Mark was only 20 years old when an unfortunate sequence of events dramatically altered his life. In July 1989 he sustained two subarachnoid haemorrhages within a fortnight, first from a left anterior communicating artery aneurysm and then from a right middle cerebral artery aneurysm. Both aneurysm were successfully clipped but Mark remained hemiplegic with severe physical and behavioural problems, including incontinence, sexual disinhibition, aggression and uninhibited spitting. The present study is a crossvalidation in the Fair Oaks Raleigh Hills Hospital of a 1970 outcome study by Wiens et al. from the Portland Raleigh Hills Hospital. The 1975 Fair Oaks crossvalidation sample included all 275 patients treated during 1975. These results were further recrossvalidated using all 290 patients treated in the Fair Oaks hospital during 1976. Noncontingent escape (NCE) was used to reduce disruptive behavior in 3 children during regularly scheduled speech therapy sessions. Results showed rapid decreases in disruptive behavior and accompanying increases in compliance across children. Findings suggest that speech therapists with little expertise in behavior analysis can effectively implement NCE.

Rumination is repeated regurgitation without nausea or associated gastrointestinal illness with concomitant weight loss and malnutrition. This paper describes a ruminating infant who was becoming severely malnourished. Hyperalimentation was used to provide necessary nutrition, and a paradigm of aversive behavior modification was instituted. This form of therapy was successful after less than four weeks, with no harmful side effects and no recurrence of the disorder.

Transsexualism is a rare disorder, and there is little literature available on its treatment. A case is presented of transsexualism with homosexual orientation in a 24 year old male. Since the disorder appeared to have behavioral antecedents, it was treated with a behavior therapy package comprising relaxation, aversion therapy with aversion relief, modelling, hypnosis, orgasmic reconditioning, behavioral counselling and sex education. Therapy resulted in normalization of gender identity, but the homosexual orientation persisted.

Two subjects currently undergoing covert sensitization treatment were asked to imagine alternately the covert scene and a pleasant imagery scene while EMGs from the corrugator and forearm extensor muscle sites were continuously monitored. Visual inspection of the data suggests that corrugator EMG can reliably discriminate the covert-sensitization scene from the pleasant imagery and from resting baseline conditions. Facial EMG may have potential for objectively studying imagery and its role in covert sensitization and other imagery-based treatments.

A combination of aversive therapy and orgasmic reconditioning failed to produce the expected changes in sexual activities and arousal patterns. BACKGROUND: Overconcern with food and shape/weight stimuli are central to eating disorder maintenance with attentional biases seen towards these images not present in healthy controls.
