Intra-operative Radiation Therapy for Colorectal or Anal Cancer at Risk for Margin-Positive Resection: Initial Results of a Single-Institution Registry.

PURPOSE: Pelvic recurrence of rectal or anal cancers is associated with considerable morbidity and mortality. We report our initial experience with an aggressive intra-operative radiotherapy (IORT) program. METHODS: Patients with locally advanced or recurrent rectal or anal cancers considered to have a high likelihood of R1 or R2 resection after multi-disciplinary review underwent surgical excision and IORT using a high-dose-rate afterloader (Ir-192) and HAM applicator. Endpoints included local or distant recurrence, and acute and late toxicity graded using the American College of Surgeons (A

Carcinoma of the anal region.

Preservation of anorectal function makes chemoradiotherapy attractive as the primary treatment in patients with squamous cell carcinoma of the anal region. Despite variations in techniques of chemoradiotherapy administration, the accumulated experience of a number of institutions indicates substantial improvement over previous approaches, which included surgery or radiation therapy individually. Although no longer providing the definitive therapeutic role in this disease, the surgeon is frequently asked to evaluate lesions suspected of being anal malignancies. In addition, it is the surgeon wh

[Gastric carcinoid. A clinical case and review of the literature].

INTRODUCTION: Endoscopic examination of the gastrointestinal tract is very useful, because it allows a rapid diagnosis and, in some cases, a therapeutical effect. CLINICAL CASE: The authors have seen a woman, 72 years old, with epigastric pain, dryness of the mouth, diarrhoea and meteorism. She had an old simple gastritis (of two years standing) and her therapy, since the time of the diagnosis, is omeprazole (20 mg/die). The patient was submitted to gastroscopic examination that revealed two spot lesions and a polypous lesion on which was made a biopsy. The patient was submitted also to the f

Differences in the Impact of COVID-19 on Pathology Laboratories and Cancer Diagnosis in Girona.

INTRODUCTION: The recent COVID-19 pandemic has compromised socio-health care, with consequences for the diagnosis and follow-up of other pathologies. The aim of this study was to evaluate the impact of COVID-19 on cancer diagnosis in Girona, Spain. METHODOLOGY: Observational study of samples received in two pathology laboratories during 2019-2020 (tertiary hospital in Girona and county hospital in Figueres). Date, sample type, and location and morphology were available. Samples were recoded to determine malignancy and grouped by location. Comparisons were made by calendar year and period of e

Expression of CD3, PD-L1 and CTLA-4 in mammary and extra-mammary Paget disease.

BACKGROUND: Mammary and extra-mammary Paget disease is a rare form of intra-epithelial glandular neoplasm which is characteristically recurrent and necessitates multiple excisions that have an important impact on morbidity. Local immuno-modulating treatments have been applied with promising results, but the local immune markers of Paget disease have not been studied. AIM OF THE STUDY: To investigate the local immune micro-environment of Paget disease. MATERIALS AND METHODS: Sixty-four specimens from 41 patients, including cases with multiple recurrences and underlying primary neoplasm, have

Actinomycosis of the ano-rectum: a rare infectious disease mimicking carcinomatosis.

Abdominal actinomycosis is an infectious disease caused by Actinomyces israeli, a usual germ of the gastro intestinal tract. It gives unusual tumoral lesions with abcesses and fistulas. More rarely is abdominal actinomycosis at the end of the abdominal tube. Cope in 1949 said: "Actinomycosis occurs so seldom in the colon and the rectum that no surgeon, even if he be a proctologist is likely in a life time to see more than a few cases" (4). An anorectal actinomycosis case that looked like rectal cancer is reported. It was treated successfully by surgery and antibiotherapy. Some anatomopathologi

Idiopathic anorectal ulceration in patients with human immunodeficiency virus infection.

Opportunistic infections and neoplasms of the anorectum have been reported in patients with human immunodeficiency virus (HIV) infection. More recently, idiopathic ulcerative lesions of the colon and rectum have been described. At our center over a 3-yr period, four patients were identified with ulcerative lesions of the rectum and/or anus that remained idiopathic despite an extensive clinical, serologic, and histopathologic evaluation. Three patients had the acquired immunodeficiency syndrome, and in one anorectal disease was the index manifestation of HIV infection. Only one of the patients

Risk of Anorectal Cancer Associated with Benign Anal Inflammatory Diseases: A Retrospective Matched Cohort Study.

PURPOSE: The purpose of our study was to evaluate the relationship between benign anal inflammatory diseases and anorectal cancer and assess its risk factors. METHODS: A retrospective matched cohort study was conducted that included data from 2002 to 2013. The National Health Insurance Service National Sample Cohort data from 2002 to 2013 was used for the study. Of a total study population of 143,884 individuals, 28,110 individuals with anal fissures were assigned to the case group, while 115,774 individuals without anal fissures were assigned to the control group based on the 1:4 propensity

A rectal gastrointestinal stromal tumor case accompanied by elevated CEA mimicking rectal carcinoma on 18F-FDG PET/CT.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasm arising from gastrointestinal tract and can be benign or malignant. Rectal GISTs are rare and have poor prognosis. We here reported an older male who presented with features of distending discomfort in the rectum and pain in the anus due to a large rectal tumor. Physical examinations detected a mass in the rectum without blood staining on the gloved finger. Carcinoembryonic antigen (CEA) was found to be slightly elevated and the prostate-specific antigen level was normal. 18F-FDG PET/CT showed a soft tissue densit

Immunotherapy in advanced anal cancer: Is the beginning of a new era?

For decades metastatic squamous cell carcinoma of the anus (SCCA)has been considered a rare disease with very limited treatment options and a dismal prognosis. Prior to 2017, no data from prospective studies on the management of metastatic SCCA were available with scant information from retrospective analyses and few treatment options. Recently, InterAAct trial showed an advantage of carboplatin plus paclitaxel over the historical standard of care represented by cisplatin plus 5-fluorouracil. Unfortunately, there is no established second-line treatment after progression to first-line platinum-

Integrating Multi-sensor Time-series Data for ALSFRS-R Clinical Scale Predictions in an ALS Patient Case Study.

Gastrointestinal stromal tumors (GISTs) are the mesenchymal tumors of the gastrointestinal tract. Extra Gastrointestinal stromal tumors (EGISTs) are rare neoplasms that occur outside the gastrointestinal tract [1]. Miettinen et al. [2] described the first EGIST in 1999. EGIST accounts for 10% of all GISTs [3]. The main distinction between GIST and EGIST is the site of origin of the primary tumor, as GIST occurs throughout the GI tract, from the esophagus to the anus, whereas EGIST is a tumor without any connection with the intestinal wall and are reported in the retroperitoneum, mesentery, and

Potential role of a novel endoscopic retrieval bag for large colorectal resected specimen: a proof-of-concept study.

Endoscopic submucosal dissection enables en bloc resection of large superficial colorectal neoplasms. However, it is sometimes challenging to retrieve a large resected specimen via the anus without sample fragmentation. A novel "bag-type" retrieval device has been developed to accomplish complete isolation and non-destructive delivery of oversized specimens. This single-center retrospective study was performed to demonstrate the efficacy of this device for large colorectal resected specimens. Among 17 patients, we identified 18 superficial colorectal lesions for which the use of a novel retrie

Eosinophil-rich syphilis: a report of four cases.

The differential diagnosis for eosinophil-rich skin lesions often includes a drug reaction, allergic contact dermatitis and rarely, response to a helminth infection. However, many unrelated entities, such as infections, neoplasms and inflammatory dermatoses, can have a prominent eosinophilic infiltrate. Syphilis is classically associated with plasma cells, but other patterns of inflammation have been reported, including ulcerative, granulomatous and eosinophil-rich. Classic teaching might indicate that the presence of eosinophils argues against a diagnosis of syphilis. We present four cases of

Risk of anogenital cancer after diagnosis of cervical intraepithelial neoplasia: a prospective population-based study.

BACKGROUND: The first vaccine against human papillomavirus (HPV)-related disease is now available. Although it has been designed and tested mainly to protect against cervical lesions, it is also expected to be effective against other anogenital cancers. Associations between HPV and vaginal, vulvar, and anal cancers are well established, but the full extent in terms of age and time since diagnosis of these associations is not well known. METHODS: We established a cohort of all women in Sweden who were aged 18-50 years at some timepoint from 1968 to 2004. Using national registration numbers, we
