An 83-year-old woman with multiple comorbidities and severe aortic stenosis presented with recurrent pulmonary edema. In light of her high surgical risk, a percutaneous strategy for her aortic stenosis was decided. Transcatheter aortic valve replacement using a balloon-expandable Edwards Sapien XT valve was performed under rapid ventricular pacing. Soon after valve deployment, the patient went into hemodynamic collapse due to annular root rupture with pericardial tamponade, necessitating urgent pericardial decompression. Using a valve-in-valve technique, with the deployment of a second Edward …

Unicuspid aortic valve is a rare congenital cardiac abnormality, leading to aortic stenosis or regurgitation. We report the case of a 55-year-old man with severe aortic regurgitation caused by a unicuspid valve mimicking quadricuspid valve.

Transcatheter aortic valve implantation is increasingly being used to treat severe aortic stenosis in patients with high operative risk. In an aging population the incidence of aortic stenosis is rising, and increasing numbers of elderly patients are undergoing aortic valve replacement with bioprosthetic valves. Therefore, there is a corresponding increase in prosthetic degeneration. This presents cardiologists with a cohort of patients for whom the risk of re-do aortic valve surgery is prohibitive. We present the first series of such patients with degenerative bioprosthetic stenosis or regurg…

Patients with bicuspid aortic valve (BAV) commonly have associated aortic stenosis and aortopathy. The geometry of the aortic arch and BAV is not well defined quantitatively, which makes clinical classifications subjective or reliant on limited 2D measurements. The goal of this study was to characterize the 3D geometry of the aortic arch and BAV using objective and quantitative techniques. Pre-TAVR computed tomography angiogram (CTA) in patients with BAV and aortic stenosis (AS) were analyzed (n = 59) by assessing valve commissural angle, presence of a fused region, percent of fusion, and calc…

Circumflex aortic arch is a rare congenital anomaly. This case describes transcatheter aortic valve replacement (TAVR) via a transaortic approach, and to our knowledge is the first case of TAVR in a patient with circumflex aortic arch.

INTRODUCTION: The natural history of bicuspid aortic valve (BAV) remains inadequately characterized, leaving uncertainties regarding whether associated aortic dilatation arises from an inherent susceptibility or primarily results from altered flow dynamics across the aortic valve. We aimed to describe the evolution of valve function and aortic dilatation at preschool age in children diagnosed with BAV neonatally.
METHODS: The population study, the Copenhagen Baby Heart Study (n > 25,000), which was performed in 2016-18, diagnosed 196 newborns (0.8%) with BAV. In the present follow-up study, c…

Aortic valve disease is common and has significant impact on prognosis and quality of life. In this educational review, we cover the pathophysiology, presentation and assessment of aortic stenosis (AS) and aortic regurgitation (AR), including the role of imaging modalities beyond echocardiography. We review current treatment strategies and emphasise the use and indications for transcatheter aortic valve implantation (TAVI) in view of recent data highlighting its emergence as a novel treatment option for patients with AS, who are unsuitable for conventional aortic valve replacement (AVR). We al…

Quadricuspid aortic valve is a rare congenital heart defect usually detected by echocardiography or at the time of aortic valve surgery. Preoperative recognition of this abnormality has clinical significance because it is frequently associated with other congenital cardiac abnormalities and anomalies of coronary origin. We report a case of quadricuspid aortic valve, which was detected preoperatively by transthoracic echocardiography and was associated with severe aortic regurgitation.

We reported a case of a 45 year-old woman who had a quadricuspid aortic valve associated with moderate aortic regurgitation. The valve abnormality was detected by transthoracic echocardiography. Transesophageal echocardiography showed mild thickening of 4 symmetric aortic valve cusps, a small rectangular central regurgitant orifice, and moderate aortic insufficiency. In addition visualised this anomaly in 3D transesophageal echocardiography too.

Aortic valve disease, especially aortic stenosis, is increasing due to the increase in age related aortic valve degeneration. Although increase in aging population and co-morbidities such as diabetes, renal dysfunction and systemic vascular disease, the result of aortic valve replacement is improving. Operative mortality of isolated aortic valve replacement was 1.9% in Japan. Concomitant procedure such mitral valve surgery, tricuspid valve surgery, or coronary artery bypass grafting resulted in nearly 2 fold increase in mortality. Regarding reoperation, there were 2 to 3 fold increases in mort…

Aortic valve replacement options are limited in children, and all of them have disadvantages. Aortic valve repair techniques have evolved slowly and have not gained wide acceptance; however, large series using a variety of techniques demonstrate that valve repair is possible with excellent early hemodynamics and satisfactory intermediate durability. The results of aortic valve repair at the Children's Hospital of Wisconsin are presented. Simple repairs (blunt valvotomy, commissurotomy, or commissurotomy with leaflet thinning) directed at congenital aortic stenosis resulted in 86% +/- 5% freedo…
