[Diagnosis of biliary tract carcinoma].

There are several diagnostic tools available in the diagnosis of bile duct cancer. Tumors of the middle and distal part of the extrahepatic bile duct are accessible to endosonography. Endoscopic retrograde cholangiopancreatography (ERC) and percutaneous transhepatic cholangiography (PTC) are the most invasive procedures for diagnosis of bile duct cancer. However, they offer the opportunity to obtain material for cytological or histological investigation. Moreover, bile flow can be assured by inserting endoprostheses during the procedure. Cholangioscopy and/or intraductal ultrasonography can be

Gallbladder and biliary tract candidiasis.

BACKGROUND: The clinical significance of Candida spp isolated from the gallbladder on the biliary tract is relatively unknown. METHODS: To provide this information, patients with Candida spp isolated from gallbladder and other biliary tract sources during a 10-year period were identified through the records of our clinical microbiology laboratory. Medical records were analyzed for biliary disease causes, culture data, treatment, and outcome. RESULTS: Twenty-seven patients were identified. Five of seven patients with cholecystitis were critically ill intensive care unit (ICU) patients in whom…

Trends in biliary tract cancer.

Incidence data from a large number of cancer registries throughout the world are presented for cancers at various sub-sites of the biliary tract, to assess whether the epidemiology of gall bladder cancer can be distinguished from cancer of the extra-hepatic bile ducts and cancer of the ampulla of Vater. Cancers of the biliary tract appear to be strongly sex-related. A marked genetic susceptibility emerges from this review, not only for gall bladder cancer, as previously reported for American Indians, but also perhaps for cancer of the extrahepatic bile ducts, for which Japanese of both sexes h…

Biliary-tract cancer in Chile.

This epidemiological study in Chile shows a marked increase in biliary-tract cancer based on mortality data, from an age-adjusted rate (1970 world population) of 5.1 per 100,000 in 1970 to 12.0 per 100,000 in 1988. There is an increased risk of this cancer in all age groups but especially in young adults (15-44 years). The female ratio of 3:1 persists. The increase in biliary-tract cancer in 1970-1985 was particularly important for young women but occurred in all female age groups whereas in men it was mostly in the elderly (65 years and more) and less in the middle-aged (45-64 years); no chan

Septicemia from biliary tract infection.

One hundred four strains of microorganisms were isolated from the blood in 76 episodes of septicemia originating from biliary tract infection. The 70 patients involved included 40 with acute cholecystitis without previous surgery, 17 with cholangitis following previous surgery, and 13 patients with malignant disease, with or without previous surgery. The most common organisms were Escherichia coli, Klebsiella, Streptococcus, and Proteus. Various streptococci, most of them from group D, were involved in 21% of the episodes. Twenty-five patients underwent surgery following the bacteremia. In ten…

Biliary tract infection or colonization with Elizabethkingia meningoseptica after endoscopic procedures involving the biliary tract.

OBJECTIVE: This study was performed to investigate the clinical significance of detecting Elizabethkingia meningoseptica in the bile. METHO … five cases, one of which also involved secondary septicemia, and colonized the nasobiliary tubes or biliary tract in the remaining 15 cases. All but two patients recovered and were discharged. Two patients died of septicemia; E. meningoseptica and Escherichia coli were the causative pathogens in one case and other organisms were the causative pathogens in the o …

Dysbiosis of the Fecal and Biliary Microbiota in Biliary Tract Cancer.

Characteristic bile duct and gut microbiota have been identified in patients with chronic biliary tract disease. This study aimed to characterize the fecal and bile microbiota in biliary tract cancer (BTC) patients and their relationship. Patients with BTC (n = 30) and benign biliary disease (BBD) without cholangitis (n = 11) were included. Ten healthy, age-matched subjects were also recruited for fecal microbiota comparison. The fecal and bile duct microbiotas were analyzed by sequencing the 16S rRNA gene V3-V4 region. Live bacteria were obtained in the bile from three BTC patients by culture…

Use of biliary drainage in diagnosis of biliary tract disease.

Thirty-five patients with biliary-type symptoms and normal oral cholecystograms were found to have either cholesterol crystals, calcium bilirubinate pigment or leucocytes in gallbladder bile collected through duodenal drainage. All underwent cholecystectomy. The gallbladders all showed histological evidence of chronic cholecystitis and/or cholesterolosis. In addition, four gallbladders contained calculi. Cholecystectomy is recommended as treatment for a selected group of patients with duodenal bile crystals or pigment.

Immunobiology of the biliary tract system.

The biliary tract is a complex tubular organ system spanning from the liver to the duodenum. It is the site of numerous acute and chronic disorders, many of unknown origin, that are often associated with cancer development and for which there are limited treatment options. Cholangiocytes with proinflammatory capacities line the lumen and specialised types of immune cells reside in close proximity. Recent technological breakthroughs now permit spatiotemporal assessments of immune cells within distinct niches and have increased our understanding of immune cell tissue residency. In this review, a…

Anatomy and embryology of the biliary tract.

Working knowledge of extrahepatic biliary anatomy is of paramount importance to the general surgeon. The embryologic development of the extrahepatic biliary tract is discussed in this article as is the highly variable anatomy of the biliary tract and its associated vasculature. The salient conditions related to the embryology and anatomy of the extrahepatic biliary tract, including biliary atresia, choledochal cysts, gallbladder agenesis, sphincter of Oddi dysfunction, and ducts of Luschka, are addressed.

[Imaging of the postoperative biliary tract].

For a long time, imaging of the biliary tract after surgical procedures was performed with invasive procedures such as endoscopic retrograde cholangiopancreatography or percutaneous transhepatic cholangiography. Due to recent advances in diagnostic imaging, non-invasive techniques are now favored. While US remains the initial imaging modality, it is frequently followed by CT and/or MRCP. Image interpretation should always be performed in keeping with clinical and laboratory findings as well as the type of surgical procedure. The most appropriate imaging modality is selected based on these data…

Iatrogenic lesions of the biliary tract.

Lesions of the biliary tract can happen during the performance of different types of surgery, not only biliary surgery itself. Mainly five types of surgery are responsible for these events: Biliary surgery, Liver surgery, Portal Hypertension surgery, Pancreatic surgery, Gastric surgery. We will concentrate on biliary surgery itself, excluding the specific problems from liver transplant surgery. Surgery for biliary lithiasis, tumours, inflammatory conditions, bilio-digestive anastomosis, placement of prosthesis or tumour intubations or biliary re-interventions, can all be cause for complication…

Viral infections of the biliary tract.

Bacterial infections of the biliary tract are often considered to be an important cause of acute cholangitis. Viral infections of the biliary tract however, are very often mistaken as viral hepatitis. This article highlights various viral causes of common biliary tract infections. Viral cholangitis is both less common and less discussed than viral hepatitis. Hepatotropic viruses (A, B, C, and E) are generally regarded as hepatocellular pathogens, yet cholangitic manifestations are now well described in association with these diseases. Systemic viral diseases also lead to cholangitis in varying…
