[Tumor hemorrhage in intracranial tumors].

From 1975 to 1981, 50 patients with spontaneous hematomas due to intracranial tumours were treated in the Neurochirurgische Universitätsklinik Erlangen-Nürnberg. 33 tumours were located within the cerebral hemispheres, 6 within the oral brain stem, 6 tumours had a cerebellar origin, 2 neoplasms were located within the cerebellopontine angle, and 3 tumours grew in the sellar region. In most of the patients we found metastatic neoplasms (26%) and glioblastomas (22%), 6 tumours (12%) could not be identified histologically; 2 of these cases were assumed to be malignant gliomas. Two congenital tumo

Pediatric Brainstem Tumor Biopsy: Surgical Planning and Execution for Maximal Safety and Tissue Yield.

Brainstem tumors account for 10-20% of pediatric brain tumors with a peak age of diagnosis between 7 and 9 years old and are often fatal. Historically, diagnosis of brainstem tumors has been largely based on imaging; however, recent studies have demonstrated the incongruities between preoperative MRI diagnosis and postoperative pathological findings highlighting the importance of brainstem biopsy for diagnostic accuracy. Stereotactic brainstem biopsy for pediatric brainstem tumors has been proven to be safe with a high diagnostic yield (96.1-97.4%) and relatively low morbidity and mortality. S

Endovascular restorative neurosurgery: a novel concept for molecular and cellular therapy of the nervous system.

The amalgam of molecular biology and neurosurgery offers immense promise for neurorestoration and the management of neurodegenerative deficiencies, developmental disorders, neoplasms, stroke, and trauma. This article summarizes present strategies for and impediments to gene therapy and stem cell therapy of the central nervous system and advances the concept of a potential new approach, namely endovascular restorative neurosurgery. The objectives of gene transfer to the central nervous system are efficient transfection of host cells, selective sustained expression of the transgene, and lack of

Radiologic guidelines in assessing children with intracranial tumors.

Computed tomography (CT) is currently the primary and generally the definitive imaging modality for the diagnosis, evaluation, and management of intracranial neoplasms in children. The value of CT in the postoperative period is discussed. The role of CT and myelography is stressed in the evaluation of cerebrospinal fluid seeding. CT is also important in demonstrating the iatrogenic disturbances of the central nervous system as a result of radiation and chemotherapy. The authors recommend that CT scans with and without contrast be obtained 2 weeks after surgery and before starting any form of a

Central nervous system germ cell tumors: an update.

PURPOSE OF REVIEW: Malignant germ cell tumors in the central nervous system are rare and not well known because they occur mainly in adolescents, an age in which patients are dispersed in adult and pediatric wards. Their biology starts to be unraveled by high-throughput genomics and their treatment is now well defined thanks to international studies, including patients from childhood through adulthood. RECENT FINDINGS: Chemotherapy gained definitively a role apart from radiotherapy in order to improve tumor control in secreting neoplasms, but also to decrease the volume or dose of radiation t

Ganglioglioma associated with cerebral cortical dysplasia: an unusual case with extensive leptomeningeal involvement.

Ganglioglioma is a tumor occurring in children and young adults and characterized by a superficial cortical location and biphasic histologic differentiation encompassing neuronal and glial elements. Ganglioglioma may arise anywhere throughout the neuraxis, including the optic nerve, brain stem, pineal gland, cerebellum, cerebrum, and spinal cord; however, the majority of glioneuronal neoplasms involve the temporal lobe. Gangliogliomas may show focal leptomeningeal involvement, but predominant leptomeningeal involvement by gangliogliomas is extremely rare; only 2 cases of ganglioglioma extensiv

Where and When to Cut? Fluorescein Guidance for Brain Stem and Spinal Cord Tumor Surgery-Technical Note.

BACKGROUND: Spinal cord and brain stem lesions require a judicious approach with an optimized trajectory due to a clustering of functions on their surfaces. Intraoperative mapping helps locate function. To confidently locate such lesions, neuronavigation alone lacks the desired accuracy and is of limited use in the spinal cord. OBJECTIVE: To evaluate the clinical value of fluoresceins for initial delineation of such critically located lesions. METHODS: We evaluated fluorescein guidance in the surgical resection of lesions with blood-brain barrier disruption demonstrating contrast enhancement

Coexistence of radiation-induced glioma and acute pontine infarct 40 years after radiotherapy for glioma: A case report.

We report an unusual case of radiation-induced glioma with a very long latent period. The patient had a history of brain stem glioma diagnosed 40 years earlier treated by radiotherapy. For treatment of radiation-induced glioma, radiotherapy was utilized again. Following therapy, the patient presented with an acute pontine infarct. To the best of our knowledge this may be the first report of radiation-induced glioma and radiation-induced stroke occurring within the same patient.

Gamma knife radiosurgery for a rare Rosette-forming glioneuronal tumor in the brainstem region: A case report and literature review.

RATIONALE: Rosette-forming glioneuronal tumor (RGNT) is a rare primary nervous system tumor, with limited treatment guidelines due to its rarity, especially in the brainstem. This report presents a unique case of brainstem RGNT treated with gamma knife radiosurgery (GKRS). PATIENT CONCERNS: A 35-year-old woman sought medical attention after sudden syncope and rapid decline in consciousness. Magnetic resonance imaging revealed a mass in the pineal region, extending to the brainstem and thalamus. Due to the critical location, only partial resection of the pineal tumor was possible, leaving most

Rapid Transformation of Myelodysplastic Neoplasm to Myelodysplasia-related Acute Myeloid Leukemia with Central Nervous System Leukemia in a Young Adult.

Myelodysplastic neoplasms (MDSs) are clonal hematopoietic stem cell disorders that progress to acute myeloid leukemia (AML) in 25%-30% of patients, typically over many years. Rapid transformation and central nervous system (CNS) involvement at AML diagnosis are uncommon in adults and confer a poor prognosis. We report a 35-year-old man with MDS who progressed to AML within 8 months and presented with extreme hyperleukocytosis, pulmonary infiltrates, leukemic retinopathy, and acute neuropsychiatric symptoms. Flow cytometry demonstrated nonmonocytic AML with aberrant CD7 expression and co-expres

Clinicopathological analysis of extraneural sporadic haemangioblastoma occurring in the tongue.

Haemangioblastoma is a morphologically distinctive tumour with uncertain histogenesis, typically occurring in the cerebellum, brain stem or spinal cord and less commonly in extraneural locations. Here, we present a case of haemangioblastoma occurring in the tongue, which is the first reported case in terms of the pathogenic site. The tumour was morphologically indistinguishable from central nervous system haemangioblastoma, that is, neoplastic stromal cells with cytoplasmic vacuolisation and abundant small vessels. Immunohistochemical studies revealed that the tumour cells were positive for S1

Total Resection of Clival Chordoma with Brainstem Invasion via Endoscopic Transnasal Approach: Technical Note.

OBJECTIVE: Clival chordomas with brainstem invasion are extremely rare; only 5 cases have been reported in the literature. In such cases total tumor resection is associated with high intraoperative risks and is a notable challenge for the neurosurgeon. However, it is the quality of surgical treatment that influences the future prognosis, despite the efficacy of proton therapy. METHODS: We describe the clinical case of a 39-year-old male patient with the sudden onset of double vision and left trochlear nerve paresis, who was diagnosed with a clival chordoma with brainstem invasion. RESULTS: T

Sonic Hedgehog Signaling in Organogenesis, Tumors, and Tumor Microenvironments.

During mammalian embryonic development, primary cilia transduce and regulate several signaling pathways. Among the various pathways, Sonic hedgehog (SHH) is one of the most significant. SHH signaling remains quiescent in adult mammalian tissues. However, in multiple adult tissues, it becomes active during differentiation, proliferation, and maintenance. Moreover, aberrant activation of SHH signaling occurs in cancers of the skin, brain, liver, gallbladder, pancreas, stomach, colon, breast, lung, prostate, and hematological malignancies. Recent studies have shown that the tumor microenvironment

A modified transcondylar screw to accommodate anatomical skull base variations.

BACKGROUND: Occipitocervical instability may be attributed to congenital, bony/ligamentous abnormalities, trauma, neoplasm, degenerative bone disease, and failed atlantoaxial fixation. Indications for occipitocervical fixation include the prevention of disabling pain, cranial nerve dysfunction, paralysis, or even sudden death. METHODS: The screw trajectory for the modified transcondylar screw (mTCS) is optimally planned utilizing a three-dimensional skull reconstructed image. RESULTS: The modified mTCS technique is helpful where there is a loss of bone, such as after prior suboccipital crani
