical Perioperative Nursing Care.

PURPOSE: To deepen the understanding of and describe nurse anesthetists' and operating theater nurses' experiences with inadvertent hypothermia in clinical perioperative nursing care. DESIGN: Qualitative, explorative, and descriptive study that carries an inductive nature. METHODS: Focus group interviews with 16 nurse anesthetists and operating theater nurses from four hospitals in Norway. Gadamer's hermeneutics has guided the study's interpretation. FINDINGS: Taking the temperature and measures against inadvertent hypothermia perioperatively are not always systematically implemented. In th…

AANA Journal course. 15: new technologies in anesthesia: update for nurse anesthetists--continuous spinal anesthesia.

The production of microcatheters small enough to be threaded through 22- to 26-gauge spinal needles has focused renewed attention on the technique of continuous spinal anesthesia. This technique has a specific combination of advantages which cannot be duplicated by any other method of regional blockade. The most important of these advantages is that sensory blockade can be produced quickly and precisely with small doses of local anesthetic, that the duration of anesthesia can be extended indefinitely, and that recovery is rapid when short-acting local anesthetics are used. Inadequate anesthesi

Danger, Duty, Resilience: A Phenomenological Study of Nurse Anesthetists' Lived Experiences During the Rwanda Marburg Outbreak.

Marburg virus disease, a hemorrhagic fever without approved vaccine or cure, emerged in Rwanda in late 2024. During the outbreak, nurse anesthetists provided critical care to severely ill patients, including infected colleagues. Healthcare workers comprised most confirmed cases, with anesthetists and nurses among the earliest fatalities. The voices of nurse anesthetists reveal the essence of the phenomenon. This study used the methods of inductive phenomonology to discover the essential meaning of the lived experiences of nurse anesthetists working in Kigali referral hospitals during the outbr…

Triad of Idiopathic Thrombocytopenic Purpura, Preeclampsia, and HELLP Syndrome in a Parturient: A Rare Confrontation to the Anesthetist.

Idiopathic thrombocytopenic purpura (ITP) with HELLP represents a rare complication that requires combined care of obstetrician, anesthesiologist, hematologist, and neonatologist. At 37-week gestation a 35-year-old parturient (G2A1P0) a known case of chronic ITP presented with severe pregnancy induced hypertension (PIH), thrombocytopenia, and elevated liver enzymes. We describe successful anesthetic management of this patient who was taken for emergency caesarean section.

Anesthetist to patient transmission of hepatitis C virus associated with non exposure-prone procedures.

A 44-year-old lady was diagnosed with acute hepatitis C virus (HCV) infection 8 weeks after hysterectomy at which the attending anesthetist was known to be hepatitis C seropositive. Comparative nucleotide sequence analysis and phylogenetic comparison proved that transmission had occurred from the anesthetist to the patient. The patient had received general anesthesia with endotracheal intubation and peripheral intravenous cannulation. No exposure-prone anesthetic procedures had been performed. This is the first case described in UK involving transmission from an anesthetist to a patient during

Supervision of Pediatric Anesthesia After-Hours: A Survey of Pediatric Anesthetists in Australia and New Zealand.

BACKGROUND: After-hours pediatric anesthesia may pose increased risks, with a heightened potential for sudden cardio-respiratory decline. While mortality rates are low in Australia and New Zealand, critical events and morbidity occur more frequently and present ongoing challenges. However, little is known about how trainees are supervised during these high-risk periods. METHODS: An anonymized online survey of members of the Society for Pediatric Anesthesia in New Zealand and Australia (SPANZA) was conducted to explore supervising anesthetists attitudes towards after-hours supervision. The sur…

Surgical Treatment of Amiodarone-Induced Thyrotoxicosis in an Adult with Congenital Heart Disease: An Anesthetists Perspective.

BACKGROUND Amiodarone-induced thyrotoxicosis can be refractory to medical treatment seldom requiring surgical intervention. Herewith, we present the first case to date of amiodarone-induced thyrotoxicosis in a congenital heart disease post-Senning procedure requiring emergent surgical management. CASE REPORT A 19-year-old-male patient with a history of a complex congenital heart disease developed severe pulmonary baffle stenosis and atrial fibrillation that was medically treated with amiodarone, upon which he developed. Amiodarone-induced thyrotoxicosis that was refractory to medical treatment

Staged intraperitoneal brachytherapy and hyperthermic intraperitoneal chemotherapy in an adolescent: novel anesthetic challenges for pediatric anesthetists.

Newer techniques that have found a place in cancer management in adults are offered far less commonly in pediatric patients. We present a case of a patient with recurrent Wilms' tumor managed with a novel combination of cytoreductive surgery, intraperitoneal brachytherapy, and subsequent hyperthermic intraperitoneal chemotherapy. Each stage presents challenges that the pediatric anesthetist is unlikely to have faced before. Such cases require flexibility and thorough planning to manage the combination of major surgery, remote anesthesia with brachytherapy and hyperthermic chemotherapy with its

AANA Journal course: update for nurse anesthetists--a comprehensive review of remifentanil: the next generation opioid.

Over the last two decades, profound changes have taken place in the way elective surgery is practiced with the majority of surgical patients now admitted and discharged through ambulatory surgical facilities. After initial hesitation on the part of the surgeons, anesthesia care providers, and patients and their families, this practice is now clearly accepted. A variety of surgical advances (e.g., new endoscopic procedures, laser technology, and extracorporeal shock wave procedures) have allowed surgeons to simplify and abbreviate surgical procedures. Anesthesia care providers now, more than ev

Impact of Scope of Practice Laws for Certified Registered Nurse Anesthetists on the Utilization of Anesthesia Services.

OBJECTIVE: To assess the impact of pandemic-related changes in state scope of practice law (SOPL) for certified registered nurse anesthetists (CRNAs) on the utilization of anesthesia services during the COVID-19 pandemic, which served as a natural experiment. STUDY SETTING AND DESIGN: We used a difference-in-differences approach to compare changes in the utilization of anesthesia services during the COVID-19 pandemic in areas that expanded SOPL (removed requirements for supervision or direction) to those that did not. Additionally, we examined if the impact of changes in SOPL on patient acces…

Outcomes of Non-Anesthetist Led Conscious Sedation for 2000 Transcatheter Aortic Valve Implantations.

BACKGROUND: A minimalist approach to transcatheter aortic valve replacement (TAVR) has accompanied the increase in TAVR procedures worldwide. Comprehensive evidence regarding the safety of non-anesthetist-led conscious sedation for TAVR is lacking. AIMS: We aimed to evaluate the outcomes of patients undergoing TAVR with non-anesthetist-led conscious sedation. METHODS: This retrospective analysis included consecutive patients who underwent percutaneous transfemoral TAVR with non-anesthetist-led conscious sedation, from March 2018 to 2025, in a single high-volume center. Thirty-day outcomes we

Abnormal bleeding in the cardiac operating room: An observational study of interrater reliability between anesthetists and surgeons.

OBJECTIVES: Postcardiopulmonary bypass bleeding is frequent and distinctive because of its associated coagulopathy. Despite diagnostic and therapeutic progress, substantial variability persists in hemostatic transfusion practices. Because transfusion of hemostatic components should target abnormal bleeding, this study assessed the situational awareness of anesthetists and surgeons in such situations. METHODS: A nationwide questionnaire using real intraoperative video clips-3 normal, 8 ambiguously abnormal, and 6 clearly abnormal situations-evaluated interrater reliability for identifying abno…
