Fortress brain.

Neurodegenerative diseases are associated with neuronal inclusions, comprised of protein aggregates. In Alzheimer's Disease (AD) and Lewy Bo … he rarity of AD-like pathology in most other older mammals. Over evolutionary timescales, the human brain should be adapted to predation by neurotropic agents. Viewed from this perspective, the prion-like pro-inflammatory and pro-apoptotic properties of β-amyloid and other proteins may be adaptive, and anti-microbial. Reductions in synaptic density may …

Brain metastases.

Approximately 10% of patients with cancer develop brain metastases. Some evidence indicates that as techniques for treating systemic tumors improve, the incidence of brain metastases, sequestered as they are behind the blood-brain barrier, is increasing. Although usually appearing late in the course of the disease, a brain metastasis may cause the initial symptoms, before the primary cancer has been identified. The diagnostic and therapeutic approach depends on the number and location of brain lesions and the stage of the cancer. Patients with brain metastases are rarely cured. However, approp…

Brain metastases.

Approximately 10% of patients with cancer develop brain metastases. Although usually appearing late in the course of the disease, the brain metastasis may be present before the primary cancer has been identified and may present as a single lesion or as multiple lesions. The diagnostic and therapeutic approach depends on the number and location of brain lesions and the stage of the cancer. Patients with brain metastases are rarely cured. However, appropriate treatment can improve both the quality and duration of the patient's life. Treatment must be directed not only at the brain metastasis (de…

Brain tuberculomas.

Although brain tuberculomas have become rare in developed countries, diagnosis should be kept in mind when confronted with brain space-occupying lesion(s), particularly in immigrants from endemic countries. Surprisingly, Human Immunodeficiency Virus (HIV) infection does not seem to have affected the incidence of this tuberculous lesion. Clinical, biological and radiological signs are only suggestive. Thus, when no other active extracranial tuberculous process is found, the diagnosis should be confirmed by a biopsy before beginning antituberculous treatment which is rapidly effective. Adjunctio…

Brain Abscess.

Optimal treatment of a brain abscess requires early clinical suspicion, and the diagnosis is usually made by identification of the abscess on contrast-enhanced computed tomography (CT) or magnetic resonance imaging (MRI). The immediate first step is to reduce the potentially life-threatening brain mass (abscess and surrounding cerebral edema) and secure the diagnosis with culture specimens. This is usually accomplished by reducing the increased intracranial pressure (ICP) through surgical aspiration with or without drainage of the abscess pus. The surgical procedure chosen depends on several f…

Brain abscess.

The past 20 years have seen major advances in the diagnosis and management of brain abscess, with a corresponding improvement in the survival rates. The advances in radiographic scanning, the availability of new antimicrobials, and the development of novel surgical techniques have all contributed to the decreases in associated morbidity and mortality. The relative rarity of brain abscess and the frequent delays in making the diagnosis render this condition a significant challenge for the clinician. A high index of suspicion is required so that effective therapy can be instituted as soon as pos…

Brain imaging.

Imaging with radionuclides should be used in a complementary fashion with other neuroradiologic techniques. It is useful in the early detection and evaluation of intracranial neoplasm, cerebrovascular accident and abscess, and in postsurgical follow-up. Cisternography yields useful information about the functional status of cerebrospinal fluid pathways. Computerized axial tomography is a new technique of great promise that produces a cross-sectional image of the brain.

[Brain Abscess].

Brain abscess is a potentially life-threatening intracranial infection progressing from localized cerebritis to an encapsulated cavity. Infection routes are classified into contiguous spread from adjacent infectious foci, hematogenous dissemination, and cryptogenic origin. Because the classic clinical triad is often absent, diagnosis based solely on clinical symptoms is challenging. Diffusion-weighted magnetic resonance imaging is crucial in early diagnosis and differential assessment. Treatment comprises antimicrobial therapy combined with surgical intervention, selected according to abscess …

[Brain Amoebiasis].

Amoebic infections of the brain can cause brain abscesses and meningoencephalitis. Brain abscess is a rare form of extraintestinal amoebiasis caused by hematogenous spread from intestinal infection with Entamoeba histolytica, which occurs through oral intake of contaminated water and food or fecal-oral transmission. It is mostly associated with amoebic liver abscesses and can be effectively treated with metronidazole. Meningoencephalitis caused by pathogenic free-living amoebae is extremely rare. Primary amoebic meningoencephalitis (PAM), caused by Naegleria fowleri infection of the olfactory …
