Five patients developed diplopia following cosmetic blepharoplasty. While temporary diplopia and visual disturbances are known to occur following this type of surgery, the persistence of diplopia with demonstrable muscle imbalance is not common. We encountered ocular motility complications involving the inferior or oblique muscle, inferior rectus muscle, and superior oblique tendon.

To report the first case of lacrimal gland fistula after upper eyelid blepharoplasty for blepharochalasis. Standard upper blepharoplasty and the hooding excision were performed in a female with blepharochalasis. The patient developed a fistulous tract with tearing from the incision few days after hooding excision. Fistula excision and lacrimal gland repositioning were performed. There were no complications after the repositioning procedure (6 months follow up). Prolapsed lacrimal gland and fistula formation can occur after upper blepharoplasty hooding excision.

The authors report what they believe to be the first case of a blepharoplasty procedure complicated by an acute postoperative dacryocystitis that precipitated orbital cellulitis. The patient had a preoperatively compromised nasolacrimal drainage system. Because acute dacryocystitis and resulting orbital cellulitis are potential postoperative complications of a blepharoplasty, blepharoplasty candidates should undergo preoperative evaluation of their nasolacrimal drainage systems.

Blindness is a rare but realistic complication in blepharoplasty procedures. Review of 55 case reports in the literature supports this view. The cause or causes of blindness are not precisely clear, but orbital hemorrhage after removal of fat seems to be the most common event associated with visual loss. Preoperative ophthalmologic assessment is highly desirable to document preexistent eye and systemic disease. Exquisite care with removal of fat is essential. Careful postoperative observation without patching is necessary. Management of acute visual loss may be medical, surgical, or both.

INTRODUCTION: Traditional upper blepharoplasty is a subtractive form of surgery that involves the excision of variable amounts of skin, muscle, and fat from the eyelid. The goal of surgery is to improve field of vision and/or appearance. While surgical debulking of the eyelid may improve appearance early on, the volume loss inherent to this process (especially fat excision) can contribute to a hollowed appearance with an associated deep and sunken superior sulcus. This skeletonized look may be mitigated by repositioning a prominent nasal fat pad, if present, to the central upper eyelid.
