Cerebrovascular accidents (CVA's) of the brain stem have the same causes and physiopathological mechanisms as CVA's of the hemispheres, but they are ten times less frequent. They correspond to ischaemic or haemorrhagic lesions in the territories of the vertebral arteries and the basilar artery, including their collateral, circumferential and perforating branches. Clinically, CVA's of the brain stem are characterized by the association of unilateral motor and/or somatosensory symptoms on the side opposite the lesion, and by involvement of the ipsilateral cranial nerves, which helps in evaluatin…

Three cases of tuberculous abscess of the brain stem were treated with excision of the abscess supplemented with antitubercular therapy for 12 to 18 months. The lesions were frankly purulent and tubercle bacilli were demonstrated in the pus. A computed tomography scan demonstrated the site and extent of the lesion. Two of three patients developed the abscess during the course of antitubercular therapy for associated tubercular lesions. In spite of modern antitubercular treatment the abscess did not resolve and surgical excision had to be employed as a curative treatment.

Seven cases of an encephalitic brainstem syndrome are reported. All had an initial flu-like phase, with a febrile state. Then, all had eye movements disturbances and ataxia. Six had a variable dysfunction of other cranial nerves, five had dysmetria of limbs and three had pyramidal signs. In all, the topographic diagnosis was based on clinical, neurophysiological (electromyography, evoked potentials) and neuroradiological (swelled brainstem on CT scan) findings. Analysis of cerebro-spinal fluid revealed pleocytosis (mean; 165 cells/ml), without profile indicative of multiple sclerosis. Serologi

During the last decade there has been a remarkable increase in the interest of neurosurgeons in surgical approaches along the base of the sk … urosurgeon who currently undertakes surgery of complex lesions involving the base of the skull, the brain stem and the associated cranial nerves and arteries must have a thorough understanding of this anatomy and frequently must engage the help of an ear-nose-and-throat colleague to enhance his or her own expertise with these complex cases. A 33-year old male Tunisian developed, without any apparent cause, an abscess of the brainstem which was shown by various mycological investigations to be due to a Blastomyces dermatitidis infection. This case is of interest not only because of the exceptional location of the abscess in the brainstem, but also because of its treatment: this was the first case where the fluconazole-flucytosine combination was used against blastomycosis with fully satisfactory results after a 28-month follow-up.

Movement of the upper brain stem (inferior colliculus) was correlated with the alterations in the amplitude of wave V of the auditory brain-stem responses (BER's) during supratentorial brain compression in cats. In vivo observation of the brain stem and postmortem inspection show that suppression of the amplitude of BER wave V reflects the extent of caudal displacement of the inferior colliculus. Marked suppression of the amplitude of BER wave V (approximately 30% of control) correlates with the beginning of transtentorial herniation, and complete suppression of the wave V indicates complete t…

Auditory brain-stem responses (BSR) were recorded in 20 comatose patients in whom the level of brain-stem dysfunction was defined by clinical assessment of brain-stem reflexes and posture. No BSR abnormalities were found in the 10 cases with cortico-subcortical or diencephalic levels. The other 10 patients showed a clear relationship between alteration of the different components of the BSR and the clinical levels of brain-stem dysfunction caused by the rostro-caudal evolution. Alteration of wave P5 seems related to a midbrain dysfunction, of P3 to a pontine dysfunction and of P1 or P2 to a lo

Simultaneous bilateral recordings (C3 to A1 and C4 to A2) of brain stem auditory evoked responses have been studied in 67 supratentorial lesions, nine midbrain lesions, 21 intrinsic pontine lesions, and 23 extrinsic compressions of the pons. The responses in supratentorial lesions showed completely normal records. In midbrain lesions, wave V was specifically altered. As wave 1 has been shown to be a far-field seventh nerve potential, and wave V the midbrain potential, waves II to IV can be inferred to originate in the central auditory pathway between the seventh nerve and the midbrain. Alterat…

OBJECTIVES: The Nucleus auditory brain stem implant (ABI) has been used in the hearing rehabilitation of totally deaf individuals for whom a cochlear implant is not an option such as in the case of neurofibromatosis type 2 (NF2). Intraoperative electrically evoked auditory brain stem responses (EABRs) are recorded to assist in the placement of the electrode array over the dorsal and ventral cochlear nuclei in the lateral recess of the IVth ventricle of the brain stem. This study had four objectives: (1) to characterize EABRs evoked by stimulation with an ABI in adolescents and adults with NF2,…

PURPOSE: CNS malignancies are currently the most common cause of disease related deaths in children. Although brainstem gliomas are invariably fatal cancers in children, clinical studies against this disease are limited. This review is to lead to a succinct collection of knowledge of known biological mechanisms of this disease and discuss available therapeutics.
METHODS: A hallmark of brainstem gliomas are mutations in the histone H3.3 with the majority of cases expressing the mutation K27M on histone 3.3. Recent studies using whole genome sequencing have revealed other mutations associated w
