Acquired apraxia of speech (AOS) is a motor speech disorder that affects the implementation of articulatory gestures and the fluency and intelligibility of speech. Oral apraxia (OA) is an impairment of nonspeech volitional movement. Although many speakers with AOS also display difficulties with volitional nonspeech oral movements, the relationship between the 2 conditions is unclear. This study explored the relationship between speech and volitional nonspeech oral movement impairment in a sample of 50 participants with AOS. We examined levels of association and dissociation between speech and

OBJECTIVE: The goal of this study was to explore the prevalence of nonverbal oral apraxia (NVOA), its association with other forms of apraxia, and associated imaging findings in patients with primary progressive aphasia (PPA) and progressive apraxia of speech (PAOS).
METHODS: Patients with a degenerative speech or language disorder were prospectively recruited and diagnosed with a subtype of PPA or with PAOS. All patients had comprehensive speech and language examinations. Voxel-based morphometry was performed to determine whether atrophy of a specific region correlated with the presence of N

A 57-year-old right-handed man complained of difficulty using his hands post-coronary artery bypass graft (figure). Neurologic examination revealed signs of callosal disconnection without hemiparesis. When asked to perform limb gestures like "brush your teeth" or "wave goodbye," the right hand performed flawlessly whereas the left hand was severely apraxic (video on the Neurology Web site at www.neurology.org).

We provide a comprehensive account of the distinctive clinical history and neuroimaging characteristics of a patient seen in a tertiary referral center, experiencing progressive dysfunction of the somato-cognitive action network and degeneration of the Rolandic association cortex. The patient is a 51-year-old woman who presented with progressive left arm rigidity, apraxia, spastic dysarthria, and dyspnea while speaking and eating. The patient underwent neurologic examination and neuroimaging in the form of a brain MRI and FDG-PET CT, as well as a comprehensive pulmonary evaluation. Her pulmona

PURPOSE OF REVIEW: Limb apraxia is one of the most common and most disabling disorders caused by brain damage. However, apraxia is one of the least recognized disorders associated with cerebral disease. This article discusses the signs and symptoms of, means of testing for, the pathophysiology of, and possible management of upper limb apraxia.
RECENT FINDINGS: Upper limb apraxia has four major forms: ideomotor, limb-kinetic, conceptual, and ideational. Although recent findings are included in this article, a full understanding of these disorders, including the means of testing, their possible

Apraxia is one of the most frequent clinical presentations of vascular, traumatic, infectious, tumor and neurodegenerative diseases of the brain. However neurologists have difficulties with diagnosis. The author presents a review of current classifications of apraxia and methods of its clinical diagnosis.

Studies in nonhuman and human primates have demonstrated that sound-producing actions are mapped on the same mirror circuits that are activated during the visual recognition and execution of actions [1-12]. However, no causative link between the auditory recognition and execution of actions has been provided thus far. Here, we sought to determine whether patients with apraxia, who are by definition impaired in performing specific gestures, are also impaired in recognizing sounds specifically linked to human actions. Twenty-eight left-hemisphere-damaged patients with or without limb and/or bucc

Ataxia-telangiectasia (AT) belongs to a group of recessively inherited disorders characterized by progressive ataxia and oculomotor apraxia. Included in this group are AT, ataxia-telangiectasia-like disorder (ATLD), ataxia with oculomotor apraxia type 1 (AOA 1), ataxia with oculomotor apraxia type 2 (AOA 2), and the recently described AOA3. Common to this group is the underlying cellular defect in the recognition and repair of double-strand or single-strand DNA breaks. Clinical and laboratory features allow one to distinguish between these various disorders. In this report, we describe a child

We include insights on how to distinguish IMA from the other motor apraxias and confounding disorders. We discuss testing for IMA and the need for more rigorous tests that examine more elements, such as imitation, actual use, task selection, and recognizing proper use. From neurophysiological insights, we propose hypotheses …

The definition of apraxia specifies that the disturbance of performed skilled movements cannot be explained by the more elemental motor disorders typical of patients with movement disorders. Generally this does not present a significant diagnostic problem when dealing with 'higher-level' praxic disturbances (e.g. ideational apraxia), but it can be a major confound in establishing the presence of limb-kinetic apraxia. Most motor disturbances characteristic of extrapyramidal disorders, particularly bradykinesia and dystonia, will compromise the ability to establish the presence of loss of dexter

Prosody is a complex process that involves modulation of pitch, loudness, duration, and linearity in the acoustic stream to serve linguistic and affective communication goals. It arises from the interaction of distributed neural networks that may be anatomically and functionally lateralized. Intrinsic prosody is mediated largely through left hemisphere mechanisms and encompasses those elements of linguistic microstructure (e.g., syllabic magnitudes and durations, basic consonantal and vocalic gesture specifications, and so) that yield the segmental aspects of speech. Extrinsic prosody is proce

Corticobasal degeneration (CBD) is a degenerative disease that often presents with an asymmetric progressive ideomotor limb apraxia. Some apraxic subjects may fail to perform skilled purposive movements on command because they have lost the memories or representations that specify how these movements should be performed (representational deficit). In contrast, other apraxic subjects may have the movement representations but are unable to utilize the information contained in them to execute skilled purposive movements (production-execution deficit). To learn if the apraxic deficit in CBD is ind

Five cases of limb-kinetic apraxia following primary degenerative cerebral pathology are reported. Apraxia appeared as the main symptom and was not concomitant to aphasia or to widespread cognitive impairments. Apraxia was not a consequence of elementary motor or sensory deficits, lack of coordination, or the result of the presence of rigid-akinetic symptoms. The apraxia was usually unilateral, being present in both routine activities and testing sessions and consisting of the coarse, unilateral, awkward execution of correctly planned movements. Conceptual knowledge of the movements and their

Ever since Liepmann's original descriptions at the beginning of the century apraxia has usually been attributed to damage confined to the cerebral cortex and/or cortico-cortical connecting pathways. However, there have been suggestions that apraxia can be due to deep subcortical lesions, which raises the question as to whether damage to the basal ganglia or thalamus can cause apraxia. We therefore analysed 82 cases of such 'deep' apraxias reported in the literature. These reports consisted of a small number (n=9) of cases studied neuropathologically, and a much larger group (n=73) in which CT …
