Glimpse into Pathophysiology of Sellar Arachnoid Cysts.

BACKGROUND: Sellar arachnoid cysts are a rare occurrence but may impinge on vital parasellar anatomy and thus are often symptomatic. The etiology of sellar arachnoid cysts is contentious, fueled by heterogeneity in cyst wall structure and contents between cases. The "ball-valve" mechanism is 1 of 2 predominant theories describing their formation, which contends that an aperture in the diaphragm allows cerebrospinal fluid to enter the cyst, propelled by pulsatile flow, but its egress is obscured by the pituitary during the ebb of the pressure wave. CASE DESCRIPTION: Here we present a case of a…

Neuroendoscopic management of middle fossa arachnoid cysts.

OBJECTIVE: The purpose of this study is to analyze symptomatic middle fossa arachnoid cysts and describe the advantages of the endoscopic technique to manage those cases that should be resolved surgically. METHODS: The physiopathology is described, and the clinical manifestations are analyzed. The diagnostic methodology and the surgical indications are presented. The endoscopic surgical technique is described thoroughly, and the advantages of the method are stated based on a series of 28 operated cases. The endoscope of choice has been a rigid endoscope with a 0° lens. RESULTS: All the cases…

Arachnoid cysts in childhood with endocrinological outcomes.

Arachnoid cysts are mainly manifested with the consequent neurological disorders. Even though these cysts may interfere in many systems due to their localizations, there is little information concerning their involvement in endocrinological disorders. We emphasize endocrinological functions together with cyst localizations and neurological disorders in childhood. A total of 50 patients diagnosed with arachnoid cysts were screened for cyst localizations, neurological symptoms and endocrinological outcomes evaluated by pubertal and growth status and hypothalamopituitary insufficiency. We investi…

Diagnosis of arachnoid cysts on prenatal ultrasound.

Our understanding of Late Jurassic sauropod faunas heavily relies on the fossil record of the northern hemisphere. During the last two decades, paleontological fieldwork in the Oxfordian-Kimmeridgian Cañadón Calcáreo Formation of Argentina has yielded several sauropod remains. Here we present a new taxon, Bicharracosaurus dionidei gen. et sp. nov., represented by a partial vertebral column and a fragment of the ilium. Anatomical as well as histological evidence suggest that the new specimen represents an adult individual. The posterior dorsals of Bicharracosaurus show great similarity with iso

Large arachnoid cysts at the cranial base.

Two infants who presented with macrocephaly had very large noncommunicating cysts at the cranial base that were displacing the brain. Only one of the patients was hydrocephalic; he eventually underwent ventriculoperitoneal shunting after craniotomy and attempted internal drainage of the cyst. The other infant had a cystoperitoneal shunt. Both have done well after 4 and 6 years of follow-up, respectively. These large arachnoid cysts are bilateral lesions with the potential for severe surgical complications and a questionable possibility of cure. The authors recommend systematic radiological inv…

Arachnoiditis ossificans with progressive syringomyelia and spinal arachnoid cyst.

We present a 30-year-old man with progressive spastic paraparesis. Spinal imaging revealed extensive calcification of the thoracic cord and cauda equina arachnoid, an intradural extramedullary cyst and evidence of rapidly progressing syringomyelia. Radiological diagnosis was arachnoiditis ossificans and an attempt at surgical decompression was made because of progressive neurologic deterioration. Due to tenacious adhesion of the calcified plaques to the cord and roots, only cyst drainage was achieved; the patient had no clinical improvement. A literature review revealed only two other cases re

Arachnoid cyst of the posterior fossa.

Arachnoid cysts of the posterior fossa are uncommon. Our case of a 49-year-old man presented with a 2 month history of headaches, nausea, and vertigo associated with walking instability. An MRI revealed a median well-circumscribed cystic lesion of the posterior fossa, with similar signal characteristics to CSF, and without connection to the fourth ventricle. This aspect suggested either arachnoid or hydatid cysts. Direct open surgery was performed allowing complete removal of the cyst wall, with a good outcome.

Recurrent intrasellar arachnoid cyst.

A 48-year-old man presented with complaints of decreased visual acuity persisting for 6 weeks in December 1997. Neurological examination demonstrated defects in the superior bitemporal visual field and bilateral optic atrophy. Computed tomography and magnetic resonance (MR) imaging showed an intrasellar cystic lesion. The cyst wall was excised via a transsphenoidal approach. The diagnosis was intrasellar arachnoid cyst. The sellar floor was reconstructed after packing fat in the sellar turcica. The visual complaint improved, but he was lost to follow up. Four years later, he was re-admitted co

[Arachnoid cyst of the sphenoid sinus].

BACKGROUND: Arachnoid cysts of the paranasal sinus are rare. They have not been described yet in the sphenoid sinus. PATIENT AND METHOD: Microscopic-endoscopic endonasal surgery of the sphenoid sinus was performed on a 34-year-old female with a history of chronic headaches and a suspected mucocele of the sphenoid sinus in computed tomography (CT) and magnetic resonance imaging (MRI) studies. An extended arachnoid cyst was found in the enlarged sphenoid sinus, which was obliterated with collagen, fibrin glue, and abdominal fat. RESULTS: There were no complications after the operation, and 12 …

Arachnoid cyst of the lateral ventricle.

BACKGROUND: Arachnoid cysts rarely occur within the lateral ventricles, with only 10 reported cases in the literature. They may arise from the arachnoid layer that is present in the choroidal fissure. CASE DESCRIPTION: This 25-year-old man with headache and a left jacksonian seizure had an intraventricular arachnoid cyst of the right occipital horn and trigone found on radiologic examination by computed tomography (CT) and magnetic resonance imaging (MRI). He was successfully treated by evacuation of the cyst and removal of its wall through a right parietooccipital craniotomy. Histologic stud…

Arachnoid cyst in the quadrigeminal cistern.

Two cases of arachnoid cysts in the quadrigeminal cistern are presented, and one occurred in an adult. Signs and symptoms of increased intracranial pressure were noted, and obstructive hydrocephalus was shown by computed tomography scans in both cases. Arachnoid cysts in the quadrigeminal cistern have rarely been reported, especially in adults. The definition, classification, and etiology of arachnoid cyst are still confused in the literature, and various terms have also been used to indicate this particular location of cyst. In this study, these confusing problems are reviewed, and the useful…

Arachnoid Cyst: An Asymptomatic Exuberance.

A 66-year-old man presented to the emergency department with sudden onset of dysarthria, left central facial palsy, and left hemihypesthesia involving the tongue. The patient was hemodynamically stable (blood pressure of 153/84 mmHg and heart rate of 80 bpm) and normoglycemic, without a history of trauma or toxic exposure. Assuming an acute stroke, the patient immediately underwent a brain CT scan that revealed a large left-sided fronto-parieto-temporal arachnoid cyst, with approximately 9.5 x 5.1 cm of maximum diameters (anteroposterior and transversal), compressing the brain parenchyma and t

Extradural Arachnoid Cyst Excision.

Arachnoid cysts are an uncommon postoperative complication and can result in back pain, radiculopathy, and/or cauda equina syndrome. For symptomatic postoperative arachnoid cysts, surgical management is the accepted treatment. While excision of arachnoid cysts is the preferred surgical method, cysts with adhesions to nerve roots and/or dura can be treated with cyst fenestration to reduce morbidity of excision. Surgeons should be prepared for dural defects after cyst excision, which can require dural grafting and lumbar drains for cerebrospinal fluid diversion.

Asymptomatic giant arachnoid cyst.

A 51-year-old male bricklayer without cerebral symptoms underwent whole-body FDG PET for suspicion of mediastinal sarcoidosis. PET/CT incidentally demonstrated a voluminous right frontal arachnoid cyst with normal glucose metabolism in the adjacent cortex. MRI demonstrated compression of the ipsilateral cerebral parenchyma and precentral, angulate, callosal, and superior frontal sulci. Functional bilateral finger-thumb paradigm MRI revealed right hemispheric reorganization of cortical motor activation with prominent activation between the inferior frontal gyrus and the postcentral gyrus.

Spinal extradural arachnoid cyst.

BACKGROUND CONTEXT: Spinal extradural arachnoid cysts are uncommon expanding lesions. Idiopathic arachnoid cysts are not associated with trauma or other inflammatory insults. If they enlarge, they usually present with progressive signs and symptoms of neural compression. PURPOSE: Total removal of the cyst and repair of the dural defect is the primary treatment for large thoracolumbar spinal extradural arachnoid cysts causing neurogenic claudication. Laminoplasty may prevent spinal deformities in long segmental involvement. STUDY DESIGN: A clinical case was performed. PATIENT SAMPLE: We repo…
