Congenitally corrected transposition (CCT) of the great vessels is an uncommon anomaly usually detected in children because of associated severe cardiac malformations. When these are absent, patients may be hemodynamically normal, but associated cardiac abnormalities are usually present in CCT, leading to symptoms in adulthood. Axial projections provide a new and important dimension to cardiac angiography. These techniques, however, must not be used blindly. There are several contraindications to their use as an initial approach, for example, the distortion of the usual spatial relationships between ventricles. But these projections are well suited for imaging the atrial septum, atrioventricular septal defects, anomalies of the atrioventricular junction, and other diseases.

Twenty-three patients with congenital subvalvular aortic stenosis are reviewed. The importance of classifying this stenosis into four types based on angiocardiographic findings is stressed, as are the indications for left heart catheterization during the preoperative and postoperative course of the disease. The type of congenital subvalvular aortic stenosis should be an important consideration during the discussion of operative indications in both asymptomatic and symptomatic patients.

Investigations of left ventricular function were carried out in 59 patients with mitral stenosis and 8 healthy subjects by means of one-plane quantitative angiocardiography. Fifteen patients with pure mitral stenosis (MS) underwent high-temporal-resolution radionuclide angiocardiography for calculation of the ratio of peak left ventricular (LV) filling rate divided by mean LV filling rate (filling ratio). Congenitally corrected transposition of the great arteries (CTGA) may constitute a pitfall for interpretation of radionuclide angiocardiography, since the two anatomic discordances cancel each other from a physiologic point of view. Pulmonary to systemic flow ratios (Qp/Qs) were estimated by quantitative radionuclide angiocardiography (QRAC) in 135 children. The Qp/Qs ratios were derived from pulmonary time/activity curves using a gamma variate model. Eighty-five of these children also had Qp/Qs ratios estimated by oximetry at cardiac catheterization.
