OBJECTIVE: To investigate the various surgical techniques and their results for different types of blepharoptosis.
METHODS: 620 eyes of 500 cases with blepharoptosis who underwent surgical treatment were retrospectively analyzed. The patients were diagnosed as congenital, neurogenic, myogenic, traumatic, senile and mechanical ptosis. The used techniques included resection of levator muscle, anastomosis of frontalis muscle and levator aponeurosis, Whitnall's ligament sling, Friedenwald- Guyton's frontalis muscle fixation, levator aponeurosis reconstruction, modified Hotz's tarsectomy.

Congential ptosis (CP) represents a significant reconstructive problem. Numerous studies have not yet provided full and satisfactory results. In this study, we have presented our experience in the surgical treatment of 108 patients by the use of Son Ye Guang's modified method--frontal muscle transposition. A total of 108 patients with CP were surgically treated at the Clinic for Plastic Surgery and Burns of the Military Medical Academy in the period 1991-2000. UNLABELLED: The authors discuss the indications for the operation, the surgical technique and results obtained in 92 patients who have been operated for ptosis (109 operations) by the modified method of Berke and Neuhaus. From the experience of the authors it appears that the best approach is a subciliary one especially in congenital ptosis. The Neuhaus method showed to be successful in patients with ptosis connected with the "jaw-winking" Marcus-Gunn phenomenon; in these patients one obtained the elevation of the upper lid and liquidation of the synkinesis.

A frontalis sling procedure for blepharoptosis using a 1 mm solid silicone rod is described. This procedure is ideal for patients requiring frontalis suspension who are at risk for postoperative exposure keratopathy, or for patients likely to need postoperative eyelid position adjustment. We present the case of a 56-year-old man with a recessive RYR1 mutation (10318G>A) who presented with bilateral ptosis and ophthalmoplegia. Despite a history of rhabdomyolysis and Graves' disease, he had not previously reported visual complaints. Following bilateral frontalis fixation surgery, his visual acuity improved.
